       2021 Iberia Medical Center Open Enrollment
     November 1st to November 30th

     Please read your Open Enrollment booklet very carefully.

   When enrolling in the Employee Self‐Service Portal, be sure to place your
    initials in the box on the last page and click save.
   This will be a “Passive Enrollment”. If you do not take action to make
    changes, your coverages will continue as they are. Premiums may change
    due to age or salary changes.

     Insurance representatives will be here on November 10th and November 13th to assist
     you in changing or enrolling in benefits. Due to Covid, the sessions will be limited to
     the following dates and times. Social Distancing will be followed.

Parochial Employees’ Retirement System of Louisiana
There is no better retirement than the Parochial Employees’ Retirement System (PERS) and, as a benefit
employee of IMC, you will have the privilege of participating in that system.

PERS is a public defined benefit pension plan. The employee is vested after just 7 years, qualifying the individual
for a lifetime pension payment (upon reaching the appropriate age).

Please see the “Principal Features Booklet” or check with an HR representative for further information.

IMC contributes 12.25% in addition to the 9.5% employee contribution on a pre‐tax basis. Employee receives
3% per year of average highest consecutive annual salary (3‐year average for employees hired prior to 1/1/07
and 5‐year average for those hired after 1/1/07) upon retirement up to 100%.

Full and Part time employees participate and are eligible immediately (mandatory).

The Parochial Employees Retirement System is a public defined benefit pension plan, established by the
Louisiana Legislature and made effective January 1, 1953.

WHO IS ELIGIBLE?
If you are an Iberia Medical Center full‐time employee (working 36 or more hours per week), a part‐time
employee (working 28 hours or more per week), or a Retiree not yet eligible for Medicare, you are eligible to
enroll in the benefits described in this guide. The following family members are eligible for medical, dental and
vision coverage: Spouses and dependent Children up to age 26. Children include natural children, stepchildren,
foster children, adopted children, children placed in the home for adoption and children for whom the
employee is a legal guardian. Spouses who have employment related coverage offered to them are not eligible.

HOW TO ENROLL
Are you ready to enroll? The first step is to review your current benefits. Did you move recently or get married?
Verify all of your personal information and make any necessary changes. Once all your information is up to date,
it’s time to make your benefit elections. The decisions you make during open enrollment can have a significant
impact on your life and finances, so it is important to weigh your options carefully.

You can enroll online (on Netchex) for most benefits this year. The exceptions are:

      Cancer Insurance ‐ you must Call NetEnroll or visit the NetEnroll representative during Benefits Week

      Pet Insurance must be done over the phone or online. Call 877‐738‐1874 ‐ YOU MUST TELL THEM YOU

WHEN TO ENROLL
      Open enrollment begins on November 1st and runs to November 30th. The benefits you choose during
       open enrollment will become effective on January 1, 2021.

HOW TO MAKE CHANGES
Unless you experience a life‐changing qualifying event, you cannot make changes to your benefits until the next
open enrollment period. Qualifying events must be reported within 30 days. Qualifying events include things like:

      Marriage, divorce or legal separation
      Birth or adoption of a child
      Change in child’s dependent status
      Death of a spouse, child or other qualified dependent
      Change in residence
      Change in employment status or a change in coverage under another employer‐sponsored plan

             DENTAL INSURANCE (No Change)
In addition to protecting your smile, dental insurance helps pay for dental care and usually includes regular
checkups, cleanings and X‐rays. Several studies suggest that oral diseases, such as periodontitis (gum disease),
can affect other areas of your body—including your heart. Receiving regular dental care can protect you and
your family from the high cost of dental disease and surgery.

We’re happy to say that there are no cost changes to your dental benefits for 2021. The following chart outlines
the dental benefits we offer.

 LONG TERM DISABILITY INCOME BENEFITS
         (RELIANCE STANDARD)
Iberia Medical Center provides full‐time employees with long‐term disability income benefits. Without disability
coverage, you and your family may struggle to get by if you miss work due to an injury or illness.

At Iberia Medical Center, we want to do everything we can to protect you and your family. That’s why Iberia
Medical Center pays for the full cost of long‐term disability insurance—meaning that you owe nothing out of
pocket. This coverage is provided by Reliance Standard Life Insurance Company.

In the event that you become disabled from a non‐work‐related injury or sickness, disability income benefits will
provide a partial replacement of lost income. The waiting period for this benefit is the first of the month
following 6 months of continuous employment.

                           BASIC LIFE INSURANCE
                           (RELIANCE STANDARD)
Iberia Medical Center provides full‐time employees group life and accidental death and dismemberment
(AD&D) insurance and pays the full cost of this benefit. This coverage is insured through Reliance Standard
Insurance Company. Job class determines the amount of coverage with the minimum initial amount being
$12,500 for active employees. There are reductions at age 65, age 70 and at age 75. This coverage is provided
by Reliance Standard Insurance Company. The waiting period for this benefit is the time from hire to the first of
the month after 90 days of continuous employment.

                   HEALTH SAVINGS ACCOUNTS
Health savings accounts (HSAs) are a great way to save money and budget for qualified medical expenses. HSAs
are tax‐advantaged savings accounts that accompany high deductible health plans (HDHPs). HDHPs offer lower
monthly premiums in exchange for a higher deductible (the amount you pay before insurance kicks in).

                WHAT ARE THE BENEFITS OF AN HSA?
There are many benefits of using an HSA, including the following:

      It saves you money. HDHPs have lower monthly premiums, meaning less money is being taken out of your
       paycheck.
      It is portable. The money in your HSA is carried over from year to year and is yours to keep, even if you
       leave the company.
      It is a tax‐saver. HSA contributions are made with pre‐tax dollars. Since your taxable income is decreased
       by your contributions, you’ll pay less in taxes.

The maximum amount that you can contribute to an HSA in 2021 is $3,600 for individual coverage and $7,200
for family coverage.

Additionally, if you are age 55 or older, you may make an additional “catch‐up” contribution of $1,000. You may
change your contribution amount at any time throughout the year as long as you don’t exceed the annual
maximum.

                   VOLUNTARY LIFE INSURANCE
                     (RELIANCE STANDARD)

Employees who want to supplement their group life insurance benefits may purchase additional coverage.
When you enroll yourself and/or your dependents in this benefit, you pay the full cost through bi‐weekly payroll
deductions. You can purchase coverage on yourself in $10,000 increments. Minimum coverage is $10,000 and
maximum coverage is $190,000 (GI). Coverage on your spouse is in $5,000 increments up to $50,000 (GI).
Coverage on children is in $2,000 increments up to $10,000. This coverage is insured through Reliance Standard
Life Insurance Company. Amounts above Guaranteed Issue (GI) can be applied for with medical questions and
full underwriting. It may or may not be approved depending on health.

Premiums depend on your age and amount of insurance requested. The waiting period for this benefit is the
first of the month after 90 days of continuous employment.

The Internal Revenue Service will allow you to pay for health, dental, and vision insurance premiums before taxes,
thus resulting in a TAX SAVINGS to you!

   When those medical emergencies occur, people are suddenly faced with lengthy medical treatment, drastic

   will help cover the costs of you or your dependents treatment.

     PTO Time accrues according to status and hours paid each pay period. If hours worked for Full Time
      Employees total less than 72, or for Part Time Employees, less than 56 for a pay period, PTO will accrue
      according to a proportionate amount per hour for hours actually worked.
