• Out-of-network (OON) testing, if medically appropriate for individual as
  determined by their doctor.
• These rules will be in place until the end of the COVID-19 public health
emergency (or longer if Plan Administrator so determines).

part of public health safety guidelines relating to work.

Social Security Number - Dependents
The bswift platform has been updated to require an SSN be entered for
spouses and for dependents over age one. Here’s why:
• This is a requirement by the Centers for Medicare & Medicaid Services
(CMS) and the Affordable Care Act (ACA).
• This is also necessary for proper plan administration in coordination of
benefits, subrogation, and in other areas.
The details are outlined in the Summary Plan Document (SPD) in the
eligibility section. You can find the 2021 SPD here.
If needed, there is an annual opt-out form and an annual acknowledgment
form that you can find here.

                                                                             SD
Two Factor Authentication

Two-factor authentication (2FA) is live. We
encourage you to remind your employees to
configure this on their Ascend to Wholeness
account to increase their account security.
