13th ANNUAL MUD VOLLEYBALL TOURNAMENT
Empower students to succeed!
The 13th Annual Communities In Schools Peace.Love.Mud Volleyball Tournament will be held
on Saturday, June 5, 2021 at 2614 FM 1585 (130th and University) in Lubbock, Texas. The intent of
the Peace.Love.Mud Volleyball Tournament (MVB) is to have some muddy fun in the sun while
raising funds to combat student dropout rates. In order to guarantee the success of this very
important fundraiser, we are seeking community partners to sponsor the event. All funds raised by
Communities In Schools of the South Plains go directly to providing assistance to at-risk students in
our community.
The mission of Communities In Schools is to surround students with a community of support,
empowering them to stay in school and achieve in life. The objective is to prevent kids from
dropping out of school by giving them the tools they need to be successful. These tools include
evidence-based programs and services that address the academic, non-academic, cognitive, and
social and emotional factors that directly impact students’ ability to persist and be successful not
only in school but throughout their lives. The difference Communities In Schools of the South Plains’
services make in the lives of students and families is well-documented and plays out in 21 districts
and on more than 82 campuses in the Lubbock Independent School District (LISD), the greater
Lubbock area (Frenship, Lubbock-Cooper, Slaton, Shallowater ISD), and throughout the South
Plains (including districts as far away as Dimmitt, and Plainview).
Community Partners are the key to our success and make it possible for Communities In Schools of
the South Plains to provide at-risk students and their families with assistance and critical services.
We hope that we can count on your support to help us to continue to empower students to succeed.

PART ONE
This is a legally binding release, waiver, discharge and covenant not to sue Communities In Schools
of the South Plains (AGENCY), Inc. made by me, the undersigning, to the AGENCY, its employees,
volunteers, sponsors, or other event participants. I fully recognize there are dangers and risks to
which you may be exposed by participating in the AGENCY'S PEACE LOVE MUD Volleyball
Tournament, which involves physical activity and/or including, but not limited to, other similar activity.
Release of Liability & Acknowledgement of Risk
With full understanding of the risks involved and despite this Release, I am voluntarily and willingly
participating in the AGENCY'S PEACE LOVE MUD Volleyball Tournament.
I therefore agree to assume and take upon myself all of the risks and responsibilities in any way
associated with this event. In consideration of and in return for services, facilities, and other
assistance provided to me by the AGENCY and other assistance proved by its employees,
volunteers, sponsors, or other event participants from any and all liability, claims and actions that
may arise from injury or harm from me, from death or from damage to my property in connection with
this event.
I understand that this Release covers liability, claims and actions caused entirely or in part by any acts
or failure to act by the AGENCY, its employees, volunteers, sponsors, or other event participants,
including but not limited to negligence, mistake or failure to supervise.
I recognize that this Release means I am giving up, among other things, my right to sue the
AGENCY, its employees, volunteers, sponsors, or other event participants for injuries, damages or
losses I may incur while participating in this event. I also understand that this Release bind my heirs,
executors, administrators, and assigns, as well as myself.
I AGREE NOT TO SUE nor bring any type of lawsuit against any persons or entities mentioned
above for any of the claims or liabilities that I have waived, released or discharged herein; and
I INDEMNIFY AND HOLD HARMLESS the persons or entities mentioned above from any claims
made or liabilities assessed against them as a result of my actions. I hereby affirm that I am eighteen
(18) years of age or older and I have read this document and I understand its contents. I understand
that I have given up substantial rights by signing this document and sign it voluntarily.

or health care facility to treat the minor named herein for the purpose of attempting to treat or relieve
any injuries received by said minor arising or relating to the sporting event. I authorize any such
Medical Provider to perform all procedures deemed medical advisable. I realize and appreciate that
there is a possibility of complications and unforeseen consequences in any medical treatment, and I
assume any such risk for and on behalf of myself and said minor.
