comfortable as possible for people and their loved ones.
At NLH we are passionate about providing quality palliative
and end of life care education to develop knowledge and

Clinical Record Keeping & Documentation

WHO All those required to keep records in a healthcare setting.

This short course is aimed at both registered and non-
registered care staff. It will explore the legal requirement for
accurate, contemporaneous record keeping alongside the
need to ensure that the patient’s voice is heard. The course
CONTENT
will encourage discussion around issues such as ‘fact’ and
‘opinion’; how documentation makes a difference to the
patient’s experience and whether how we document care
           could be more efficient.
