Changing Your Benefits

• Remember, your Open Enrollment elections will
 be in effect January 1 – December 31, 2020.
• You will not be able to make mid-year changes
 unless you have a “Qualified Status Change,”
 which can include (but isn’t limited to) marriage,
 domestic partnership, divorce, birth or adoption
 of a child, or death of spouse or child.
• Changes are allowed within 30 days of a
 qualifying event.

ID Cards
Medical
§ You will not receive a new ID card if you are not making any plan changes.
§ If you newly enrolled in UHC or did a plan change, you will receive your new ID card in
  the mail in January.
§ To access a temporary UHC medical card, please visit www.myuhc.com, or download
  the free UnitedHealthcare.

Dental & Vision
§   No ID cards. Please use your SSN with your provider.
§   MetLife Dental – you can register at www.metlife.com and print a personalized ID card.
§   VSP Vision - you can register at www.vsp.com and print a personalized ID card.

Flexible Spending/Health Savings Account
§ If you enrolled in the FSA and/or HSA plan, you will receive a new debit card from
  UHC/ Optum Bank, our new administrator for 2020.
§ Download the free UnitedHealthcare app (FSA) and the Optum Bank app (HSA) to
  access your benefits, view contributions and submit claims.
§ 2019 FSA claims should continue to be submitted to Navia, not Optum. Submit by
  March 31, 2020!

How to Enroll or Make Changes

• Refer to Treehouse and an e-mail sent from Total Rewards on instructions
 for how to make changes within Workday
• Visit www.ancestrybenefits.com, under Open Enrollment for additional
 information
• Complete all enrollments or changes by November 18, midnight PST
• All changes are effective January 1, 2020
