Rob Thompson, known as “The Funny Magic Guy,” has been featured in corporate
events, theaters, and festivals across the nation. Having performed more than 200
shows, Rob has honed his skills to a level of perfection that is both mystifying and
entertaining. He is an “awe-inspiring magician with a sense of comedy that will delight

Brian “Fox” Ellis has been a full-time storyteller, historian, and Lincoln scholar for more
than 40 years. He has been performing as William Herndon for more than a dozen years
and portrays several friends of Lincoln, with more than 100 shows each year in Spring-
field. Fox is also a museum consultant who has worked with The Field Museum and the
Abraham Lincoln Presidential Library. He is the artistic director for Prairie Folklore Theatre,
a unique theatre company that celebrates ecology and history through original musical
theatre. Fox is the author of more than 20 books, including the critically acclaimed
Learning From the Land: Teaching Ecology Through Stories and Activities and The Web
at Dragonfly Pond.

Speaker Biographies
Pat Schou is the executive director of the Illinois Critical Access Hospital Network, the
first statewide critical access hospital network (established in 2003 and comprised of 57
CAH and small, rural hospitals) which provides a number of hospital support services and
educational programs. She also manages the Medicare Rural Hospital Flexibility Grant,
the Small Hospital Improvement Program, and several other grant programs on behalf of
the Illinois Department of Public Health. In addition, Ms. Schou is the executive director
of the Illinois Rural Community Care Organization, which is a statewide rural accountable
care organization comprised of 27 critical access and small, rural hospitals. She is also the
immediate past president of the National Rural Health Association.

This course is designed to provide health professionals with the opportunity to improve care
coordination for patients with chronic conditions and behavioral health issues, as well as
incorporate health promotion into their practice.
The education in this course is developed based on the “Seven Domains of Care Coordination in the
Primary Care Setting” by Kristy Baker, APRN-CNP, a Duke-Johnson & Johnson Nurse Leadership
Program Fellow. The course is divided into 10 modules and will be delivered in a virtual
environment using a combination of pre-recorded didactic presentations, asynchronous discussion
threads, and a virtual meeting space for a live monthly discussion with the instructors.
Objectives for the course include, but are not limited to:
• Developing patient-centered goals for effective self-management
• Identifying appropriate community resources to incorporate into patients’ care plan
• Conducting an annual wellness visit
• Analyzing research to incorporate into practice
Participants can earn 20 continuing educations credits through HealthTech S3, and a certificate
of completion will be provided upon successful completion.
Cost for ICAHN member hospitals is $150 per individual. Partial scholarships are available for
ICAHN member critical access hospitals. Interested individuals can sign up any time during the
