I will send you team assignment codes for coaches and players.
Online Registration:
Opens 5/27/2021 for U9-U10 X-League teams only
Opens 6/4/2021 everyone else
Registration is Due:
June 19h 10:00 PM
After the 6/20/2021 A $50 late fee will be charged, $10 will be added each additional day
Nothing will be accepted after June 28th at 5:00 PM.
*Late fee- only applies to late team, not individual players
Exception: High School girls (U16 or older) due approximately August 10th
New This Season:
The Alliance Development League: check out their website for all the information.
They offer a local gaming league that focus on developing players and giving back to the teams and clubs that
participate.
They will have divisions for birth years 2014 through 2012. They will offer both 2014 4v4 or 2014’s can play up
with 2013. *1 year play up is allowed with in all age groups.
Please address the following items:
TEAM Items:
• Roster an affinity generated roster signed by the head coach {There is a tab on the bottom of the
roster page – print team roster}
• Roster Size NEW THIS YEAR- Must have # of players on field +2 {see below for details bottom of
the roster that allows printing of the roster}
• Birth Certificates
o First time teams playing competitive- photocopies of birth certificate for All players {submit
these electronically in a file with the complete team included-do not have parents send
individual.}
o Submit any New players Birth Certificates- the birth certificate box will display a letter b
inside a circle if there is one in the system
• Fields: Communication with Field/city or league representative- allowing field usage and fees paid,
this can be an email chain, or copy of receipt.
• Multiple roster players. Turn in registration form, player action form and the fees.
• State teams only Alignment forms- (a google form will be sent to state teams to fill out)
Coach Items:
• Verification each coach has appropriate coaching license or waivers {I should be able to see this
completed on roster}
• A Concussion certificates uploaded for all team administrators must be dated 2021
• Safesport Training completed by all Administrative Staff –
• NO one will remain on the roster after July 5th without both certificates uploaded.
Player Registration:
To register a player the parent will need a team assignment code and the following URL: https://uysa-

The team assignment code will allow parents to place a player directly on the roster. Online registration will be
used to process player’s fees; Parents can use MasterCard, Visa, Discover, or E-check to make payments. If
you have parents that can’t remember their user name and password or don’t have one please have them
email me and request it. If parents are having a difficult time with registration make sure they have clicked the
blue registration tab at the top right and then enter their user name and password. Please feel free to contact
me with any questions or if you are having difficulties with registration. Any players registering after July 1st will
have to be accepted on the roster, please notify me and I will add them, otherwise they will remain in pending
on your roster.
Players proof of birth:
UYSA requires all players have a valid Birth Certificate copy or other legal proof of birth on file for every player.
Valid forms of Proof of birth are clear legible copies of: Birth Certificates (not hospital certificates), Military or
state id, driver’s licenses, or passports.
Coaching:
Every team competing as a competitive team is required to have at least 2 licensed coaches on the roster to
be eligible to participate.
Coaches Registration:
Coaches will need the team assignment codes to register. There is only one assistant coach code; it can be
used multiple times. During the online registration you will be required to fill out the coach’s code of ethics,
volunteer discloser You do not need to submit copies to the registrar.
Coaches License:
All Coaches within Challenger League and State Competitive Leagues are required to hold the following
license:
1. U9 Local or State coaches must hold a grassroots license*, E, or a D license.
Any State coach that coaches U14 or older state premier teams, must hold a D license or D waiver.*
South Cache Soccer League will reimburse coaches ¾ the registration fee upon completion of the grassroots
and ½ D course (please email a copy of your license and receipt for payment to the registrar)

License Waivers:
* A waiver is not a U.S. Soccer coaching license; the required coaching course must be completed to receive a
license.
To receive a waiver the following 3 steps must be completed.
1. Complete the Introduction to Grassroots in U.S. Soccer's Learning Center
2. To pay your U.S. Soccer Grassroots License waiver fee $25, click HERE (good for one season, Fall or
Spring, not both)
If you are applying for a D Waiver, please email your FILLED OUT APPLICATION to Jenni to get a payment
link.
3. Click HERE for License Waiver Application (This completed application MUST be emailed to Jenni
(jbreckenridge@utahyouthsoccer.net) for processing or no waiver will be given
All Team Administrators:
Required to take the concussion course, Certificated must be dated in 2021 to be consider current, any dated
previously will be deleted.
1. Print certificate or safe it to your computer if the program will let you.
2. Scan and save
3. Login to the affinity system
4. Select your personal profile
5. Click the box next to your picture this will allow you to search your computer for the certificate you
have saved
6. Upload the certificate (you may need to adjust the size, the program gives prompts to resizes)
Safesport Training: This is a federally mandated program that must be completed each year. • All returning
coaches will need to take the refresher course just login no code are fee required • New coaches create a
username and password and then complete the training Select U.S. Soccer as the organization and use code
YC3E-6P5G-YYIL-CS2M. (SAFE your user name and password, for further use you will need to do a refresher
course) There is a spot on the profile to upload the certificate repeat the upload instructions above – or send
them to me and I’ll do it.
Home Field Approval:
All teams must have a home field and approval to play on the field, below is a list of the South Cache area
home fields and contact information.
Registration will not be considered complete without appropriate approval.

Cache allows each team to communicate with the field that best fits their needs Fees will apply.General Information: Age Groups: The Age groups for the Fall 2021 / Spring 2022 season are determined by Year of Birth: U8– 2014 U11– 2011 U14– 2008 U17- 2005 U9- 2013 U12- 2010 U15- 2007 U18– 2004 U10- 2012 U13- 2009 U16– 2006 U19– 2003 Medical Release: Coaches need to have a copy of each player’s registration form in their possession. Have parents print, and sign a copy for you to keep this season. Tournaments will require them at check in. Minimum and Maximum Roster Size: U9- U10 No less than nine (9) and no more than twelve (12) players U11- U12 No less than eleven (11) and no more than fifteen (15) players. U13-U15 No less than Thirteen (13) and no more than eighteen (18) players. U16-U-19 No less than eleven (13) and no more than twenty-two (22) players with special game day roster requirements. Play up Rule: No player younger than U8 may play on a competitive team. Players U8-U10 are only allowed to play up 1 year. A player who is U11-U14 may be registered to a team that competes in an age group not more than two years older than the player's age group. U-15 and older will be unrestricted for play-up. The actual age of the player shall determine the play-up, not the age division the player is registered in. Multiple Rostering: Multiple rostering allows players to play on two teams at the same time. Players must be U11 or older and both teams must teams be in different age brackets –AND play in different divisions. Players must pay fees for both teams. U11 teams can have no more than 6 Multi-rostered players and U12 and older teams may have no more than 8 Multi-rostered players. Club Pass: Player regularly rostered to a Premier League team, (Premier 1, Premier 2, or Premier Colors) may C-pass no lower than Division 1 (D1) A player regularly rostered to a non-premier division Team may C-pass down no more than two (2) divisions in the player’s own age group or any older age group, may C-pass up any number of divisions in the player’s own age group or any older age group, but may not C-pass to another Team in the same division of the player’s own age group. The same rules apply to a female player C-passing to a boys’ team. Find the complete club pass rules here rule 6324 #3 Tournaments: Any team wanting to attend summer tournaments should make every effort to have ALL of their player and team registration done as soon as possible. Check with each tournament for a list of required documentation. Out of State tournaments will require player cards, the cost is not included in the registration fee, $50 per team or $5 per player. Tournament only cards are available for players that are not registered on a team, the cost is $20 per player, this included the state insurance fee that must be paid on ANY player in UYSA.
Player and Coach Pictures: Parents will be prompted to upload a picture when they register, please ask them to upload a headshot of their child only not the whole family. There are instructions on the site for uploading pictures. The Photo must be a head shots, no silly expressions NO hats. Good picture Not a good picture Incorrect photos will be deleted, and new ones will be requested.

Fees will be paid on the field in advance of the game, in the following increments: (remember there are two
linesman per game and one center.) All Refs will be paid through VENMO at the Field.
