Assessing and managing risk of an individual with a mental illness is pivotal to mental health rehabilitation
and recovery. For each individual, a daily risk assessment is completed, which enables dynamic responses
to individual presentation within any 24 hour period. Through this process we provide integrated care to
make informed choices over positive risk-taking.
A further layer is added with robust management plans correlating to presenting risk, so that supportive
and therapeutic strategies can be implemented based on presenting need. The daily risk assessment
permeates all our mental health rehabilitation and recovery services providing safe and effective care
that is proactive to longer term success.

services. The tool allows us to measure an individual’s progress throughout their placement, plot the
positive developments, setbacks and changes in wellbeing but also identify the warning signs of
any possible deterioration, whilst informing care planning and being transparent about future risks.
