journey!                                                         Be Well Derby is an incentive-based wellness program
                                                                 designed to encourage employees to live a healthy

                                                                 successfully achieve Platinum, Gold or Silver status will

                                                                 The table below outlines how many wellness points
                                                                 must be earned to achieve each status. New employees
                                                                 will receive a prorated point total.
