The Wisconsin Optometric Association 2021 Primary Care Symposium will be held at the
Glacier Canyon Lodge in Wisconsin Dells on October 29-30. The classes will begin at 6:00 p.m.
on Friday and will run until 3:15 p.m. on Saturday. This will be the final education seminar
provided by the WOA in 2021 and the last CE event of the current licensing biennium. Ten
hours of excellent continuing education will be offered.

Registration Information:
To register at early bird rates for the Primary Care Symposium, please complete the registration
form and return it to the WOA on or before October 4, 2021. There are two options for
registration: (1) Full registration, which includes all education and meals (you must still check off
each class and meal you plan to attend), or (2) Partial registration, which allows you to choose
courses and meals individually. The regular registration deadline is October 22nd. If you
are currently not a member of the WOA, but would like to join, please complete and submit
your membership application via email to WOA Assistant Executive Vice President Ms. Joleen
Breunig at joleen@woa-eyes.org or by fax at 608-824-2205. For registration questions, you may
email Ms. Breunig.

There is a room block set aside at the Glacier Canyon Lodge with a rate of $100 per night for
Friday and $119 for Saturday for a double queen or $150/$205 for 2 bedroom Deluxe. Call the

block will be released September 28, 2021, and group rates are not guaranteed after that date.

                     Featured Speakers
                        Mark T. Dunbar, O.D., FAAO
                        Dr. Mark Dunbar has been a member of the optometric staff at the
                        University of Miami’s Bascom Palmer Eye Institute for over 30 years
                        and currently serves as the Director of Optometric Services. He is
                        also Co-Chairman of the Conference Advisory Board for Vision Expo.

                      Dr. Dunbar is a Fellow of the American Academy of Optometry and is
                      a founding member of both the Optometric Glaucoma Society (OGS)
                      and the Optometric Retina Society (ORS). Dr. Dunbar serves on the
                      Editorial Boards for Review of Optometry where he writes the month-
ly column “The Retina Quiz.”

                        Mitch Ibach, O.D., FAAO
                          Dr. Mitch Ibach is a residency trained optometrist at Vance Thomp-
                          son Vision in Sioux Falls, SD. Dr. Ibach attended the Pacific Universi-
                          ty College of Optometry where he graduated summa cum laude.
                          Mitch completed his residency training at Minnesota Eye Consultants
                          with a concentration on cornea, refractive surgery, external disease
                          and glaucoma. In September of 2014, he joined Vance Thompson
                          Vision to focus on advanced anterior segment surgery care and pa-
                          thology. Mitch is a fellow of the American Academy of Optometry,
                          and an Intrepid Eye Society member, a member with the American
Optometric Association, Optometric Glaucoma Society (OGS), and the South Dakota Opto-
metric Society. He actively teaches in the Vance Thompson Vision residency and is also an
adjunct clinical faculty for the Illinois College of Optometry and the Pikesville College of Op-
tometry.

                       Nathan Lighthizer, O.D., FAAO
                       Born and raised in Bismarck, North Dakota, Dr. Nathan Lighthizer,
                       O.D., F.A.A.O., is a graduate of Pacific University College of Optome-
                       try. Upon graduation, he completed a residency in Family Practice
                       Optometry with an emphasis in Ocular Disease through Northeastern
                       State University Oklahoma College of Optometry. Dr. Lighthizer has
                       since joined the faculty at the Oklahoma College of Optometry and
                       serves as the Chief of Specialty Care Clinics and the Chief of Elec-
                       trodiagnostics Clinic. In 2014, he founded and now heads the Dry
                       Eye Clinic at the College of Optometry. He was recently named the
Director of Continuing Education as well as the Assistant Dean for Clinical Care Services at
the Oklahoma College of Optometry. Dr. Lighthizer lectures nationally on numerous topics,
most notably advanced ophthalmic procedures, electrodiagnostics, and ocular disease.

Things You Need To Know:
   All courses will be filled on a first-come, first-served basis.

   Please select either “Full Registration” or “Partial Registration.”

   There are two choices for registration:
                Full Registration: One fee, including your choice of meal and all education classes.
                Partial Registration: You choose which classes and meal you want to attend, and
                each is priced separately. Please be aware that even if you aren’t taking all of the
                classes, sometimes doing the full registration is cheaper. Simply select which
                classes you want to take, and pay the full registration fee.

   If you plan to attend all of the WOA Primary Care Symposium, full registration is the way to go. If you can
    only attend a class or two, your best option may be partial registration. Choose the registration option
    which provides the best price.

   Incomplete forms will delay processing. Please make sure you have provided all necessary information
    and type or print clearly on the form.

   Payment must be included with registration forms!

   Whether you do full or partial registration, you must check which courses and meals you plan on
    attending. You will only receive tickets for classes and meals that are marked.

   Confirmation notices will be sent at least two weeks prior to the seminar to all doctors who have pre-
    registered by the early bird date, listing which classes and meals you have selected. This information will
    be sent via email or fax to registrants.

   All classes and meals will require either a ticket or name badge for admittance.

   Early Bird Registration deadline is Monday, October 4, 2021.

   Registration deadline is Friday, October 22, 2021.

   No registrations will be allowed on-site.

Refunds:

   Doctors who pre-register and later discover they are unable to attend may obtain a full refund (less a $30
    administrative fee), if their written notice of cancellation is received in the WOA office on or before October
    22, 2021.

   No refunds will be provided for cancellations received after October 22, 2021.
