When You Turn 65 or Become Eligible for
    Medicare
When you or any of your eligible dependents turns 65 or becomes eligible for Medicare, you enroll in health
care coverage through Via Benefits, a private Medicare exchange. Via Benefits offers medical, prescription
drug, dental and vision coverage. Via Benefits does not provide coverage until you (and/or your covered
    dependents) become eligible for Medicare.

There are separate elections for Medicare-eligible participants and their Medicare-eligible family members,
because the coverage through Via Benefits is individualized coverage. Elections are made approximately 60
days before you become eligible for Medicare due to turning age 65. If you become eligible for Medicare
sooner than age 65, please contact HR Answers. Once you are eligible for Medicare, JPMorgan Chase coverage
    will be discontinued.

If you have questions about the Medicare-eligible coverage through Via Benefits, they can be reached at
1-844-448-7300, 8 a.m. to 9 p.m. Eastern Time, Monday through Friday. You may also access the Via Benefits
website for JPMorgan Chase members at https://my.viabenefits.com/jpmc.

Have you been disabled for more than 29 months?

You may qualify for Medicare benefits. If so, under the terms of the JPMorgan Chase Medical Plan,
Medicare would be considered the primary source of medical coverage, and coverage would no longer be
offered through JPMorgan Chase. You would then be eligible for health care coverage through Via Benefits.
Call HR Answers (see page 13) to update your status so your information may be provided to Via Benefits.

What’s Changing for 2022
No major changes to the Medical Plan
Great news! For 2022, there are no major changes in the types of services covered under the JPMorgan Chase
Medical Plan with Aetna and Cigna or the Prescription Drug Plan with CVS Caremark.
In 2022, the Medical Plan will continue to offer:
• Eligible in-network preventive care (including physical exams and recommended preventive screenings)
and eligible preventive generic drugs covered at 100% with no deductible, coinsurance or copayments.
• In-network primary care office visits covered at 90% with no deductible.
• The same deductibles and coinsurance maximums.
• The same types of covered medical services.

2022 benefits changes and enhancements
Benefits enhancements and changes include:

• Infertility benefits support: As of July 1, 2021, if you are enrolled in the JPMC Medical Plan, you and your
covered family members can receive help from WINFertility nurses with selecting a high-quality provider,
understanding treatment options, and providing clinical and emotional support. Enroll with WINFertility
and complete a nurse consultation in order to have access to a $30,000 medical lifetime maximum benefit
($10,000 medical benefit if member does not engage with WINFertility). Medical benefits (e.g., provider
network, claims administration) will continue to be managed by your health care company — Aetna or
Cigna. Prescription drug benefits will continue to be managed by CVS Caremark. You can reach WINFertility
by calling 1-833-439-1517, Monday through Friday, 9 a.m. to 7:30 p.m. Eastern Time.
• Prescription Drug Plan: Review the 2022 formulary drug lists for the latest information on covered drugs.
Remember to check frequently throughout the year for up-to-date information.
• Beginning January 1, 2022, if you are enrolled in the JPMC Core Medical Plan and move to Arizona or Ohio
during the calendar year, you’ll automatically be enrolled in the JPMC Simplified Medical Plan, effective
when you submit your address change. Please note that your health care company (Aetna or Cigna),
election (Option 1 or 2) and dependent coverage will remain the same and cannot be changed as a result of
      your move.
• Group Personal Excess Liability Insurance (PEXL) Plan: Claims administration will transition to Marsh
McLennan Agency as of January 1, 2022. Plan benefits remain the same, including underlying coverage
requirements, and will continue to be offered by The Chubb Insurance Company. If you’re currently
enrolled, your 2021 election will carry over to 2022. See PEXL details on page 5.
• 2022 MRA Action Plan: New resilience training through meQuilibrium and My Finances and Me activities,
which you can complete to earn funds in your 2022 MRA. See MRA details on page 4 for more
      information.
• Changing health care companies. If you change health care companies (Aetna or Cigna) for 2022, your
2021 MRA balance will automatically transfer to your new health care company in April 2022.

A word about dependent eligibility

If you are enrolled in benefits, you may also cover eligible dependents (e.g., spouse/domestic partner,
children). Once you add a dependent to your coverage, you will be notified and asked to provide copies of
documents, such as a marriage certificate/domestic partner registration, birth certificate, etc. Children are
eligible until the end of the month in which they turn age 26. For more on eligibility requirements, please
see the Summary Plan Description on My Health.

If you are adding a dependent to coverage, you will need to provide that dependent’s Social Security number
    (SSN) during enrollment.

Medical Plan surcharge for unvaccinated/undisclosed employees
The Operating Committee has recently communicated that employees who are not fully vaccinated against
COVID-19, or who have not disclosed their vaccination status, could have higher costs in the JPMorgan Chase
U.S. Medical Plan. More specifically, employees enrolled in the JPMC U.S. Medical Plan who are not fully
vaccinated against COVID-19 or have not disclosed their vaccination status (as indicated in the JPMC Vaccine
Record Tool) will pay additional dollars toward their medical payroll costs for 2022. For employees covered by
the Medical Plan with Total Annual Cash Compensation (TACC) less than $60,000, an additional $250 will be
deducted from their pay for Medical Plan coverage in 2022 ($10.41 per paycheck); for all other employees
covered by the Medical Plan, an additional $500 will be deducted from their pay for Medical Plan coverage in
2022 ($20.83 per paycheck).

For employees who are on approved JPMC long-term disability (LTD) leave on October 5, 2021, or later, the
surcharge will not apply while on LTD leave. However, upon return to active employment, you will be
subject to the surcharge 45 days following your return date if you have not entered your information into
the JPMC Vaccine Record Tool indicating that you are fully vaccinated. If you are not fully vaccinated or have
not disclosed your vaccination status within the 45-day period, an additional payroll deduction for medical
coverage of $10.41 or $20.83 (depending on your TACC) will apply prospectively (unless you have an approved
COVID-19 vaccine accommodation).

The medical contributions you see on the Benefits Web Center during annual enrollment do not reflect the
cost of this unvaccinated surcharge. Upon your return to active employment, if you do not complete the
necessary steps within the 45-day period, an additional payroll deduction of $10.41 or $20.83 (depending on
your TACC) will be withheld.

For this purpose, “fully vaccinated” is defined as:

• Having received two doses of the Pfizer or Moderna vaccine, or one dose of the J&J vaccine, and
• Having entered your vaccination information into the JPMC Vaccine Record Tool, including having
uploaded a copy of your vaccination card.

Answers at me.jpmorganchase.com.

employees residing in all states except Arizona, California, and Ohio.
No changes to the Prescription Drug Plan
You’ll continue to have prescription drug coverage administered by CVS Caremark®, regardless of the health
care company you choose. Additionally, there are no Prescription Drug Plan changes for 2022. That means you
won’t receive a new prescription drug ID card for 2022.

As always, you can check to see if your prescription drug is covered on CVS Caremark’s Covered Drug List
on the CVS Caremark website, which you can access through My Health or at www.caremark.com. Drug lists
are subject to change quarterly by CVS Caremark. If your drug is not on the list, ask your doctor if there’s an
alternative you can take.

Note: Your cost will be lower for generic and preferred brand name prescriptions (versus non-preferred brand
name or drugs that are not covered). If you use an excluded/not-covered drug, you will pay the full cost.

Your Medical Reimbursement Account
Your Medical Reimbursement Account (MRA) will be managed by the health care company you choose for
2022 — either Aetna (via Payflex, an Aetna Company) or Cigna.

Reminders:
• Your MRA is funded by JPMorgan Chase when you and your covered spouse/domestic partner complete
certain wellness activities. You cannot contribute to the MRA. MRA funds can only be used to pay for
eligible medical and prescription drug expenses.
• Payment methods: During Annual Benefits Enrollment, you can choose to have expenses paid from your
MRA through automatic claim payment or with a debit card.

4 | 2022 Pre-Medicare-Eligible LTD Annual U.S. Benefits Enrollment Bulletin for the Core Medical Plan Pre-Medicare-eligible LTD
employees residing in all states except Arizona, California, and Ohio.
Automatic claim payment vs. debit card
You have two ways to pay claims from your MRA: Automatic claim payment and debit card. During Annual
Benefits Enrollment, you’ll be able to select one of these payment methods:

 • Automatic claim payment method. Both Aetna/Payflex and Cigna will automatically use your MRA
funds to pay for your portion of eligible out-of-pocket medical and prescription drug expenses.

 • Debit card payment method. You can choose to use your debit card or pay with your personal funds for
eligible expenses — giving you more control over when you’re using your MRA funds. Keep in mind that
you’ll need to keep your receipts and be prepared to substantiate any debit card claims, as required by
   the IRS.

If you’re currently enrolled in the Medical Plan and do not change your payment method election or take
no action during Annual Benefits Enrollment, your 2021 payment method will carry over to 2022.

To make a change to your 2022 payment method, select View/Change on Your Benefit Summary.
Please keep in mind that automatic claim payment assumes you did not pay your portion of your eligible
medical expense at the time of service and pays your provider/facility your portion directly. If you pay your
portion at the time of service, your provider/facility will be double-paid — once from you at the point of
service and once from your MRA. It will be your responsibility to recoup the overpayment from your
provider/facility (although your health care company can try to assist you). Therefore, if the providers or
facilities you visit are requiring you to pay at the point of service, the automatic claim payment method
may not be right for you; you should consider the debit card payment method.

Learn more about your payment options on My Health.

Group Personal Excess Liability Plan
The claims administrator for Group Personal Excess Liability (PEXL) Insurance will transition to Marsh
McLennan Agency as of January 1, 2022. Plan benefits, including underlying coverage requirements, remain
the same and will continue to be offered by The Chubb Insurance Company. Further, your election will
automatically carry over to 2022, if you do not make an election otherwise.

Below is a preview of services available to enrolled JPMorgan Chase members through Marsh McLennan
Agency:

     • Complimentary Personal Risk Management Review: Enrolled JPMorgan Chase members can elect to
receive a complimentary personal risk analysis from a Marsh McLennan Agency personal risk advisor.
Through this consultation, a personal risk advisor will evaluate the adequacy of your existing policies,
identify exposures, and determine whether there are sufficient underlying liability limits in your
current coverage. The advisor will also provide customized recommendations to improve the
effectiveness of your coverage and ensure you have the broadest coverage available for the best
         value.

5 | 2022 Pre-Medicare-Eligible LTD Annual U.S. Benefits Enrollment Bulletin for the Core Medical Plan Pre-Medicare-eligible LTD
employees residing in all states except Arizona, California, and Ohio.
• Liability Estimator Tool: You’ll have access to a confidential online liability estimator tool to quickly
get a preliminary estimate of the liability coverage amount that may be appropriate for you.
    • Claims Advocacy: In the event of a claim, Marsh McLennan Agency will serve as your advocate
throughout the claims process. The highly experienced team of personal insurance Field Claims
Executives and Claims Analysts are dedicated to:
o Proactively help you get a prompt and fair settlement from insurers
o Provide clear and timely information about your claim status
o Advocate on your behalf with insurers in complex cases

Visit the Benefits Web Center to learn more or to enroll. Please note: Claims you have through December 31,
2021, should be reported to Arthur J. Gallagher. If you have an existing PEXL claim, continue to work with your
Arthur J. Gallagher claim representative. You’ll separately receive information from Marsh McLennan Agency
about the transition process if your claim isn’t resolved by December 31, 2021.

Definition and use of Total Annual Cash Compensation (TACC)
Several features of the Medical Plan vary based on an employee’s Total Annual Cash Compensation (TACC),
such as the monthly contribution cost, deductible level and annual out-of-pocket coinsurance maximums.
TACC is calculated based on:
• Your annual rate of base salary plus applicable job differential pay (for example, shift pay) as of each
    August 1, plus
• Any cash earnings from any incentive plans (for example, annual incentive compensation, commissions,
draws, overrides, and special recognition payments or incentives) that are paid to or deferred by you for
the previous 12-month period ending each July 31.
• Overtime is not included.

For purposes of determining the Medical Plan pay tier that applies to you, your TACC is recalculated as of each
August 1 to take effect the next January 1 and will remain unchanged throughout the year. For most
employees hired on or after August 1, TACC will be equal to base salary plus job differentials.

For LTD participants, your level of TACC was frozen on your last date of active employment with JPMC and will
remain at that level throughout your LTD period. You can find your TACC on the Benefits Web Center through
My Health. Just click Your Profile > Personal Information > Personal Details.

Wellness Rewards
A key part of our Medical Plan and wellness strategy is to engage employees in specific wellness activities by
providing Medical Reimbursement Account (MRA) funds. JPMorgan Chase contributes money to your MRA to
help you pay for eligible out-of-pocket medical and prescription drug expenses (deductibles, coinsurance and
copayments).

Consider Completing both your Wellness Activities by November 19, 2021
(11:59 p.m. Eastern Time)
Last year, we paused on asking our employees and their spouses/domestic partners to complete biometric
Wellness Screenings as part of our annual wellness campaign. Plus, many of us delayed receiving other
preventive care, like annual physicals and health screenings. We believe it’s important to return to more usual
routines when it comes to your health — that includes preventive care.

employees residing in all states except Arizona, California, and Ohio.
Because knowing their numbers (e.g., blood pressure, body mass index [BMI], blood sugar, cholesterol,
triglycerides and A1C) has helped so many people understand their health, uncover health risks and receive
earlier treatment, biometric Wellness Screenings are back.
This means if you complete both the biometric Wellness Screening and online Wellness Assessment by
November 19, 2021 (11:59 p.m. Eastern Time), you will earn $200 in your 2022 Medical Reimbursement
Account (MRA).
If your covered spouse/domestic partner completes both of these activities by the deadline, you will earn
another $100 in your 2022 MRA.

If you’re currently on an approved Leave of Absence, including long-term disability, we encourage you and
your covered spouse/domestic partner to participate in our Wellness Rewards program. However, if you
are on an approved Leave of Absence, including long-term disability, for at least 45 consecutive days
between September 1 and November 19, 2021, and do not complete your online Wellness Activities during
that period, then you will not lose the $500 in 2022 medical contribution savings ($1,000 if covering a
spouse/domestic partner). Other provisions of the Medical Plan and Wellness Rewards program will
continue to apply, including the opportunity to earn MRA funds by completing wellness activities.

How to complete the biometric Wellness Screening
You have several options to complete your biometric screening, details of which can be found on the Wellness
Screening Portal run by Quest Diagnostics.

Options available to you if you have COBRA coverage under the JPMorgan Chase Medical Plan are:
         • Download a Physician Wellness Screening form and take it to your doctor. Have them complete
ALL biometric fields (plus Date of Service) and sign the form. You can submit the completed
form via fax or upload it to the Wellness Screening Portal. OR
         • Click on the Quest Patient Service Center section to locate and schedule a screening at a lab or
retail location (advance registration is required). OR
         • Download and print a CVS MinuteClinic Screening voucher and take it to a CVS MinuteClinic for
              your screening.

How to complete the online Wellness Assessment
Go to My Health > Complete your Wellness Assessment or go directly to mycigna.com.
Everyone will complete the online Wellness Assessment with Cigna — even
if their health care company is Aetna. Have questions? Contact

LTD individuals: The first time you access mycigna.com, you’ll need to
register and provide your name, date of birth, ZIP code and Social Security
number (or Cigna Member ID if Cigna is your health care company).
Covered spouses/domestic partners: You will need to register on mycigna.com as yourself, if you haven’t
already. You’ll be asked to provide the JPMorgan Chase former employee’s Social Security number and ZIP
code, as well as your name and date of birth.

Additional Wellness Activities
Changes for 2022:
• Online training, Mindfully Managing Stress online training through meQuilibrium (for employees only):
Earn $50 in your MRA and learn about the powerful science of resilience and techniques to help gain
control over your emotions, thoughts and stress.
• My Finances and Me activities: Earn $50 in your MRA by (1) completing a short online Financial Wellness
Assessment* and (2) attending a group education session on a variety of financial topics, such as basic
money management, navigating student loans and retirement planning. Please note: Spouses/domestic
partners can participate, but cannot earn MRA dollars for doing so.

*The Financial Wellness Assessment is not part of the online Wellness Assessment that, along with the biometric Wellness Screening, is
part of the Initial Wellness Activities.

Don’t forget your 2021 MRA funds!

Plus, if you cover a spouse/domestic partner, you earn up to an additional $300 in your MRA when they

*If you commenced a short-term disability leave prior to January 1, 2011, immediately preceding your current LTD claim, this rule
