1Welcome to County Durham and Darlington NHS Foundation Trust,
we hope you enjoy your time with us and find it a valuable training experience!
This handbook aims to provide you with useful education and training information
and act as a reference guide for your Foundation Programme training within the
Trust and the Northern Foundation School. It will be supplemented by additional
information throughout your training.
In addition to this, as a Trust employee you will be expected to become familiar with
Trust policies relevant to you and your work. These are available on every
department in the Trust Policy and Procedure files and on the Trust Intranet.
Foundation programme training reflects a new era in medical education that should
be trainee centred, competency assessed, service based, quality assured, flexible,
coached, and structured & streamlined.
If you have any concerns, please do not hesitate to contact any of the key contacts
listed in this document.

The Director of Medical Education manages postgraduate medical and dental
education for the County Durham and Darlington NHS Foundation Trust. In relation
to Foundation Training she will support the Foundation Tutors, Educational and
Clinical supervisors.
Foundation Programme Tutors
These individuals will be responsible for the overall management and quality
assurance of the Foundation Programme consisting of 119 placements. The
Programme Tutors will ensure that each placement of the Foundation Programme
meets HEENE standard for training and that each foundation doctor is able to
access a comprehensive range of experiences which will enable them to gain the
competencies necessary for full registration and completion of the Programme.
SUPERVISORS
Educational Supervisors
This named individual will act as your educational supervisor for the duration of your
Foundation training at CDDFT and will take a special interest in your training and
professional development. The educational supervisor will meet with you throughout
the year. You should make arrangements to meet with your educational supervisor
within a couple of weeks of starting each post, at the middle and towards the end of
each post. You should set out and agree your educational and training needs and
goals for the post and how you are going to achieve them. Meetings must be
recorded on the ePortfolio using the appropriate forms.
The name and contact details of your Educational Supervisor will be emailed to you
by the Medical Education Team before your 1st working day.
Clinical Supervisors
Every foundation doctor will have a named clinical supervisor for each placement.
The clinical supervisor is responsible for overseeing your clinical work during the
placement, providing constructive feedback and completing the clinical supervisor’s
initial and end of placement reports. The doctor responsible for direct clinical
supervision may change on a daily basis, but the clinical supervisor will remain the
same throughout each placement. You should meet with your clinical supervisor at
the beginning of each placement to discuss what is expected in the placement,
learning opportunities available and your learning needs and at the end for an
assessment discussion. Meetings must be recorded on the ePortfolio using the
appropriate forms.
The Clinical Supervisor is allocated by your Department.
4ROTAS
Exception Reporting and Rota Monitoring
Medical Education Rota Team consists of Michelle Todd and Stephanie Court and
can be contacted via email at: cddft.med-edrotateam@nhs.net
The team are available to help with all of your queries in relation to your rota
template, rota monitoring and exception reporting.
Trainees on the 2016 T&C’s will have access to Exception Reporting – Reports
should be submitted when trainees day-to-day work varies significantly and/or
regularly from the agreed work schedule. Primarily these variations will be:
a. differences in the total hours of work (including opportunities for rest breaks)
b. differences in the pattern of hours worked
c. differences in the educational opportunities and support available to the doctor,
and/or
d. differences in the support available to the doctor during service commitments
Trainees on the 2002 T&C’s will be monitored twice yearly as per the annual
schedule below:

Trainees will be given notice of the exact dates via email to your nhs.net address
along with a full user guide to the system. Your user name and passwords for e-
Monitor will be also be issued to your nhs.net email address There is a mutual
obligation for the Trust and the Junior Doctors to monitor their working hours and to
contribute to the Trust’s continuing commitment to improving the working lives of
Junior Doctors.

DEPARTMENTAL INDUCTIONS
Each time you rotate into a new placement you will be required to attend a
mandatory local departmental induction, details of which will be forwarded to you
approximately two weeks before rotation date.
The local departmental induction is designed to ensure you have received
appropriate information about your working environment and the
expectations/limitations placed upon you, so enabling you to undertake your duties
effectively.
The signed departmental induction form must be returned to cdda-
tr.CDDFTFPTeam@nhs.net within 14 days of changeover.
5Annual Leave Request forms are available from your department. You should not
book holidays until you have obtained permission to take annual leave. Leave is
centrally co-ordinated within each department, this includes annual leave, study
leave and sick leave, this ensures that the service is covered at all times.
Please plan leave well in advance, and make sure that if you are on 4 monthly
rotations or 6 monthly rotations, that you divide your annual leave accordingly within
each rotation.
Sickness If you need to ring in due to illness, please follow the procedure below:
Contact your rota coordinator (list included in this handbook) and remember to
let them know if you are due to do an on-call or night shift.
Contact your clinical supervisor, if they are not available leave a message with
a senior team member in your department.
Inform the FP Team on cdda-tr.cddftfpteam@nhs.net of your sickness and
complete the self-certification form which is emailed to you monthly.
This procedure should be followed whether you are calling in sick from home or
leaving work ill.

THE TRAINING PROGRAMME
Educational Approval of Programmes
The General Medical Council (GMC) is responsible for ensuring that the placements
within the Foundation Programme meet the required standards of training,
education, appraisal and assessment.
Sometimes this responsibility is delegated to the Postgraduate Dean who will inspect
the posts and programmes on a regular basis and will wish to talk to you about your
experiences. Attendance at these inspections is therefore mandatory and you will be
informed in advance of the date of any inspection.
Responsibilities of the Foundation Doctor
As outlined by the Director of Medical Education and Foundation Programme Tutors,
with respect to the in-work assessment programme, the responsibilities of the
foundation doctor are that they:
Abide by the conditions of taking up a training post
Demonstrate professional behaviour in line with Good Medical Practice
Work within CDDFT policies and procedures as outlined by the local education
provider during induction
Become familiar with the requirements for satisfactory completion of F1/F2 and
the Foundation Programme
7 Become fully involved in the educational, supervised learning and assessment
processes, including attending mantatory training sessions, meeting regularly
with your educational and clinical supervisors and maintaining an up-to-date
ePortfolio
Take part in systems of quality assurance and quality improvement in your
clinical work and training. In particular, foundation doctors should complete the
YSYS trainee survey, the GMC trainee survey, the F2 outcome survey, and end
of placement or end of year surveys required by the deanery/foundation school.
ePortfolio
The Northern Foundation School uses the Horus ePortfolio.

All foundation doctors in a recognised training programme must maintain an
ePortfolio and use it to support their educational and professional development and
career planning. The ePortfolio is a record of a foundation doctor’s progress and
development through the foundation years. Successful completion of the Curriculum
requires the achievement of competence in a variety of domains based on Good
Medical Practice. Evidence of achievement of outcomes and increasingly improving
performance will be recorded in the ePortfolio.
The completed ePortfolio will contribute to the end of year report. Elements of the
ePortfolio may also be used in specialty interviews by the foundation doctor to
demonstrate competence and highlight achievements. This means that the ePortfolio
may be used to help the foundation doctor gain future employment.
Engaging with the ePortfolio is mandatory.
Usernames and passwords for the ePortfolio will be issued by the Foundation
School.

The Curriculum
The Foundation Programme Curriculum 2016 will be used to set out the framework
for educational progression for both foundation years one and two doctors.
The Curriculum and Reference Guide can be can be downloaded from the

Foundation Professional Capabilities
Each section of the Curriculum contains several ‘foundation professional capabilities.
The 20 foundation professional capabilities describe the key clinical or professional
aspects of medical practice expected of a Foundation Doctor. Foundation doctors
must provide evidence of how their achievements related to each foundation
professional capability meet or exceed the expected minimum standard of
performance for their year of Foundation training. In order to progress to the next
stage of training Foundation doctors will be assessed at the end of each year of
8training. Progression will be dependent on having met or exceeded the minimum
expected standard of performance in each of the 20 foundation professional
capabilities.
Personal Development Plan
Foundation doctors must have a personal development plan for each placement
within each foundation year. This should be agreed with your educational supervisor
and recorded in your ePortfolio at the beginning of each placement. The PDP should
set out the specific aims and learning objectives and be based on the requirements
for satisfactory completion of the Foundation Programme. It should also be mapped
to the Curriculum. The PDP should form the basis of all appraisal discussions.
SUPERVISED LEARNING EVENTS, ASSESSMENTS AND ARCP
The key principles of the assessment and supervised learning events (SLEs)
process is to:
Provide immediate feedback
Highlight achievements and areas of excellence
Promote regular feedback
Supply and demonstrate evidence of progression linked to the Curriculum
Suggest areas for further development
Identify doctors who may need additional help
SLEs are not assessments, you cannot pass nor fail an SLE, but you can fail to
engage with the learning process. You, with the support of your supervisors, are
responsible for arranging SLEs.
SLEs use the following tools:
Mini-clinical evaluation exercise (Mini-CEX)
Direct Observation of Procedural Skills (DOPS)
Case Based Discussion (CBD)
Developing the Clinical Teacher
Each type of SLE must cover a spread of different acute and long-term aspects of
patients’ conditions.
A different teacher/trainer should be used for each SLE wherever possible, including
at least one at consultant or GP level per placement. Others can include SASG,
SpR, StR (level 3 or above) doctors, nurses or an appropriate allied health
professional. The educational or clinical supervisor should also perform an SLE.
Teachers/trainers should have sufficient experience of the area under consideration,
typically at least higher specialty training (with variations between specialties); this is
particularly important with CBDs.

Each SLE must be demonstrated using the appropriate form on the ePortfolio
Mini CEX (Clinical Evaluation Exercise)
Mini CEX is a snapshot of a doctor/patient interaction. It is designed to assess the
clinical skills, attitudes and behaviours of trainees essential to providing high quality
care.
Each Mini CEX must represent a different clinical problem, sampling one of the
acute care, chronic illness, psychiatric care etc.
A minimum of 3 Mini CEX or 2 Mini-Cex plus 1 DOPS assessment forms need to
be completed each rotation
DOPS (Direct Observation of Procedural Skills)
This is a structured checklist for giving feedback on the foundation doctor’s
interaction with the patient when performing a practical procedure.
This is optional to supplement mini-CEX.
10 Different assessors should be used for each encounter where possible
CBD (Case-based Discussion)
This is a structured discussion with your supervisor about a clinical case you have
been involved in. It is designed to assess, in particular, clinical reasoning and
judgement and will focus on the actual written record in a patient’s notes.
Each CbD must represent a different clinical problem sampling one of the acute
care, chronic illness, psychiatric care etc
Different teachers/trainers should be used for each CbD
A minimum of 2CbDs need to be completed each rotation
Developing the Clinical Teacher
This is a tool to aid the development of a foundation doctor’s skill in teaching and/or
making a presentation and should be performed at least once a year. The foundation
doctor will be encouraged to develop skills in preparation and scene-setting, delivery
of material, subject knowledge and ability to answer questions, learner-lead and
overall interaction with the group. The Developing the Clinical Teacher form on the
ePortfolio must be used to demonstrate this.
ASSESSMENTS
There are 5 different types of assessments:
Core Procedures (F1 only)
Team Assessment of Behaviour (TAB)
Clinical Supervisor’s End of Placement Report
Educational Supervisor’s End of Placement Report
Educational Supervisor’s End of Year Report
Patients cannot complete any of the assessments.
Core Procedures
The General Medical Council requires demonstration of competence in a series of
procedures in order for a provisionally registered doctor with a licence to practise to
be eligible for full registration.
It is a requirement that the foundation year one doctor provides evidence within the
ePortfolio of satisfactory performance of each core procedure.
F2 doctors do not need to complete core procedures.
TAB (Team Assessment of Behaviour)
This assessment uses a questionnaire to collate views on how you are doing from a
range of co-workers, as well as a self-rating form. A minimum of 10 satisfactory
returns are required. You must also complete a Self-TAB before you can ask for
assessments from others.
The completed TAB can only be released by your Educational Supervisor.
11One complete TAB should be completed in your first placement; only one TAB
per year is required unless you are requested to complete additional TABs by
your educational supervisor or FP Tutor.
The mix of raters/assessors must include at least:
2 consultants or trained GPs. The named clinical supervisor should normally
be used as an assessor. However, there may be occasional circumstances
where this is not possible, hence, inclusion of the CS is not mandatory.
1 other doctor more senior than F2.
2 Senior nurses (band 5 or above)
2 allied health professionals / other team members including ward clerks,
secretaries and auxiliary staff.
For each TAB, the foundation doctor should approach at least 15 raters/assessors.
Up to two foundation doctors may be used but attention must be paid to ensure an
appropriate mix of raters or the TAB will be invalid
Clinical Supervisor’s End of Placement Report
Towards the end of each placement, the foundation doctor and clinical supervisor
will meet to complete a summative assessment of the foundation doctor’s overall
performance and progress in the placement. The outcome of this assessment should
be agreed by both the foundation doctor and the clinical supervisor and recorded in
the doctor’s ePortfolio using the Clinical Supervisor’s End of Placement Report form.
Educational Supervisor’s End of Placement Report
The educational supervisor’s end of placement report requires review of the clinical
supervisor’s report along with the evidence provided within the ePortfolio and any
other source. The outcome of the final assessment discussion should be agreed by
both the foundation doctor and the educational supervisor and recorded in the
doctor’s ePortfolio Educational Supervisor’s End of Placement Report form.
Educational Supervisor’s End of Year Report
This is an overall professional assessment and judgment of the foundation doctor.
Placement reports are drawn together by the educational supervisor in an
Educational Supervisor’s End of Year Report. This will form the basis of the
Foundation Tutor’s recommendation regarding satisfactory completion of F1 and F2.
Annual Review of Competence Progression (ARCP)
Both F1 and F2 doctors will have an ARCP towards the end of each foundation year.
The ARCP provides a formal process for reviewing foundation doctors’ progress
which uses the evidence gathered by them and supplied by their supervisors.
Foundation doctors will be given notice to ensure that their ePortfolios are up to date
and so that they can meet with their educational supervisor to complete the required
reports, including the end of year report.
12YEAR 1
The outcomes recorded following clinical performance reviews and evidence of
engagement in the educational process will feed into the overall performance report
at the end of the year (the Educational Supervisor’s End of Year Report.) The ARCP
Panel will review this report and your other documentation to inform your medical
school as to whether you should complete and issue the Attainment of Foundation
Year 1 Competence (AF1C). Once the certificate is issued, you will be eligible to
apply for full registration with the GMC. The GMC expects satisfactory achievements
in all domains set out in The Trainee Doctor (2011) and reproduced in the
Foundation Programme Curriculum syllabus outcomes.
YEAR 2
The ARCP’s Panel’s overall judgement of satisfactory completion of F2 will allow the
foundation doctor to be eligible to enter core, specialty or general practice training.
This judgement will include an assessment of a foundation doctor’s ability to take on
increasing levels of responsibility, and will be marked by the issuing of a Foundation
Achievement of Competence Document (FACD).
ARCP Outcomes
Achieving progress and competences at the expected rate – this is what you’re aiming for at

TEACHING AND EDUCATION
All foundation doctors must have ‘in-house’ formal education as part of their working
week, which should be relevant and protected (bleep free). At times teaching
sessions may be combined and there will be planned whole/half days of generic
training.
YEAR 1
13In order to be signed off at the end of the year, you must achieve at least a 70%
attendance rate made up from MANDATORY teaching. The calculation for the 70%
attendance is as follows:
Number of hours attended x 100 = attendance %

MANDATORY TEACHING
Mandatory F1 Wednesday Teaching Programme held on alternate weeks in the
Darlington Memorial Hospital Education Centre and Prospect House Learning
Centre at Durham. The teaching takes place on Wednesday from 12.30-2.30pm.
You will be required to attend the following;
Wednesday Teaching Programme = 2 Hours per week (If no teaching is arranged
for your site use the 2 hours protected time to complete e-Learning, ePortfolio,
SCRIPT)
Simulation Training = 15 hours
You will be offered 2 sessions of simulation training which is run at the Clinical
Simulation Centre at Bishop Auckland Hospital, spaces are limited to 12 trainees per
session, sign up to a session as soon as possible.

This workshop is aimed at doctors contemplating which specialty to apply for.
Falling Below the 70% Teaching Attendance
Foundation doctors are required to attend at least 70% of the teaching provided
outside of nights, leave, study leave, etc. However, if your attendance level overall
falls below 70% you must complete self-directed learning to make up for the shortfall.

Educational Supervisor before a % is recorded by the Medical Education Team.

STUDY LEAVE
In line with the UKFP Reference Guide May 2017 the generic teaching programme
and study leave are provided to support the objectives of the Foundation
Programme. In particular, both the generic teaching programme and study leave
should be used to support the acquisition of the outcomes set out in the FP
Curriculum and to enable Foundation doctors to explore career options.
Each Local Education Provider (LEP) must provide a mandatory generic teaching
programme in both F1 and F2 which is mapped to the curriculum. These
programmes are often referred to as ‘Professional Learning Programmes’ and a
minimum target of 70% attendance is specified for each year.
Foundation doctors training less than full-time are entitled to a pro-rata access to
study leave funding and time allocations.

SUPPORT
What to do if you need help or support?
If you do have concerns about your training, it is important that you contact either
your educational or clinical supervisor immediately. This should be done in a formal
setting. If you are still not happy after having spoken to the educational or clinical
supervisor, please contact a foundation programme Tutor or a member of the
Medical Education Team (details on the Contact section).
There are also services and organisations who deal with doctors in personal trouble.

Medical Education Centres
Clinical Simulation Centre Bishop Auckland Hospital (BAH) DL14 6AD
Situated on the Ground Floor of the hospital, the Centre comprises of a Simulation
Suite, De-Brief Room and 4 Clinical Skills Rooms. This is a state of the art facility
and has SIM Man, SIM Mum, SIM Junior and SIM Baby. The centre is used for inter-
professional simulation and human factors training. There are various Clinical Skills
drop in sessions arranged throughout the year for the Foundation Programme and
training specifically designed for higher specialty trainees, as well as ALS, ILS and
NLS. If you have any questions regarding SIM facility please contact the reception
on Ext: 55105.
Education Centre, Darlington Memorial Hospital (DMH) DL3 6HX
Situated on the ground floor, the Centre comprises of 3 Seminar Rooms, Clinical
Skills Room, Common Room, Lecture Theatre, Library and IT room. Assistance is
available at the reception area. The Library provides access to thousands of print
and online resources. A computer room with a Cotag system card, 24-hour access to
the Library and WIFI service is available.

There is a new junior Doctors Mess at DMH. The mess is situated on the 4th Floor,
behind the lifts. This new facility has:

Prospect House, University Hospital of North Durham (UHND) DH1 5TW
The Education Centre is currently situated at Prospect House, DH1 5TS Aykley
Heads Business Centre (a short walk from A&E). The centre is comprised of 7
Seminar Rooms (3 of which convert to a lecture theatre), Library, IT Rooms,
Radiology Training Room, Clinical Skills Room and a coffee area. The Library
provides access to thousands of print and online resources. Guest WI-FI is available
by obtaining a username and password from the Library. Library is open 09.00-
17.00hrs Monday to Thursday and 09.00-16.30hrs Friday. All enquiries Ext: 38600

MESS Room UHND

BAH Opening Hours Monday to Friday: 10.15-12.00noon and 12.30-14.30hrs
Post room is situated next door to the ISS helpdesk on the ground floor at the
delivery entrance. Mail may be left or collected during the opening times, however
outside of these times there is a Post box so post can be dropped off at any time. If
the mail is addressed to the doctor and the ward then it is delivered to that ward. All
collected mail must be signed for.
DMH Opening Hours: Monday to Friday: 08.00-16.00hrs. Post room is situated on
the ground floor, opposite the Renal Dialysis Unit, next to entrance C. Mail may be
left or collected during the opening times stated. If the mail is addressed to the
doctor and the ward then it is delivered to that ward.
UHND Opening Hours: Monday to Friday: 07.00-09.00hrs, 9.30-12.30hrs and 13.00-
15.30hrs. Post room is situated on the Lower ground floor adjacent to the stores
department. Mail may be left or collected during the opening times, however outside
of these times there is a Post box so post can be dropped off at any time. There is
an on-site delivery round to wards and departments twice daily.
