April 2021 TOPICS Discussion Topics 1. Peripheral Vascular Disease (PVD) • Be sure abnormal findings in ROS/PE are validated w/diagnosis • Accurately report PVD when present • If patient is also diabetic, link PVD to the diabetes 2. Medication Linking • Medication list should always be paired with problem list • In HPI or Assessment, indicate when a med is being used to treat a condition • Meds that treat multiple conditions have to be linked 3. Quarter 4 2020 UDC Payments • Payments are being finalized and will be sent out within the next 5 business days • Final 2020 Progress Reports will also be posted to NaviNet for practices to access within the same period 4. UDC Portal Access • All users needing access must be set up before they are able to login • Access can be obtained by emailing UDCHelp@highmark.com providing the Users Name, email address, NaviNet User Name, Practice info • Users no longer needing access –email UDCHelp so they can be removed 5.
