Message from the CEO
As we emerge from seven days of lockdown across the
state, I’d like to highlight the ongoing positive work
happening across country South Australia, by focusing on
one of the many great programs Country SA PHN is
funding.
The Community Resilience and Readiness program was
prompted by the recent experience of bushfires on
Kangaroo Island, in the Adelaide Hills, on Yorke Peninsula
and in the South-East.
The program aims to increase bushfire affected communities psychological and practical
preparedness for future events. It’s designed to support these communities increase their
resilience for future bushfires and other disasters. Country SA PHN recognises the challenges
these communities continue to face in the bushfire aftermath and the ongoing uncertainty of
COVID-19.
Funded by Country SA PHN, as part of the Australian Government mental health response to
bushfire trauma, the program is led by Community Resilience Officers (CROs) who are
assisting bushfire impacted community’s recovery by supporting their mental and emotional
preparedness.
The idea germinated when Adelaide Hills CRO, Miranda Hampton was working with the Cudlee
Creek, and other hills, communities following the recent devastating bushfires and identified
gaps in people’s practical and psychological preparedness and education.
Miranda was conscious that recovery services would have an end point and that the
community needed help beyond this to be proactive and develop resilience strategies for the
long term – rather than just recovery. Miranda and fellow CRO Sue Thomas could see the
potential benefits of building a resilient community – one that is emotionally and mentally
prepared to face future disasters - rather than just react to them. That it is a marathon – not
a sprint - and the more prepared the community is for whatever comes next, the better.
Country SA PHN’s Bushfire Response Coordinator, Sally Patten, agreed and collaborated with
Miranda to make the CRO role a reality. Country SA PHN is now funding seven CROs in four
recently affected bushfire communities in the Adelaide Hills, Kangaroo Island, Yorke Peninsula
and the South-East.
Kangaroo Island communities were severely affected by the devastating bushfires that tore
through the Island in January 2020. Kangaroo Island CRO Maree Baldwin says the bushfires
exacerbated mental health challenges already present on the Island. With the added trauma of
COVID-19, the community was restricted in meeting socially to support each other through
the crisis. As such, many Islanders are still struggling to move beyond the traumatic
experience.
Maree and fellow Kangaroo Island CRO Kate Brooksby say their particular focus is on
supporting families, strengthening social networks, improving well being and increasing
participation of people in community life to enhance family and community interactions. The
CROs recognise that bushfire recovery will take more than just rebuilding physical structureand that it is the social bonds that uphold the community that need to be repaired to ensure it
is prepared for future such events.
Yorke Peninsula communities experienced significant fires in 2019 at Yorketown, Price and
Maitland. While not as densely populated as some of the state’s other bushfire affected areas
Yorke Peninsula CROs Kate Martin and Katie Hughes were aware the fires highlighted many
real gaps in the community’s preparedness and exposed other well-being issues that were
bubbling beneath the surface.
Keilira, in the state’s South-East has also been severely impacted by bushfires that started on
December 30, 2019. South-East CRO, Kristen Wilks, has been concentrating on the critical role
in her community of developing and establishing good relationships and understanding what
particular assistance will help the Keilira community in their recovery and resilience building.
Country SA PHN recognises the ongoing importance of continuing the great work in supporting
these four recently bushfire affected individual communities and building on that momentum
in a capacity building role. Country SA PHN is very pleased to fund the CROs in such an
important initiative to assist South Australian bushfire affected communities to face inevitable
future challenges with resilience, preparedness and a sense of empowerment.
Supporting your patients with Chronic Pain
Did you know: One in five Australians will experience chronic pain in their lifetime and
in SA a GP focus group involved in a SA pain project estimated that 25% of their patients
experienced chronic pain. (SA Health & Model of Care for Chronic Pain Management in SA).
Chronic Pain within General Practice
The majority of chronic pain is managed in the primary care sector with General Practitioners
responsible for patient care. Managing patients can be complex, and often requires a
collaborative, multidisciplinary approach with the GP playing a central role within the
treatment team.

Free Cultural Awareness Online Training Available Now
Country SA PHN (CSAPHN) has purchased 120 subscriptions to the RACGP Cultural Awareness
Active Learning Module (ALM) for Non-Member GPs, general practice staff and health
professionals in country SA.
RACGP Members have access to the Active Learning Module via the RACGP GP Learning
Website.
The Introduction to Aboriginal and Torres Strait Islander Cultural Awareness in General
Practice aims to improve the health of Aboriginal and Torres Strait Islander peoples by
enhancing the cultural awareness of GPs, general practice staff, medical students, primary
health care staff, and other health care providers.
This 6 hour Active Learning Module aims to help participants to:
• extend their knowledge about Aboriginal and Torres Strait Islander history and culture,
• explore how attitudes and values can influence perceptions, assumptions and
behaviours in a clinical setting, and
• discuss key ways the practice team can be more culturally aware.
The module is accredited for 40 Category 1 QI & CPD points.
The value of the subscription for Non-Members is $179, however CSAPHN is offering this free
to health professionals working in country SA.

Use the link: https://videocall.direct/csaphn to join.Prescribing in Aged Care – what is changing In May, this year the Federal Government published its response to the Royal Commission on Aged Care: https://www.health.gov.au/resources/publications/australian-government-response-to-the- final-report-of-the-royal-commission-into-aged-care-quality-and-safety There were several recommendations in relation to medication safety all of which have been approved by the Government. You will see these recommendations being implemented in the Residential Aged Care Facilities (RACFs) you work with. There are two areas which will have an immediate impact upon medication safety in RACFs. Medication Reviews and Antipsychotic prescribing: The first area commenced on July 1, 2021 with additional Quality Indicators for the administration of medication, which are now mandatory. These indicators are for those who are prescribed nine or more mediations and those who are on antipsychotic medications. It will mean that you will be asked to review medication charts regularly. Changes (RC65) regarding GP’s capacity to prescribe antipsychotics are currently being reviewed and changes around the prescribing access could be in place by November 2021. This change will be supported further from January 2022 (RC64), with increased access to medication reviews by pharmacy. Electronic medication charts are necessary for RACFs to meet these indicators. As such, the Government will be supporting Aged Care providers to implement such systems. See: https://www.health.gov.au/initiatives-and-programs/national-aged-care-mandatory- quality-indicator-program-qi-program Electronic National Residential Medication Charts: The second impact is that RACFs will be implementing Electronic Medication Charts (RC56, RC67 and RC68). You will find that your use of them will be supported by changes in the (RC56 and RC61) Aged Care Access Incentive payment. Medication Charts software programs unlike RACF Care planning systems, are required to be conformant to Australian Medical Terminology and will also enable In Chart prescribing. Please note that using this system is for the overall benefit. It will ensure your prescribing is acted upon immediately, with the added benefit of being able to clearly see in real time what has or hasn’t occurred. Professional Development Workshop Ceduna GPEx and Country SA PHN were very pleased to deliver a full day Professional Development Team Workshop in Ceduna on Saturday 19 June 2021. This interactive day, which was open to all health care teams in Ceduna was very well received. Staff attended from Ceduna Family Medical Practice, Ceduna District Health Services and Yadu Health. Facilitators Dan Aubin from Daring Humans; Dr Simon Hay Deputy Director of Medical Education GPEx; and Dr Dan Wilson GP & Addiction specialist provided an interactive and in- depth session on staff wellbeing, communicating care and developing strategies for supporting and activating teams in times of change along with primary care approaches to reducing drug and alcohol harm in patients. Difficult Conversations Kangaroo Island Agriculture (auspiced by WoTL) received a Country SA PHN Bushfire Community Recovery Grant. One component of the grant was to hold Difficult Conversations sessions across the Island, with a focus on the farming and agriculture communities, but with all welcome. There’s was a great deal of interest in this event, with three sessions being run (the original application was for two). By spacing the event across the Island and at different times of the day – breakfast, lunch and evening sessions - there was great uptake by the community. A total of 84 participants attended across the three events – representing almost 25% of the farmers on Kangaroo Island. A diverse range of ages attended the sessions. This was helped by having one event hosted by a ‘younger’ farmer. Mary O’Brien was a dynamic speaker who related well to the audience. The sessions were very interactive with many attendees feeling comfortable to ask questions and there was a great ‘buzz’ in the room with many follow up conversations amongst the participants. It’s acknowledged that understanding and managing mental health issues will be an ongoing conversation for the Kangaroo Island community. However, this series of workshops was a wonderful way to begin the conversation in a non-threatening environment. Mary O’Brien helped to create a ‘common language’ (especially amongst male farmers.) This will help to not
only draw attention to the issue, but also to provide a base level of skill in the community to
know and recognise the signs to be aware of and how to begin that Difficult Conversation that
may well save a life.

Australian Immunisation Register Mandatory Reporting
Requirements
The Australian Government has released the following key updates for the mandatory
reporting requirements to report all National Immunisation Program (NIP) vaccines to the
Australian Immunisation Register AIR).
Mandatory reporting of vaccinations to the AIR commenced in February 2021 for:
o COVID-19 vaccines administered on or after 20 February 2021; and
o Influenza vaccinations administered on or after 1 March 2021
Further legislative changes mean that from 1 July 2021, vaccination providers are now
required to report all NIP vaccines administered to the AIR.
The AIR is a whole of life, national immunisation register, which records vaccines given to all
people in Australia. This includes COVID-19 vaccines, vaccines given under the NIP, and
privately, such as for seasonal influenza or travel.

Emergency Response Planning Tool (ERPT) Expression
of Interest
Country SA PHN is providing funding for general practices within country SA to access and use
the Emergency Response Planning Tool (ERPT).
What is the ERPT?The ERPT is a cloud-based tool which assists general practices in better preparing for,
responding to and recovering from the impacts of emergencies and pandemics.
The ERPT is managed by Healthpoint ANZ and was developed in collaboration with the Royal
Australian College of General Practitioners (RACGP).
How does it work?
The critical information entered into the ERPT is used to create an emergency response plan
which is individually tailored to your practice, providing a roadmap to recovering from
emergencies.
Maintaining an up-to-date emergency plan helps ensure the safety of your staff, the smooth
recovery of your practice and reduces the risk of financial loss.
To ensure business continuity, the ERPT provides suggested contingency measures for staff so
that, in the event of an emergency, they will know how to respond to:
• a pandemic
• loss of power supply to the practice
• disruption to water and gas supplies
• disruption to telecommunications systems
• loss of IT systems or data
• loss of business records
• complete or partial loss of practice premises
• loss of medical supplies, equipment and furniture
• loss or non–availability of key staff
• COVID-19 safety plan

5th Map RACGP Accreditation Resource
Country SA PHN is pleased to advise that we are funding the 5th Map RACGP accepted
accreditation resource for general practices within country SA.
104 general practices are now utilising the Accreditation Tool however there are now limited
resources to offer. Please provide and Expression of Interest for your practice via email
to support@countrysaphn.com.au.
What is the 5th Map?
• The 5th map is a large [1.2 metre x 0.9 metre / A0 size] one-page infographic,
custom printed with the practice name summarising RACGP’s 184 page Standards for
general practices, 5th Edition Standards.
• The 5th map is a real map, analogue, tactile and designed to be on a wall such as in
your lunch/meeting room or practice managers office - to be seen every day by the
practice manager and partners, doctors, nurses, admin/reception and allied health.
• The 5th map infographic tightly summarises into single lines all Modules, Standards,
Criterions and Indicators into what a practice needs to get done.
• A bonus 5th map checklists companion map displays fourteen indicators which refer
to lists of specific items that need to be attended.• Both maps and all text within, have been reviewed and exclusively licensed to 5th
map as an RACGP accepted tool assisting practices to prepare and manage pre-
accreditation activities.
• The 5th map has a structured ‘flow’ of information and space is provided along the

Chronic Disease Management and Care Planning
Education for CVC Program Nurses
APNA is working in conjunction with the Department of Veterans' Affairs (DVA) to provide
practice nurses, Aboriginal and/or Torres Strait Islander Primary Health Workers and
community nurses (RN or EN who is employed by a DVA contracted Community Nursing
provider to provide community nursing services) with free education to help them deliver the
best possible care to their patients enrolled in the Coordinated Veterans' Care (CVC) Program.
Practice or community nurses and Aboriginal and/or Torres Strait Islander Primary Health
Workers who have patients enrolled in the CVC Program can now access the fully-funded
education specifically on chronic disease management and care planning.
The education comprises of two online learning modules:
• Chronic disease management (2hrs) – Providing an understanding of chronic disease
management in Australia and how nurses and Aboriginal and Torres Strait Islander
Primary Health Workers can play a vital role.
• Care planning (3hrs) - A sequential ‘how to’ series for nurses and Aboriginal and
Torres Strait Islander Primary Health Workers on how a comprehensive chronicdisease management health care plan may be conducted within the primary health
care setting.
Practice nurses are the leaders of chronic disease management in primary care. By growing
their understanding and scope of practice in the primary care space, these courses help them
best support veterans’ self-management, goals of care and coordinate an effective team
approach.
Enrolees can do these interactive modules at their own pace online with certificates of
completion provided and optional reflection templates for their continuing professional

New online tool for GP’s promotes early referral
to palliative care services for people with motor
neurone disease
Improving palliative care for people with motor neurone disease (MND)
MND SA welcomed funding from the South Australian Department of Health and
Wellbeing in 2020 to develop an MNDSA clinical pathway and palliative care referral
network for people with Motor Neurone Disease.
People with Motor Neurone Disease may have needs that are complex to manage.
www.mndsapathway.com.au is a new online tool to assist medical professionals navigate
these needs, quickly and easily by providing MND specific information, evidence and
resources in an organised format that follows the patient journey from presentation of first
symptoms to bereavement.
The tool sets out the processes for access to specialist care providers and other supports
including the MND Association of South Australia, the MND Clinic and palliative care
providers and contains referral forms and other quick links to the network of providers, you
will ultimately need in addressing and managing symptoms.How2 Program LGBTIQ Workplace Inclusion The HOW2 Create LGBTI Inclusive Services program can provide your organisation with a practical way to work towards your goals of increasing inclusivity and celebrating diversity. It is more than a tick box activity; this program will support your organisation to undertake a culture change and develop services and programs that are genuinely inclusive. Palliative Care short course
Flinders University is developing a short online course in palliative care open to students who
hold a degree with or without experience in palliative care.
The students will complete a foundation topic and then choose to continue in one of four
streams:
• aged care
• allied health
• disability
• nursing
In recent years, general practitioners have studied in the postgraduate program in palliative
care and the shorter format of this proposed course could have great appeal to GPs.
We are looking for GPs to provide advice as we develop the course – GPs with or without
extensive experience in palliative care, those who sporadically or regularly look after people
with a life-limiting illness or older people who are approaching the end of their life, those who
sporadically or regularly work in residential aged care. Ideally, we would like input from GPs
based in and outside of South Australia.Please do not hesitate to pass this information on to colleagues who may be interested in being involved.
