Eligibility

 HEA 1008 states that a school corporation or charter school may receive a grant to implement:

 (1) The System for Teacher and Student Advancement (TAP) teacher performance model program, or

 (2) The Opportunity Culture teacher performance model, or

 (3) A model teacher performance program approved by a national school employee organization, or

 (4) A teacher performance model program that includes the implementation of all the elements itemized in this grant
 application.

Potential Allocations

 This competitive grant provides $3.5 million to fund up to 30 geographically diverse school
 corporations or charter schools. The base allocation for all LEA’s is $50,000.

 ●   Part 1: Describe the planning process used by the district to develop your
     teacher performance model program (TPMP). Please include the following
     information in your narrative:
      ○ A description of the comprehensive and competitive pay progression for all
          eligible effective and highly effective teacher leaders.
      ○ A description of how each stakeholder group (teachers, administrators, and
          parents) engaged in the process and contributed to the development of the plan.
      ○ A description of the support for and commitment to the plan from each
          stakeholder group (teachers, administrators, and parents).

 ●   Part 2:
      ○ Describe the school district’s vision and goals for its TPMP. In your description,
         please explain the local context (including relevant student achievement data
         and existing goals) and how the plan will be tailored to that context.
 ●   Part 3:
      ○ Describe how the TPMP will connect to, support, and strengthen the district’s key
         school improvement initiatives and, ultimately, positively affect student
         achievement.

 ●   Part 4:
      ○ Describe how the TPMP will utilize teacher leaders and the additional funding to
         improve entry into the teaching profession for new teachers by incentivizing
         mentoring and coaching, reducing teacher load or providing release time,
         reviewing portfolios, and student performance. Include in your response an
         analysis of the effectiveness of the current induction and mentoring program, areas
         of improvement needed in the current program and how your TPMP will address
         these gaps.

 ●   Part 5: Describe each of the proposed teacher leadership roles in your plan.
     Please include the following information in your narrative:
      ○ Please provide a description of the program structure, including the process for
          teacher advancement based on performance, professional growth, or the specific
          teacher roles assumed by the teacher. Include all preliminary steps to
          implementation such as special training, designation of personnel, and purchases.
          Provide a schedule of activities and the dates when each will be accomplished.

 ●   Part 5 (continued):
      ○ A description of the responsibilities and duties for each leadership role, the
         percentage of time each role will spend engaged in student instruction, and the
         percentage of time each role will spend performing teacher leader duties.
      ○ A description of how each of the roles fit together to create a coherent
         instructional improvement strategy that will strengthen instruction and
         improve student learning and student achievement throughout the district.

      ○ Describe how teacher leaders will be selected. Please include descriptions of
         how the district will determine and evaluate the following in selecting teacher
         leaders:
          ■ Measures of effectiveness
          ■ Professional growth
          ■ Rigorous, transparent, and detailed advancement criteria locally developed by
              multiple stakeholders

      ○ Describe how the TPMP will utilize teacher leaders to improve the district’s current
         professional development program. Please include a description of the role
         teacher leaders will play in the creation and delivery of professional
         development.

          ■ A description of how the district will determine the impact/effectiveness
             of the TPMP, including short-term and the long-term measures.
