                                 AJAX PLAYER COMMITMENT AGREEMENT
                                Thank you for choosing NB AJAX Soccer Club!

Application is hereby made for registration of the player named below as a member of the NB AJAX Soccer Club for the
2018-2019 soccer year. I/We understand that this commitment is from June 1, 2018 through May 31, 2019. I/We
understand that the selection & team placement my player shall be at the sole & absolute discretion of NB AJAX Soccer
Club coaching staff. The continued participation of a player is contingent upon the player & family abiding by the rules,
agreements, & policies (including Parent / Player guidelines & responsibilities found in the NB AJAX Player / Parent
Handbook, online at www.nbajax.com) of the NB AJAX Soccer Club, the Alamo Area Youth Soccer Association (AAYSA),
Western District Designated Operations Association (WDDOA), South Texas Youth Soccer Association (STYSA), State Classic
League (SCL), and Southern Regional Premier League (SRPL). A player is not allowed to play for more than one team at the
same time in any league (recreational or select), except where the club pass rule is permitted or dual rostering between
associations is permitted. Any player rostered to a team is bound to the team to which they are first assigned from June
1 until May 31 unless the parent completes & submits a STYSA Player Transfer / Release Form. However, any player
rostered to a NB AJAX SC team is a NB AJAX SC player throughout the year. If the coaching staff feels movement between
teams is necessary to benefit the overall development of the club, teams & player, such movement between teams can
be initiated by the NB AJAX SC coaching staff any time the rosters are open & league rules permit player movement.
Players have the right to refuse to move, though doing so & going against the qualified guidance of the NB AJAX SC
coaching staff is not encouraged.

IMPORTANT DATES:
Please refer to the Team Snap Calendar, as this will be your primary source of general information & important dates for
your team & for NB AJAX SC throughout the year.

PLAYER & PARENT HANDBOOK:
The 2018-2019 NB AJAX Soccer Club Parent / Player Handbook can be found online at www.nbajax.com under
“Registration.” The handbook contains information regarding NB AJAX SC policies & procedures. All players & their families
are expected to abide by & support the policies & procedures included therein. If you have additional questions, please
contact your head coach, team manager, or the NB AJAX Soccer Club Business Manager, Jen Carr, as described in the
handbook.

**NEW** VOLUNTEER COMMITMENT:
Each family (even if you have more than one player in select soccer) is required to commit five (5) hours during the fall &
five (5) hours in the spring as a volunteer in support of Club events & needed items. There are several ways to cover this
commitment. Please refer to the Volunteer page, under the AJAX Select Soccer tab on the website (www.nbajax.com) for
a complete list of approved events and/or jobs that qualify for hours of service. These hours are tracked, and it is your
responsibility to ensure proper credit is received. Volunteers are not automatically assigned to any event. Prior to a need,
a request for volunteers will be sent by email. Volunteer opportunities do not require knowledge of soccer, but volunteers
must be over 16 years of age to participate.

Parents choosing not to volunteer must pay the Buy-out fee of $75. If the buy-out option is chosen, payment must be
made upon signing spring commitment paperwork. Parents who do not buy-out, & do not fulfill the five (5) hour
requirement will be billed $75 at the end of the season.

NB AJAX SC FINANCIAL AID PROGRAM:
There are many children in the New Braunfels area who possess the ability & desire to succeed on & off the soccer field,
but lack the financial means to play select soccer. To help address this need, NB AJAX SC has a need based financial aid
program. Money raised by the program provides financial aid for qualified families in our club. The aid assists with training
fee payments which enable them to play with NB AJAX SC. Last season (’17-’18) approximately 35 players qualified for
some amount of financial (10% of the club) aid totaling over $20,000. These awards are based on economic need using
the Federal Poverty Guidelines & State of Texas Reduced & Free Lunch Program as a decision-making tool. Awards are
reviewed & made by Scholarship Committee. Awards are not & cannot be based on talent.

As a part of our annual giving campaign, we are extending the option to add a onetime payment of $60 or $100, to be
credited to the NB AJAX SC Financial Aid Program. Please consider donating to help our financial aid program.

When applying for the spring season only, limited funds remain for financial aid awards. However, applications are still
available & are reviewed for possible awards.

If financial aid is needed, the Financial Aid Application is due in the NB AJAX SC office by July 15, 2018. A written decision
will be mailed/emailed prior to the August 1st payment due date. Payment of the $200 or $300 commitment fee is required
at the time of signing. The commitment fee cannot be included in financial aid considerations. For questions concerning

EXPECTATIONS AND REQUIREMENTS:
All players are expected to meet a high standard of commitment. Players are expected to attend all practices & participate
in all league games & tournaments. Although we strive to provide quality playing time for all players, there are NO
guarantees of playing time. Academy, Division 2, and Super II coaches teams will make every effort to play each player at
least one-half of each game, except when players are injured, ill, absent from training, or for disciplinary reasons. All
players should expect to play in several tournaments, possibly on holiday weekends, including Labor Day, Thanksgiving,
Memorial Day, &/or others.

**NEW**BIRTH CERTIFICATE/PASSPORT:
Once your player has been assigned to a team, you must submit a copy of a birth certificate or passport to verify name
and date of birth. There is a secure link on the website registration page to upload the copy.

Annual 2018-2019 FEES:
Commitment fee is required plus monthly training fees as listed below.

PAYMENT SCHEDULE:
The commitment fee is $200 (D2, Super II, D1) & $300 (SCL & Premier). Each player must pay this fee.

Payments are due on the first (1st) of each month unless otherwise requested by the parents to the Business Manager, Jen
Carr. All accounts must be paid in full by May 15. If a team/player moves up in level of play during the seasonal year,
they will pay the higher fee and if a team/player moves down a level during the seasonal year, the fees are prorated
accordingly.

Monthly payments are made directly to the Club via credit/debit card, cash, money order, or personal check. Monthly
automatic payments may be set up for MasterCard/Visa/Discover/American Express through GotSoccer. Automatic
payments will run on a the first (1st) of the month. Please check your player account for this date. Personal checks, money
orders or cashier’s checks should be made payable to the NB AJAX SOCCER CLUB and mailed to: NB AJAX SC, 244 FM
306, Ste 120 PMB 633, New Braunfels, TX 78130; or dropped in the payment box located in the field office at Weston
Soccer Fields. Please include the player’s name on memo line of your check. If setting up automatic-bill-pay through
your bank, the customer name is NB AJAX SC and the player’s name is the “account number.” We strongly encourage
setting up automatic recurring monthly payments. A $40 fee will be charged for any NSF check or declined credit/debit
card.

Failure to pay your monthly fees within sixty (60) days of fee payment due date will invoke the “NO PAY- NO TRAIN”
Club Disciplinary Policy. Players will not be eligible to participate in team practice until payment issues are resolved.

Annual fees include all training fees, fall & spring league fees & league tournament fees (i.e., qualifying tournaments,
Spring Cups, etc.). Additional costs include non-league pre/post season tournament fees (determined by Ajax Coaching
Staff), all player travel expenses & out-of-Austin team travel costs for coaches at these events. Fees remain the same
regardless of the number of days your player attends training, the number of weeks for training or number of games in
the season. During the season if the player’s division status changes, fees will be adjusted accordingly. Fees will not be
refunded or forgiven due to player absences, withdrawals, or early release from a team. The only exception to this policy
is if the player provides the office with documented proof of an injury that prevents them from playing for a at least 6
weeks. Prorated refunds for documented injuries can be arranged.

UNIFORMS

ORDERING YOUR UNIFORM:
Following your team assignment, you will receive an email with a link to our NB AJAX SC uniform page; late May, for U11
& U12’s (’08 & 07 birth year), and in early June for U13’s-U19’s (’06 – ’00 birth years). Follow the link to order your player’s
uniform. Uniform numbers (#) can be chosen by the player after confirmation with the Team Manager or Club Business
Manager. If your child will be dual rostered or play in multiple leagues, NB AJAX reserves the right to assign a jersey
number to minimize unnecessary and costly number changes and/or jersey purchases throughout this uniform cycle.
You will need to choose each required uniform pieces, as well as ordering any optional uniform pieces directly from the
Challenger Teamwear. Once you have all required uniform pieces, please check out & complete your order using your
credit card. Be sure and include your mailing address, as all uniform pieces will be shipped directly to you. Please plan for
a four (4) week delivery timeframe. UNIFORM DEADLINE FOR ORDERING IS JULY 1, 2018 TO GUARNATEE DELIEVERY OF
YORU KIT BEFORE PRESEAON TOURNAMENTS.

Samples of the uniform kit pieces for sizing will be available at the Field Office at Weston Soccer Fields during tryouts and
during training times after tryouts. Please contact NB AJAX SC Business Manager, Jen Carr, if you need assistance.

MANDATORY UNIFORM PACKAGE:
The cost of the required uniform package is $289, regardless of size (excludes shipping after July 1, 2018). The select
uniform package must be purchased through Challenger Teamwear. The uniform package includes two INARIA dry fit
game jerseys, two pairs of INARIA game shorts, two pairs of INARIA game socks (1 black, 1 grey); one INARIA dry fit practice
jersey, one full INARIA warm-up; and one INARIA backpack. The warm-up & backpack are required pieces of the uniform
package. The uniform package does not include training shirts, which are mandatory & picked up at the Field Office at
Weston Soccer Fields. Training shirts are FREE OF CHARGE to all registered players. Additional INARIA uniform pieces,
logo wear & soccer balls are available for order online through Challenger Teamwear, at the Field Office at Weston Soccer
Fields.

GOALKEEPER (GK) UNIFORM:
There are optional Goalkeeper uniform pieces available for order on the Challenger Teamwear order link online. However,
NB AJAX SC doesn’t require GKs to purchase these items. GKs are able to purchase GK jerseys wherever they would like.
GKs are REQUIRED to have an approved jersey number on the back of their preferred jersey & an NB AJAX SC logo patch
on the front, left breast of the jersey. Numbers can placed on jersey & patches can be heat sealed on the jersey through

REQUIRED SIGNATURE RELEASE WAIVER FOR PARTICIPATION FOR PLAYER ________________________________

I, _____________________________________________ (parent / guardian) hereby give and assign to NB AJAX Soccer Club, LLC,
and its legal representatives, the right and permission to publish my child’s name, likeness, photographic image, video, and
biographical data for the specific purpose of publicity and promotion of the player, team, and/or NB AJAX.

In consideration of NB AJAX allowing my child to participate in NB AJAX’s programs, I HEREBY WAIVE AND RELEASE, INDEMNIFY,
HOLD HARMLESS AND FOREVER DISCHARGE NB AJAX, CITY OF NEW BRAUNFELS, HEB SOCCER FIELDS, NEW BRAUNFELS YOUTH
SOCCER ACCOSIATION, COMAL COUNTY, WESTON SPORTS COMPLEX, SOCCER ZONE INDOOR SOCCER, NEW BRUANFELS
INDEPENDENT SCHOOL DISTRICT, AND ANY OTHER PARTNER OR PROVIDER OF FACILITIES, ITS AGENTS, EMPLOYEES, OFFICERS,
DIRECTORS, AFFILIATES, SUCCESSORS, AND ASSIGNS (HEREBY JOINTLY AND SEVERALLY REFERRED TO HEREIN AS “RELEASED
PARTIES”), OF AND FROM ANY AND ALL CLAIMS, DEMANDS, DEBTS, CONTRACTS, EXPENSES, CAUSES OF ACTION, LAWSUITS,
DAMAGES AND LIABILITIES, OF EVERY KIND AND NATURE, WHETHER KNOWN OR UNKNOWN, IN LAW OR EQUITY, THAT I OR MY
CHILD EVER HAD OR MAY HAVE ARISING FROM OR IN ANY WAY RELATED TO MY CHILD’S PARTICIPATION.
This waiver and release includes (but is not limited to) claims based on or arising from any injuries to my child resulting from said
child’s participation in any of the training, camps, or related soccer or physical activities conducted by, or on the premises of, or for
the benefit of any of the RELEASED PARTIES as well as injuries resulting from engaging in fitness or strength and endurance
workouts, practicing and/or engaging in soccer practices, scrimmages, tournaments, league games or other related activities
including travel to and from. I understand the activities that said child will participate in can be dangerous, involves risks of serious
bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of
others participating in the event, the conditions in which the event takes place, or the negligence of the RELEASED PARTIES named
above; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and
assume all such risks and all responsibility for losses, costs, and damages I and/or my minor child incur as a result of my and/or my
minor child’s participation. On behalf of myself, child, my heirs, assigns and next of kin, my child, my spouse (if applicable), and I
waive all claims for damages, injuries and death sustained that we may have against RELEASED PARTIES regarding any such activity.
I certify to the best of my knowledge that my child's current physical condition is satisfactory for participation in the NB AJAX SC’s
soccer program and that my child is free of any health problems, which would endanger his/her, participation. I will inform the
coach should his/her condition change at any time during his/her participation in this program. This waiver of liability does not
apply to any acts of gross negligence, or intentional, willful or wanton misconduct.
I further agree that in consideration of and as a material term of NB AJAX SC’s agreement to allow my child to participate, I AGREE
TO INDEMNIFY, DEFEND AND HOLD NB AJAX HARMLESS FOR THE COST OF DEFENDING AND SATISFYING ANY CLAIM THAT IS
FILED AGAINST NB AJAX BASED ON OR ARISING FROM INJURIES TO MY CHILD.
This waiver and release contains the entire agreement between the parties and supersedes any prior written or oral agreements
concerning this subject matter. The provisions of this may be waived, altered, amended or repealed, in whole or in part, only upon
the prior written consent of all parties.

I have read, understand, and fully agree to the terms of this waiver and release. I agree to pay all fees as outlined in the 2018-2019
Commitment Agreement and understand that failure to pay fees may result in my child being placed in “no train” status. I
understand and confirm that by accepting this waiver and release said child and I have given up considerable future legal rights. I
have accepted this Agreement freely, voluntarily, and under no duress or threat of duress, without inducement, promise or
guarantee being communicated to me. By signing below, I am executing a complete and unconditional waiver and release of all
liability to the full extent of the law. I understand that NB AJAX is a Texas non-profit limited liability corporation. I expressly agree
that the foregoing Release Waiver is intended to be as broad and inclusive as is permitted by the law of the State of Texas and that
if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
