SELECT PLAYER COMMITMENT FORM 2021-2022

I have received a copy of the Player Commitment Form. Classics Elite Soccer Academy is committed to each player for a full year,
and we expect each player to be committed to Classics Elite Soccer Academy for the year as well. Classics Elite Soccer Academy has
a stringent refund policy because a player's acceptance has the effect of denying another player an opportunity to play. Fees are not
refundable or transferable. The annual club fees will cover June 1, 2021 – May 31, 2022. No refunds, partial or full, will be made to
players who choose not to participate in our competitive program at any point after registration for any reason, including players
suspended from the program. Players fees will not be refunded or forgiven due to player absences, withdrawals, or early release from a
team.
All players and their families are expected to abide by and support the policies and procedures included therein. I understand that I am
responsible for all club dues, player travel expenses and any expenses not included in the club dues for the 2021-2022 season. I
understand that these expenses are non-refundable. If I default on my obligations hereunder to make payments according to the
payment plan set up in TGS and my account is sent to collections, I agree to pay all costs of collection including attorney expenses. All
players are expected to meet a high standard of commitment to training and play. Players are expected to attend all practices and
participate in all league games and tournaments. All players and parents are expected to read and abide by the Code of Conduct. If you
have any questions, please contact your team coach or the Program Director. I understand that we are an Adidas club and agree that my
player will train and play only in the Adidas gear that the club requires. Although we strive to provide quality-playing time for all players,
there are NO guarantees of playing time in any single game. All players should expect to play in several tournaments on holiday weekends,
including Labor Day, Thanksgiving and possibly others. ECNL/USC CL are non-transferable leagues. If you have any questions, please

I understand and accept my commitment to the Classics Elite Soccer Academy and agree to abide by and support the policies and
procedures included therein. I hereby release any officers, coaches, sponsors, or owners/lien holders of properties used by the Classics
Elite Soccer Academy from any and all claims and causes of action of any kind whatsoever which the undersigned has or might have,
known or unknown, now existing or might arise in the future, directly or indirectly attributable to any injury or damage that might result
from my child’s participation in soccer activities. I agree that Classics Elite Soccer Academy may use pictures of my child for any lawful
purpose for such purposes as player identification card, publicity, illustration, advertising, and Web content. I authorize Classics Elite
Soccer Academy, its assigns and transferees to copyright, use and publish my child’s sports photos in print and electronically
BY SIGNING THIS DOCUMENT, I/WE AGREE TO THE FOLLOWING:
● I/We are registering the player named above as a member of Classics Elite Soccer Academy for the 2021 - 2022
year and understand that this commitment if for June 1, 2021 – May 31, 2022.
● I/We have read the Classics Elite Refund Program and understand that players fees will not be forgiven or
refunded.
● I/We understand that the selection and team placement of the player shall be at the discretion of the Classics Elite
Soccer Academy coaching staff and is subject to change.
● I/We have read and agree to abide by the rules and agreements and policies (including Parent and Player Guidelines) of
Classics Elite Soccer Academy included in pages 1-7 of this Select Player Commitment Form.
● I/We agree to abide by the rules and agreements of all the leagues in which Classics Elite Soccer Academy participates.
● I/We agree that the continued participation of a player is contingent upon the player and his/her family continuing to
adhere to the rules, agreements and policies of all leagues and tournaments we participate and those referenced above.
1CLASSICS ELITE CLUB DUES / PAYMENT OPTIONS
CE CLUB DUES INCLUDE:

You will receive an emailed link or team code to pay your Commitment Payment and set up Payment Plan within 7
days of team assignment. Payment in Full or a Payment Plan and non-refundable Commitment Payment MUST be
completed within 24 hours of receipt of emailed link or team code or player will be removed from the assigned
team.
PAYMENT OPTIONS
PAYMENT IN FULL. Checks for Payment in Full are made payable to CLASSICS ELITE SOCCER ACADEMY.
If you choose to pay in full at the time of acceptance, you will receive $100 discount off the Annual Dues (ends May 31st,
2021)
PAYMENT PLAN. Payment plans can be set up for 10 months as outlined below. There will be a $35 penalty charged for
payments more than 15 days late if your credit card is declined or the draft is denied. PAYMENT PLANS MUST HAVE A
CREDIT CARD ON FILE FOR AUTOMATIC PAYMENTS. Credit Card fees apply.

● Above dues do not include mandatory uniform kit, player travel expenses, pre/post season events, tournaments,
cup play (non ECNL/R) teams, scrimmage expenses, staff tournament travel, spirit wear, or accessories.
Sibling Discount:
If your family has 2 or more players in the competitive program you will receive a 10% discount on the lesser dues. To
request a sibling discount go to www.classicselite.us/placement, click forms and fill out the Sibling Discount form.
Discounts cannot be combined. Discount cannot be applied towards Commitment Payment. Excludes Academy/Rec
players. See online form for additional details. Expires 8/1/2021
2PLAYER TRAVEL EXPENSES / TOURNAMENT EXPENSES:
Player’s family is responsible for Player Travel Expenses and Tournament Expenses that will be paid to the
Team Manager or Treasurer for deposit into the Team Account. Payments are due when Team Manager /
Treasurer indicate. Team fees: There will be no refund of team fees. Team fees are fluid depending on the
tournaments planned, what the team is accepted to and the variable costs of travel. Every player is responsible
for team fees regardless of game/tournament attendance.
PLAYER TRAVEL EXPENSES: Players are responsible for individual travel expenses which include but are
not limited to hotel cost, transportation cost and meals. A Budget will be provided by the Team Manager or
Treasurer.
TOURNAMENT EXPENSES: Players are responsible for expenses incurred for pre/post season play,
tournaments, cup play, and scrimmages. A Budget will be provided by the Team Manager or Treasurer.
FAILURE TO PAY:
Failure to pay your monthly dues within thirty (30) days of payment due date will invoke the “NO PAY- NO
PLAY”. Players will not be eligible to participate in team practices, games, and tournaments until payment is
received. CLASSICS ELITE SOCCER ACADEMY realizes that emergencies may result in a financial strain on
the family. In order to keep the player from being ineligible the Club will work with a family if the family contacts
the office in a timely manner.
ACCOUNT CREDIT / REFUND POLICY:
Classics Elite Soccer Academy is committed to each player for a full year, and we also expect each player to
be committed to CESA. Classics Elite Soccer Academy has a stringent refund policy because a player's
acceptance of team placement denies another player placement on the team. Dues are not refundable or
transferable. The annual club dues will cover June 1, 2021 – May 31, 2022. No refunds, partial or full, will be
made to players who choose not to participate in our competitive program at any point after registration for any
reason, including players suspended from the program. Dues will not be refunded or forgiven due to player
absences, withdrawals, or early release from a team. Club dues remain the same regardless of the duration of

3MANDATORY UNIFORMS KITS: 2020-2022
The Uniform Package for 2020 – 2022 will be purchased from www.Soccer.com. Uniform numbers will
be assigned by the club due to new club pass rules and dual rostering across teams. You will receive
an email to purchase the kit within 7 days of COMPLETED Registration. The Kits will be used for both
2020-21 and 2021-22 Seasons.
The Required Package must be purchased by June 15, 2021, and the remaining Optional Items will be
available for purchase through September 30, 2021. *Note these Items are Required for ECNL/R/CL
Uniforms must be ordered by June 15 to ensure delivery by Team Camp.

Payment for the uniform is required when the order is placed online with www.Soccer.com. Deadline for initial
required items is June 15, 2021. Please email registration@classicselite.com if an extension is needed. Please
include player name and team in subject line. We are not responsible for uniforms purchased outside of
soccer.com
FINANCIAL AID:
If financial assistance is needed, you must complete the Financial Assistance Application. Contact the Club
Administrator at registration@classicselite.com for an application. The non-refundable Commitment Payment
of $300 must be turned in with your application. All paperwork must be submitted at the same time. If award is
accepted, the money will be credited to the club dues. A decision will be sent by email from the financial
aid/scholarship committee. Your account will not be credited until the online form is filled out and accepted. You
are required to complete (10) hours of volunteer hours complete volunteer hours, per child/per season.
NOTE: Completion of the financial assistance application must be submitted by the due date listed below.
Application deadline for Current CE Players is May 15, 2021 and New Players is June 15, 2021.
4PLAYER AND PARENT AGREEMENTS
Commitment to the Classics Elite Soccer Academy is for a year-long period, unless granted a signed release by the
Director of Coaching.
THE PLAYER AGREEMENT:
• The PLAYER should maintain a level of performance at school and at home which is sufficient to allow a
commitment to participate in all team activities.
• The PLAYER should make every reasonable effort to attend practice sessions, games, and tournaments.
• The PLAYER should call the team coach in advance if unable to be in attendance of any team event.
• The PLAYER should be on time with CE gear on, shin guards, and an INFLATED ball for all practices and games.
• The PLAYER should always give the coach and staff their utmost respect and attention.
• The PLAYER should treat every team event as a learning experience.
• The PLAYER should be open to instruction and evaluation with a growth mindset
• The PLAYER should make every effort to win within the rules of the game.
• The PLAYER should respect game officials and the opponents regardless of the result.
• The PLAYER agrees that grievous unsportsmanlike conduct can be grounds for suspension or dismissal.
• The PLAYER agrees to not use any form of social media or publication to negatively comment on coach,
teammates, referees, parents, or opponents to include parents, or CE Soccer Academy.
• PLAYERS should refrain from inappropriate or negative behavior, through actions or words toward any
REFEREES, COACHES, PLAYERS and PARENTS.
• PLAYERS shall not use profane language on the soccer field.
• PLAYERS will be expected to follow all club guidelines, policies, and procedures.
Failure to comply with the aforementioned Code of Conduct will be grounds for potential PLAYER and/or
PARENT disciplinary action. During that time suspension or expulsion could be considered.

5THE PARENT AGREEMENT:
• PARENTS must understand that playing time is at the discretion of the coach in alignment with league rules and
club values.
• PARENTS should provide encouragement and create a good learning environment
• PARENTS should NOT COACH PLAYERS during games as it may contradict the coach’s wishes and numerous
voices giving INSTRUCTION DOES NOT help the PLAYER or the team.
• PARENTS should be their PLAYER’S number one fan and not their coach. The PLAYERS get plenty of coaching
from the staff what PLAYERS need from you is positive support and unconditional love.
• If your cheering involves giving direction to players it is coaching.
• PARENTS should refrain from inappropriate or negative behavior, through actions or words toward any
REFEREES, PLAYERS, PARENTS, and COACHES.
• Any unlawful act by PLAYERS or PARENTS could be cause for suspension or expulsion.
• PARENTS shall not use profane language on the soccer field.
• PARENTS should be willing to support the team by volunteering time for chaperoning, transporting PLAYERS,
fundraising as requested, etc.
• PARENTS should follow the chain of command when issues arise. The first point of contact is the team coach, if
needed the next point of contact is the Program Director of Coaching and finally the Director of Coaching. PARENTS
agree to follow this communication protocol with any concerns or issues.
• There is a 24-HOUR RULE when speaking to coaches. PARENTS and Coaches will wait 24 hours after a practice
or game to discuss team or PLAYER issues. This allows for emotions to subside, so an effective conversation may
take place.
• PARENTS should not talk negatively about any other child.
• Unrequested input from parents on coaching decisions will not be welcomed.
• PARENTS agree to not use any form of social media or publication to negatively comment on a coach, teammates,
referees, parents, or opponents to include parents, or CE Soccer Academy
• PARENTS will be expected to follow all club guidelines, policies, and procedures.
• PARENTS will not challenge, protest or file any complaint (written or verbal) against soccer officials, league
officials, referees, opposing coaches or opposing parents. If you feel something needs to be addressed
please contact your program director and/or Director of Coaching.
Failure to comply with the aforementioned Code of Conduct will be grounds for potential PLAYER and/or
PARENT disciplinary action. During that time suspension or expulsion could be considered.

6I have had sufficient opportunity to read the Select Player Commitment form. I have read and
understood it, and I agree to be bound by the terms of the Contractual Agreement. The Parent or
Court-Appointed Legal Guardian must sign for any participating minor (those under 18 years of age) and
agree that they and the minor are subject to all terms of this document, as set forth above. By completing
this registration, I certify that I am the Parent or Legal Guardian of the above minor.
