As healthcare costs continue to rise nationwide, we remain committed to providing comprehensive yet affordable plans, and have worked hard to modify our offerings to minimize the cost being passed onto you. For the 2020 plan year, you’ll see a slight premium adjustment increase across all of the medical plans, but our dental & vision premiums will not have any increase. You’ll see other changes for 2020 in this guide including: y y HDHP1400 plan – The IRS regulates what is considered a high deductible health plan (HDHP) and eligible to be paired with an HSA. The IRS has increased the deductible amount from $1350 to $1400 y y Tobacco user rates will now be applied if either the employee or the covered spouse is a tobacco user y y New Legal plan through Hyatt Legal We know that benefits are an incredibly important part of your overall employment package.
Our plans are designed to give you a variety of options, and our tools aim to make enrollment easy and efficient. As a reminder, we’re continuing to offer several benefits you’ve told us are very important to you. HealthJoy remains in place for easy centralized benefit information and terrific telehealth offerings, Advance Medical will continue to be available for second opinion medical help, and Costco’s heavily discounted Prescription services remain as our primary Rx source. We are proud to continue to offer insurance coverage that embodies flexibility, allowing you to customize your benefit plans to best fit your needs. This guide includes plan descriptions, pricing, and contact information to support you in being able to make the best decision for your insurance coverage for the upcoming year. As an additional resource, we are continuing our relationship with CBC for 2020, and they will be available to assist with answers to questions about enrollment, plans details, providers, and more.
CBC will also be available throughout the year to support you with any inquiries you may have about your coverage such as benefit ID card replacements, HSA balance changes, claims, and more.
4 www.MyAllegiantBenefits.com | (855) 676-6860 ENROLL IN YOUR BENEFITS IN THREE EASY STEPS! 1. Know your options REVIEW ALL PLAN OPTIONS AND COSTS. Finding the right coverage for you and your family is a balance between the amount deducted from your paycheck and the amount you pay at time of service. Choose carefully: The benefits you select will generally begin the first of the month following completion of your eligibility waiting period and remain in effect through December 31, 2020. If you have elected a benefit that requires Evidence of Insurability, that benefit will be effective the first of the month following approval from the carrier.
During the year, you can only change benefits, including adding or removing a dependent, if you have a Qualified Life Event such as birth, adoption, marriage, or divorce. These events are defined by the IRS or the plan. If you have a Qualified Life Event during the 2020 Benefits Open Enrollment period, you must report the event separately within 31 days of the event. 2. Decide who to cover YOU CAN ENROLL YOURSELF, AS WELL AS: y y Your spouse y y Your children under age 26, if they are your natural children, stepchildren, legally adopted children, foster children, or children for whom you have legal custody or court-appointed Guardianship.
y y Your children of any age, if they are physically or mentally unable to care for themselves as long as they were disabled before age 26. 3. Make your elections through the Allegiant Benefits Center ENROLL ONLINE! y y Log onto www.MyAllegiantBenefits.com y y To login to the Benefits Center click the employee login button on the landing page to be sent to a secure login portal. There you will use your corporate network login. Afterwards you will then be redirected to your Benefits Portal homepage. y y Click on “Enroll Now” and follow each step.

y y Once you’ve made your choices, take a close look to be sure you’ve selected the benefits you want before you submit your elections. y y Print a copy of your elections as your confirmation statement. Please check your Confirmation of Election Statement carefully before exiting the online benefits center. If you see a problem or want to make a change, simply go back to the “Enroll Now” benefits section and modify your election. When you have completed your elections, print your Confirmation of Election Statement.

As summaries, these descriptions only highlight the main characteristics of each plan. You can find more detailed descriptions of the plans in your Summary Plan Descriptions and in the materials supplied by the providers. Regardless of any statement made in this guide, benefits will be provided in accordance with the plan documents, contracts and trust agreements between Allegiant and the various provider organizations.
6 www.MyAllegiantBenefits.com | (855) 676-6860 The Allegiant Benefits Plan: A Self-Funded Model Medical insurance is complicated and risky – and its overall cost is often very difficult to predict. Employers can choose a more costly, but lower-risk design, called “fully-funded,” or a less expensive, higher-risk one, called “self-funded.” It is important you understand our model and how your choices can impact the plan costs, so we’ve outlined here a basic primer on insurance plan design. In a “fully-funded” plan design, an insurance carrier, like Anthem, collects monthly premiums to pay for medical bills.
Those premiums are partially paid by you from your paychecks and partially paid from Allegiant. That’s all the money Anthem collects to pay all the bills, so they need to charge enough to cover what they expect all the bills to be, plus a fee for doing all the bill payment work, plus a cushion for taking the risk to offset the years they don’t collect enough money to pay all the bills.
In those years, Anthem is stuck paying out more than they collected, so the following year, they raise the cost for “fully funded” insurance by 5, 10 or 15% as we’ve seen happen multiple times to Allegiant over the past decade. In a “self-funded” plan design, the insurance carrier, like Anthem, is simply allowing employees to use its network of providers at the discounted rates it has negotiated, and processing the bills, but not paying them. There is a dramatically smaller monthly fee Anthem collects for these services, but it passes all the bills to the Company to pay. The Company puts the premiums we collect from employees plus the premiums Allegiant pays, just like in the fully-funded model, into a bank account used to pay for all the medical bills received.
We save the amount of money Anthem would have taken as a fee and the cushion amount they would tack on to the premiums to ensure they collected enough to cover the bills, if the plan does not run as expected. If the bills are really, really high, though, this is where the riskiness comes in, because Allegiant has to pay them all, whereas if we were fully funded, Anthem would have to pay them. Here’s what this means for each of us: Since we’ve opted for the self-funded model, each medical decision we make is charged directly to Allegiant. Choose Urgent Care instead of Emergency Room = plan savings.
Choose your doctor to draw blood instead of asking to be sent to Quest labs = extra plan costs. Call telemedicine for your sore throat instead of going to your primary care doctor = plan savings. At the end of the year, next year’s paycheck premiums are based on this year’s actual spending – so every one of us, and every medical decision we make, can influence our future costs. Hopefully this explains the type of plan we offer, and how all of us on Allegiant’s benefit plan have ownership in it. This is also why we offer so many resources like AdvanceMedical and HealthJoy to keep your costs and Allegiant’s costs as low as possible.

Each plan presents a unique combination of coverage and costs. We will be using the Anthem / Blue Cross Blue Shield network of providers. y y HDHP 3500 (a High Deductible Health Plan) - HSA Eligible plan y y HDHP 1400 (a High Deductible Health Plan) - HSA Eligible plan HDHP (High Deductible Health Plan) Both the HDHP 3500 and HDHP 1400 plans are High Deductible Health Plans.
Preventive care costs are covered 100%. Team Members can pay for qualified medical expenses with pre-tax dollars from their Health Savings Account (HSA). With an HSA, dollars contributed roll over from year to year. If you leave the Company, the account, along with all the funds in it, goes with you. Both Team Members and Allegiant can make contributions to an HSA. Allegiant contributes to your HSA by matching any contributions you make, dollar for dollar, up to the following amounts: ANNUAL MAXIMUM MATCH* y y Team Member Only $700 y y Team Member Plus $1,400 If annual salary $40,000, match is $1 to $1.
The IRS annual limit maximum contribution for the 2020 plan year is $3,550 for those with Team Member Only coverage and $7,100 for Team Member plus Dependent(s). The annual limits include both the Team Member and employer contributions. Team Members who contribute to an HSA, are 55 or older and who are not enrolled in Medicare are allowed to contribute an additional $1,000. PPO - FSA Eligible Plan With the PPO plan you have access to both in-network and out-of-network health care providers. Your portion of the costs are less when you use in-network providers. There is no Health Savings Account (HSA) with this plan.
With this plan you may enroll in a Flexible Spending Account (FSA). However, there is no company match and funds do not roll over to the next year. All contributions need to be used within the year. Any ‘left-over’ funds are forfeited. This is often referred to as a ‘use-it-or-lose-it’ account. EPO - FSA Eligible Plan With the EPO plan you have access to certain health care providers within a limited network. Unlike other insurance plan types, care is covered only if you see a provider within the EPO network. You will pay a set co-pay when you seek health care.

PRESCRIPTIONS Costco Mail Order Pharmacy Preferred maintenance medications may be obtained through the Costco mail order pharmacy. If you are on a medical plan, you will obtain a 90 day supply for less than at a retail pharmacy. WHAT ARE MAINTENANCE MEDICATIONS? These are medications taken regularly to maintain your health, including treatment for high blood pressure, asthma, diabetes, and additional medical conditions. WHY ARE MAINTENANCE MEDICATIONS AVAILABLE AT COSTCO PHARMACIES? Costco strives to be the low cost leader in the market place for prescription drugs, which means Costco is able to pass along substantial savings to you and your benefit plan.

Generally, a life event is a situation in which your family size or your availability for other insurance coverage changes. A change in your life can make you eligible for a Special Enrollment Period to enroll in health care coverage through Allegiant outside of the Open Enrollment period. EXAMPLES OF A LIFE EVENT INCLUDE: y y Marriage or divorce y y Birth, adoption, or death of a dependent y y Loss of other insurance coverage (i.e.
your spouse loses their job-based coverage.) y y Gain of other insurance coverage WHAT IS THE TIME LIMIT TO MAKE BENEFIT CHANGES AFTER A ‘LIFE EVENT’? You must notify the Allegiant Benefits Center within 31 days of the date of the life event, even if you don’t have the required documentation.
WHAT STEPS DO I NEED TO TAKE IF I EXPERIENCE A ‘LIFE EVENT’? y y Notify the Allegiant Benefits Center within 31 days of the date of the Life Event. y y Provide required documentation necessary as evidence of the event. See the Benefits Center or G4Connect for examples of required documentation. y y Complete a life event enrollment in the Benefit Center within that 31 day period. WHAT HAPPENS AFTER I SUBMIT MY LIFE EVENT CHANGES? Your life event, documentation and benefit elections will be reviewed by the Allegiant Benefits Center. If approved, you will receive a Benefit Confirmation Statement from the Benefits Center.
Your payroll deductions will be updated and you will be reimbursed or owe for any missed premiums dating back to the original date of the life event. WILL I RECEIVE UPDATED INSURANCE CARDS?
Yes, within 14 business days of your changes being approved you will receive updated insurance cards from the benefit vendors if needed based on the changes made. WHAT HAPPENS IF I MISS THE 31 DAY WINDOW TO MAKE CHANGES? If you miss the 31 day window, you will not be able to make changes until the next open enrollment period. Open enrollment is held in the fourth quarter of the calendar year with an insurance effective date of January 1. LIFE EVENT

The contributions go into the 401(k) account and you choose the investments based on options provided under the plan. Allegiant provides a match to your contributions. AmeriBen The company that processes and pays your medical claims. Blue Cross Blue Shield (BCBS) Network of medical and facility providers for which we are contracted with.
Beneficiary A person or group of persons, designated as the recipient of the benefits of your insurance policy. Custom Benefits Consultants (CBC) The benefits administration company that can answer questions about claims, benefits, and so much more. They are available to answer questions year round as well as during open enrollment. Deductible The amount you must pay before an insurance company will make a payment. Employee Assistance Program (EAP) A company-paid benefit which provides support and/or counseling for entire family through many types of personal and professional issues.
FSA Dependent Care (FSA) A Dependent Care Flexible Spending Account lets you use tax-exempt funds to pay for childcare expenses you incur while at work.
You can also use FSAs to cover adult daycare expenses for elderly family members who live in your home. All contributions need to be used within the year. Any “unused” funds are forfeited. Health Savings Account (HSA) A medical savings account made available to you when you enroll in a high-deductible health plan (HDHP). Any funds you contribute to your HSA are not subject to federal income tax. Any unused balance can be rolled over at the end of the year. High Deductible Health Plan (HDHP) A health insurance plan with lower premiums and higher deductibles than a traditional health plan. Being covered by an HDHP is also a requirement for having a Health Savings Account (HSA). Premium The amount deducted from each paycheck to pay the employee portion of the cost for each of the benefits you have selected.
Per Paycheck Premiums will be deducted from the first two paychecks of each month.
Safe Harbor A Safe Harbor 401(k) plan is similar to a traditional 401(k) plan, but among other things, it requires the company to match your contributions up to a certain amount and the contributions must be 100% fully vested when made. The Company’s matching contributions may be limited to employees who participate in the plan.
