Bookings will close on Friday 18th December 2020. Bookings
will not be taken without a completed booking form.
Bookings are not refundable or transferable between days so please choose carefully.
Bookings received after Friday 18th December 2020 will incur a late penalty fee of $10.00.
Fees will be deducted on Wednesday 13th January 2021.
Vacation Care payments will be charged to accounts during week 1 Vacation Care 2021.
Please ensure there are sufficient funds in your account to cover fees.
PARENTS PLEASE PROVIDE EACH DAY:
• A Healthy Morning Tea and Lunch each day(unless stated on program)
• A refillable and labelled drink bottle each day
($2.00 charge for any water bottles purchased on excursions)
• A hat is compulsory for outside play
• Enclosed shoes ie: sneakers or joggers
• Spare change of clothing
EHOOSH PROVIDES
• Breakfast daily until 9.00am (except excursion days, please see program)
• Afternoon tea and fresh fruit daily
IMPORTANT PARENT REMINDERS
• Ensure you sign your child/children in and out every day. This is a legal requirement. Please do not
drop your child off at the school gate
• Notify the service in writing if someone other than yourself will be picking up your child
• Notify the service if your child will not be attending any of their booked days
• EHOOSH has a No Toys from Home Policy. We do not accept responsibility for any toys or valuable
belongings brought into the service
CHILDRENS LUNCH BOXES
Please pack a healthy lunch for your child with lots of food and a refillable drink bottle.
Reminder children who attend Vacation Care have allergies to certain products these include;
NUTS, SESAME SEEDS, KIWI FRUIT, AND ORANGES
Please do not send your child with THESE ingredients in their lunch boxes. This also includes labels on
food products, please read the back of the packets carefully:

EXCURSIONS
It is compulsory for children to wear an EHOOSH excursion hat and t-shirt. These items can be ordered
with your booking at an additional cost of $10.00 for a hat and $12.50 for a t-shirt. If you book in for an
excursion day, your child must attend the excursion. The service will be closed during the hours of the
excursion. EHOOSH will provide yellow rash vests for water excursions to ensure that supervision is easier. Children MUST wear this yellow rash vest at all times whilst in and around a pool of water. PROGRAM Please refer to the program for details of each day’s activities. The main activities will run between 9.00am – 3.00pm. Before and after these times we will have less structured indoor and outdoor play and relaxation time. Please note that the program described here is dependent upon enrolment numbers, and is subject to change. SUN SAFETY Please make sure that your child comes to Vacation Care with sunscreen already applied, or help yourself to the bottle on the sign in desk. Your child must bring a hat. Children without an appropriate hat will not be allowed to participate in outside play. Singlet tops and/or dresses with shoestring straps are not appropriate clothing according to our Sun Safe Policy. CLOTHING Please ensure your children’s clothing is labelled and that they wear enclosed shoes each day (joggers or sneakers). Children will probably get dirty during craft activities and/or playing outside so please have clothes that will cope with this. Please pack a change of clothing for water play or mud play.

1 2 3 We have included these incursions to offer children new experiences, which are in
line with our philosophy.
MONSTER SKATE INCURSION:
I give permission for my child / children to attend the incursion, Monster Skate, on Monday
11th January 2021. I understand that for this activity, children DO NOT need to bring their own
scooters and the Monster Skate staff will provide all safety gear, skateboards and scooters.
Children MUST wear enclosed footwear (no thongs, sandals etc.) and be dressed
appropriately for the activity. Children will be supervised by EHOOSH educators at all times.
ZOOM MOBILE ARCHERY INCURSION:
I give permission for my child / children to attend the incursion, Zoom Mobile Archery, on
Wednesday 13th January 2021. I understand that during this activity the children will be
guided through an archery lesson and will use normal field arrows provided by Zoom Mobile
Archery. I understand that the Zoom Mobile Archery staff are skilled instructors that will help
the children throughout the incursion. Children will be supervised by EHOOSH educators at all
times.
AUSTRALIAN WILDLIFE DISPLAYS:
I give permission for my child/children to attend the incursion Australian Wildlife Displays on
Thursday 14th January 2021. I understand that during this activity the children will be
interacting with animals and insects including but not limited to; Snappy the Crocodile,
Blossum the Possum and Princess the Python. The children will learn all about the animals
mentioned above and will be able to ask questions to the staff about the animals. I
understand my child/children must wash their hands before and after interacting with the
animals for health and safety reasons. Children will be supervised by EHOOSH educators at all
times.
BASE ZERO ROCK-CLIMBING:
I give permission for my child / children to attend the incursion Base Zero Rock Climbing on
Monday 25th January 2021. I understand that this incursion is in line with our philosophy of
‘Risky Play’ and that an appropriate risk assessment has been conducted. During this activity,
the staff from Base Zero will be using the automatic belay system to assist children in climbing
the wall. Children will be supervised by EHOOSH educators at all times.
ROCK POOL ADVENTURES:
I give permission for my child / children to attend the incursion Rock Pool Adventures on
Thursday 28th January 2021. Children will be given the opportunity to explore touch pools
with a wide variety of live creatures from the sea and be able to observe and learn about sea
creatures including but not limited to: sea stars, sea urchins, crabs, snails, sea cucumbers and
hermit crabs.

I give permission for my child / children to attend the excursion by private bus,
provided by Glenorie Coaches, to Event Cinemas at Top Ryde City Centre to
watch the newly released animated movie “Dragon Rider” which is rated PG, on
Monday 11th January 2021.
I understand my child will par�cipate in watching ‘Dragon Rider’ in a private
screening theatre with popcorn and a juice provided by Event Cinemas.
I am aware that the children will be away from the Service from 9:30am and return
by 1.15pm.
I understand that for this excursion my child/children will need to be wearing an
EHOOSH vaca�on care t-shirt and hat and that if they arrive at the service without
one they will be given a spare t-shirt and/or hat and my account will be charged
the fee of $10.00 for a hat and $12.50 for a t-shirt.
The ra�o for this excursion is one educator to eight children and the an�cipated
numbers for this excursion is sixty students with eight educators. Children will be
supervised by EHOOSH educators at all �mes. A risk assessment has been completed and

15th January 2021
I understand on this excursion the children will enjoy the parkland and join the
educa�on team for a morning full of water play games, dip-ne�ng by the lake and
leaf boat races.
I am aware that the children will be away from the service from 8:15am and return
by 3:30pm. We will have both morning tea and lunch at the venue.
I understand that for this excursion my child/children will need to be wearing an
EHOOSH vaca�on care t-shirt and hat and that if they arrive at the service without
one they will be given a spare t-shirt and/or hat and my account will be charged
the fee of $10.00 for a hat and $12.50 for a t-shirt.

such as ten-pin bowling, bumper cars, target prac�ce, arcade machine games
and more.
I understand at Sky Zone my child will par�cipate in ac�vi�es such as interconnected
trampolines and the Enduro Warrior Course and that socks will be provided by

bus, provided by Glenorie Coaches, to the Australian Walkabout Park in Calga
on Thursday 21� January 2021.
I understand on this excursion the children will interact with animals, hold a snake,
get their face painted using ochre paint and have morning tea and lunch in the park.
I am aware that the children will be away from the Service from 8:30am and return
by 3:30pm. We will have both morning tea and lunch at the venue.
I understand that for this excursion my child/children will need to be wearing an

provided by Glenorie Coaches, to Watsons Bay on Wednesday 27th January 2021.
I understand on this excursion the children will be par�cipa�ng in The Gap Walking
Trail with �aff from Kadoo Tours, and will experience an authen�c Aboriginal
culture tour that includes stories about local indigenous culture, a tradi�onal ochre
ceremony and enjoy some bush tucker
I am aware that the children will be away from the Service from 9.00am and return
by 3:00pm. We will have both morning tea and lunch at the venue.
I understand that for this excursion my child/children will need to be wearing an
EHOOSH vaca�on care t-shirt and hat and that if they arrive at the service without
one they will be given a spare t-shirt and/or hat and my account will be charged

Signature: _______________________________________________ Date:_________GENERAL INCURSION / EXCURSION PERMISSION NOTE I ________________________________________________________ hereby give permission for my child/children to participate in programmed activities and attend incursions/excursions as nominated on the program for the holiday period from Monday 11th January 2021 to Thursday 28th January 2021, as well as participate in all walking and use of contracted buses as required to attend excursions off the Service premises. (Note that the buses used may not be fitted with seat belts.) Please note the following conditions are specific to Vacation Care. I understand that: I have read the Vacation Care details provided above and EHOOSH Family Handbook and acknowledge that my child/children will be exposed to all normal risks associated with their participation in this program. My child/children must arrive by displayed time on program for excursion days. I understand that if my child/children arrive late, they will miss the excursion and will not be able to attend the service that day. I understand that a separate waiver form is required for ALL excursions and is to be filled out, signed and submitted BEFORE the date of the excursion Refunds cannot be made once bookings are confirmed by payment; bookings cannot be transferred between days. If children are unable to attend due to illness, fees are non-refundable. It is compulsory that all children wear the approved EHOOSH hat and t-shirt during excursions to maximize their safety. I understand that a hat and t-shirt will need to be purchased beforehand at an additional cost of $22.50. (Note that these items can be re-used in future Vacation Care.) If an unavoidable late change to the program occurs, a similar incursion will be substituted and parents will be notified as soon as possible I authorise my Vacation Care fees to be paid by direct debit during week 1 of the vacation care period in 2021 I understand that transport will be by private bus unless otherwise stated on the program. I am aware that risk assessments have been prepared for each excursion & incursion and that these are available at the Centre on request It is important that your child/children bring enough food and water with them (i.e. morning tea and lunch) Children are advised not to bring valuable items to the Service. Children are not permitted to bring mobile phones or electronic equipment to the Service. Educators take no responsibility for these items Parents are responsible for updating all parent contact and emergency contact details, including authorised pick- up persons Rules are established for the safety and enjoyment of all children, who are expected to respect and follow these rules. Breaches of these rules will result in children being asked to sit out from activities or, following consultation between the Nominated Supervisor and parents, from the rest of the program. Any critical incidents will result in immediate exclusion from the Service. On each EHOOSH excursion, we will carry a first aid kit, emergency contact numbers for families and the EHOOSH mobile phone. We will also ensure that at least one or more of the staff members present has an appropriate First Aid Certificate.
