relies on service level agreements for a number of specialist services such as CAMHS,
psychiatry support, learning disability psychiatry and so on.

Over recent years NHS Orkney has seen significant changes in the delivery of healthcare.
The pace of change continues to accelerate as our committed staff adopt new and
innovative ways of working fit for the coming decade.

The successful candidate in this post will have a key role in the assessment, care planning
and treatment of patients with mental disorder, and a close and central working relationship
with the multi disciplinary community mental health team and key primary and secondary
care colleagues. Our mental health inpatient services are provided primarily by NHS
Grampian and the role will also include close liaison with services out with Orkney.

 Our new hospital is planned for 2018 and we work towards the provision of and investment
in our outer island facilities, these developments allow us to drive forward service
improvement with confidence.

Profile of Orkney

Lying off the northern coast of Scotland, between John O‟Groats and the Shetland
Isles, Orkney is an archipelago of over 70 beautiful islands, 17 of which are
inhabited. The total population is approximately 22,000 with most people living on
the Mainland. Kirkwall, the capital with its spectacular red sandstone 12th century
cathedral, with a population of 7,500, it is the administrative centre of Orkney with a
good mix of shops, supermarkets and businesses. Orkney is a wonderful place to
live and work and voted the happiest place to live in the UK. Orkney also offers; low
pollution, low crime, excellent schools, good leisure facilities, unique wildlife and
amazing scenery.

Orkney‟s economy is mainly occupied by the farming industry. Within Orkney Most
farmers breed and rear beef cattle of the highest standard, although dairy cows and
sheep are also kept. Agriculture is the main industry of the islands and generates
some £30 million per year, followed closely by tourism and oil. Fishermen compete
with seals and sea birds to enjoy something of the rich bounty provided by the
surrounding seas. Orkney is at the forefront of the renewable energy drive in the
UK. Wind turbines are dotted around the islands and wave energy research is on-
going.

In economical terms, Orkney hold an impeccable rate of those in
employment/seeking. With the percentage of 84.7, the islands statistics are above
the average for Scotland and Great Britain. Thus, unemployment levels are on
average lower than Scotland and the UK, in relation to long-term unemployment.

  NHS Orkney is looking to recruit a Consultant Psychiatrist to join
               our team here in the Orkney Islands.

on the West Mainland. The excellent ferry and air connections makes for ease of travel to the

The post holder will be working with a very experienced, skilled and versatile integrated
Community Mental Health Team. The team comprises a number of services: substance
misuse, older adult mental health, psychological therapy and generic mental health. There is
also a CAMHS service supported by visiting CAMHS psychiatrist and psychologist input
provided by NHS Grampian.

We have strong links with NHS Shetland Mental Health Services and the post holder will
form part of a robust three-way partnership with Shetland and Grampian to ensure that
there is the necessary peer/clinical support and supervision.

The link with Grampian will also incorporate acute mental health bed responsibility, which
means that the post holder will be the Responsible Medical Officer (RMO) for Orkney
patients whilst they are in Royal Cornhill Hospital

This is an exciting time in Orkney, we have just reviewed our mental health services, are in
the process of establishing our Integrated Joint Board and are well progressed with plans to
develop a new acute hospital. A necessary component of the new hospital work includes the
fostering and consolidation of greater mental health skill and knowledge capacity. The post
holder will be liaising closely with doctors, nurses and therapists within the new hospital, and
in the community in order to drive this work forward.

The post holder will also be working closely with a vibrant Third Sector and local Service
User movement in order to tackle mental health stigma and increase local capacity and
resilience.

Job Description

                         Consultant Community Psychiatrist

Introduction
Applications are invited for the post of Consultant Community Psychiatrist in NHS
Orkney.

The Consultant Community Psychiatrist is a new position for a full-time 12 PA (10
programmed activities plus 2 extra-programmed activities), replacing a previous part-
time post. The post holder will provide Consultant general psychiatric services (adult
and elderly) to the population of Orkney. The post is based with the Community
Mental Health Team, who are currently located in Stromness, one of Orkney‟s two
main towns, but will be making a transition to a new team base in Kirkwall. The post
holder will operationally report to the Mental Health and All Age Learning Disabilities
Service Manager         and to the Medical Director for professional matters. The
Psychiatrist will work in a Clinical Network with NHS Grampian

General
The post provides an opportunity to enjoy an outstanding quality of life as part of a
vibrant and forward thinking community, in an area of superb natural beauty, heritage
and culture which also enjoys subsidised and frequent air and sea links to mainland
Scotland.

NHS Orkney Health Board has one Remote and Rural General Hospital and serves a
population of 21, 500. Inpatient psychiatric patients are admitted to a Transfer Bed in
the Balfour Hospital in Kirkwall, until transferred to, usually, Royal Cornhill Hospital in
Aberdeen.

NHS Orkney is at the forefront of developing sustainable and cost-effective care in a
remote and rural environment. The successful candidate will have the opportunity to
participate in the challenge of providing this evolving service in a remote and rural
setting.

NHS Orkney currently procuring a new hospital and healthcare facility which is
expected to be completed in 2018, to which the Community Health Team will relocate.

NHS Orkney is at the forefront of developing sustainable and cost-effective care in a
remote and rural environment, to ensure the delivery of quality clinical care, which is
person-centered, safe, and effective.

Orkney NHS Board

Mental Health is currently an integrated service and is part of a Joint NHS Orkney
(NHSO)/Social Services partnership, and will be part of Orkney‟s Integrated Joint
Board, known as Orkney health and Care. The Community Mental Health Team
(CMHT) work closely with Primary and Community Care, locally commissioned Third
Sector providers, and other community stakeholders, in order to reflect and address
community-wide needs and provide a co-ordinated approach to mental health care and
support .The Community Mental Health Team works closely with all other health care
professionals/ agencies on the island in order to promote equality of health access for
people with mental health needs. The post-holder will be a member of the NHSO Safe
and Effective Care Group and will support GP and Primary Care services to
understand and work effectively with people with mental health needs.
Orkney has an innovative Third Sector that promotes positive partnership working with
the Health Board and Local Authority.

Orkney Mental Health Services

The Community Mental Health Team currently consists of:
      a Service Manager
      an Operational Manager,
      2WTE Admin support staff,
      an Older Adults Mental Health Service delivered by 1WTE CMHN, 1CSN,
       1WTE MHO, 1WTE Support Worker;
      a Substance Misuse Service delivered by 1.0 WTE CMHN (Dual Diagnosis),
       2WTE CMHN‟s, I WTE Support Worker (Dual Diagnosis), 1 WTE Support
       Worker;
      a Psychology Therapies Service to Adults service delivered by 0.8WTE Clinical
       Psychologist. 0.8 WTE OT/CBT Therapist.
       1 WTE CAAP ( funded until end of March 2018)
      a CAMHS service delivered by 1WTE CAMHS practitioner and1WTE CAMHS
       CMHN. The CAMHS service is supported by a visiting CAMHS Consultant
       Psychiatry service and a visiting CAMHS Psychologist.
      1WTE CAAP (funded until end of March 2018)

The CMHT operates a “Duty System” and on call facility, staffed by CMHNs and
MHO‟s, over the 24 hour period, seven days a week.

The Social Care element of the team is enhanced by a further social worker (1WTE)
in addition to the MHO attached to the Older Adult Service above, (both with MHO
status), and a WTE Support Worker

In-patient services for both adult and older adult patients are provided via a service
level agreement with NHS Grampian at the Royal Cornhill Hospital in Aberdeen. While
awaiting transfer to Aberdeen, patients experiencing acute psychiatric emergency are
supported and assessed within a designated bed within the Balfour Hospital. A CPN
provides support while the patient awaits transfer to Royal Cornhill Hospital by a
Grampian based retrieval team. Close working relationships are maintained with the
Grampian service. The post holder will be given an Honorary Consultant Contract with
NHS Grampian. Second opinions and more specialised assessments and advice are
available through Grampian services (for example Eating Disorders, Forensic, and Old
Age Psychiatry). The appointee will spend two days a week in the Royal Cornhill
Hospital, and will hold responsible medical officer role for Orkney patients during and
after hospitalisation.
The mental health team is a highly resourceful and skilled service that operates in a
collaborative culture. Practitioners are trained in a wide range of psychological
therapies within the national Access to Psychological Therapies Framework. This work
is co-ordinated by the team psychologist who is the local Psychology Therapies
Training Coordinator. This post holder will also be expected to take part in
development work and engage with the regional network collaboration (building on the
already close links with the Grampian service in Aberdeen).

There is scope for the pursuit of special interests by the post holder in collaboration
with colleagues both in local and mainland services and for linking into existing
regional and national specialist networks.

Travel both within Orkney and the Scottish mainland for liaison, CPD and other related
activities will be required. Regular educational and other clinical / professional
development activities in Grampian will be arranged subject to mutual agreement and
might include undergraduate/postgraduate teaching.

The post holder will be the Clinical Lead for Mental Health Services in Orkney and will
work closely with the Service Manager to ensure the delivery of safe and effective
mental health services. A local steering group meets to develop strategic plans for the
service and to take a wider view of all aspects of mental health service provision. This
group includes representation from service users, carers and the voluntary sector.

Principal Duties

1. Provide clinical leadership to the CMHT and work collaboratively with the
   operational and service managers.

2. To take the lead role in developing clinical mental health services in Orkney and
   more broadly within the North of Scotland NHS mental health
   community, particularly via the MH Obligate network

3. To provide a clinical service in community psychiatry for adults and the elderly
   including; out-patient consultations, assessment and treatment of patients in the
   community. and a range of care settings, emergency assessment and treatment.

4. To provide assessments and advice where appropriate for patients in the care of
   medical and surgical colleagues and those attending accident and emergency with
   mental health problems.

5. Fulfill duties of an Approved Medical Practitioner under the Mental Health (Care
   and Treatment) (Scotland) Act, 2003.

6. The Post Holder must be an active participant in Clinical Governance and Clinical
   Risk Management, including implementation and audit.

7. To contribute to the teaching and training             of   students,   trainees    and
   multidisciplinary staff groups as appropriate.

Clinical Network with NHS Grampian

The successful candidate will have a primary appointment in NHS Orkney and a joint
appointment with NHS Grampian in the form of an honorary contract. It is envisaged
that the successful candidate will spend 2 days each working week with Royal Cornhill
Hospital, over and above requirement for attendance in order to fulfill the post-holders‟
CPD programme.

Governance

The Consultant Community Psychiatrist will be one of the key senior practitioners
responsible for developing and maintaining clinical standards within the Mental Health
team. They will report professional matters to the Medical Director and provide
professional and clinical leadership where required.

On-Call Responsibilities

The position is not associated with a full-time on-call rota. For the out-of-hours period,
Psychiatric advice is currently provided by NHS Grampian. It is therefore not required
that the post holder commits to an out-of-hours role.

Continuing Professional Development

The Board recognises this to be a crucial aspect of the appointment as part of its
provision of high-quality health care and responsibility for clinical governance. CPD is
undertaken not only in line with individual development plans, but also as part of
departmental service development.

A medical library is well established in the Balfour Hospital. Internet access to the e-
library and an online retrieval system is available locally at each Consultant‟s
dedicated laptop. Arrangements exist for access to the extensive medical library at
Aberdeen Royal Infirmary for research and study purposes not catered for in Orkney.

CPD activities are encouraged and we will fully support appropriate study leave
opportunities. A dedicated and generous consultant CPD budget exists, and CPD
activity is monitored during appraisal and job planning. Requests are considered
flexibly to allow for the difficulties of off-island travel. The Board is at the forefront of
video conference use as an innovative tool to support remote learning. In recognition
of the potential difficulties of professional isolation, the Board also looks favourably on
short secondments to other Units, especially should they be designed to update
clinical skills or to develop clinical networks further.

Research

Although research is not included as a part of the proposed duties of this post the
Board will provide active encouragement for the Post Holder to pursue any personal
research interest as it would any area of appropriate professional development.

Leave
The post-holder will be entitled to six weeks' annual leave per year in addition to the
ten public holidays. Study leave of thirty days per triennium is allowed. There is
recognition of the complexities of travel to educational events in mainland UK, and this
is taken into account when study leave is requested.

In keeping with the aforementioned commitment to improve the resilience of the
Obligate Network, the post holder will contribute to, and help develop, a mutually
workable and agreeable cross- cover arrangement between Orkney and Shetland in
the event of annual leave or extended sickness.

      The job reference number can be copied from the job advert.

      The job location will be on the job advert.

      The candidate ID number will be written in by us once you have returned the
       form to our office.

      The people who look through your completed form (short listing or short
       leeting team) to see if you have the skills and abilities needed for the job, will
       only see „Part C‟ of the form. They will only see your candidate ID number
       and all personal details will be anonymous.

Personal Details section

      This gives us your contact details such as name and address. Under „title‟
       you would put either Mr, Mrs, Ms, Dr or just leave blank if you prefer.

      We may need to contact you at some time throughout the recruitment
       process. Please let us know the most suitable method of contacting you, for
       example a text message, email or phone call and the most convenient time.

Sickness

      Please list in this section the number of occasions you were off work in the
       past year because of illness. If you would like to discuss this for further
       clarification please call our recruitment office.

Declarations

      Only if it is asked for in the job description, person specification or
       application pack about the job, you must tell us about all convictions and
       cautions regardless of how long ago the offence may have occurred.
       Convictions from other countries must also be notified. If in doubt please call
       our office at the help line number on page one of this guidance.

      Please note that having a conviction will not automatically disqualify you from
       getting a job with us. Careful consideration will be given to the relevance of
       the offence to the particular post in question. However, if you are appointed,
       and it is found that you did not reveal a previous conviction your employment
       may be terminated

Qualifications section

      Please tell us of any qualifications you have. This can include school
       standard grades, GCSEs highers, or work based qualifications such as SVQs
       or NVQs

      Remember to write down any „non formal‟ qualifications or certificates that you
       think are relevant to the job you are applying for.

Present (or most recent) post section

      If you are currently out of work, please write this in the „job title‟ space

      Please write your start date in month/year format MM/YYYY

      Please tell us briefly about your duties (what you do or did in your job). You
       could tell us your role, the main tasks, and any responsibility for supervising
       others. There is not a lot of space here so continue on a separate sheet if you
       need to.

Employment History section

      This is where you write down all the jobs you have done previously.

      Remember that if a job you have done in the past supports or is similar to the
       job you are applying for, please tell us more about it in your „support of
       application‟ statement on page 6 of the application form – use a separate
       sheet if you need more space.

References section

      Referees are people who know you at work. Please give the full names and
       addresses of 2 referees, one of whom must be your present or most recent
       employer and can confirm your job details.

      You should check that the people you have put on your form are happy to be
       referees.

      Your referees will not be contacted unless you are a „preferred candidate‟
       after interview. A preferred candidate is someone who is the preferred choice
       for the job, subject to satisfactory checks where appropriate.

Driving Licence

      You only need to complete this if the job requires you to drive. Please check
       the job description or person specification. For example, some jobs with the
       Ambulance Service require you to be able to drive class C1 and D1 vehicles.

Statement in support of your application

      This is one of the most important parts of the form. In here you say why it is
       you want this job, and can list all your skills and abilities that you think help to
       match up you against the „person specification‟. In here you could describe
       how something you have done in a non work setting (for example, planning
       and leading a group outing) shows planning skill and some leadership
       qualities.

Where did you see the advert section

      Please try to remember where you heard about this job, and tick the relevant
       box. The information you give will help us find out how good our advertising
       is.
