CHEF has moved from individual event-sponsorships to an annual sponsorship program in 2018 to ensure that your organization
can participate in CHEF Chapter events held consistently throughout the year.
With the new sponsorship program, your organization will have the opportunity to:
 Be a visible partner with CHEF for an entire year
 Engage with and learn from local healthcare senior leaders
 Connect and grow your organization’s healthcare network within the Chicagoland area

Don't miss out on the opportunity to further strengthen your relationships with CHEF members. Your organization will be
assigned a Sponsor Liaison to ensure the relationship is beneficial.

2018 Sponsorship Program Details:

           To become a Sponsor, please complete the 2018 Sponsorship Agreement Form and submit,
                                         with payment, as directed.
        Sponsorship Payment must be received by 12/31/2017 to be included in the 2018 Annual Meeting

CHEF Event Descriptions

Annual Meeting: The premier CHEF annual event, held in February, includes a networking reception and dinner for the largest
one-time congregation of membership. The event culminates in an awards ceremony, and guest speakers with national
influence, focusing on the latest topics of healthcare improvement and innovation. The average attendance per meeting over
the last 5 years is 330.
       VIP Reception: An invitation-only event held at the Annual Meeting, enabling C-Suite Executives to grow their
           network across the local healthcare landscape.
C-Suite Dinners: Hosted by local Healthcare Executives who share experience and knowledge about current healthcare issues
that keep them up at night. CHEF members have an opportunity to learn and participate in a small group setting, held at a local
restaurant venue. Meal and beverage not provided by the sponsor. Target attendance per event is 20-25.
Education Programs: Professional development with topics integral to healthcare and healthcare management. Events are held
at local healthcare organizations, hospitals, health systems, healthcare technology incubators, and may include facility tours. As
an independent chapter of the American College of Healthcare Executives (ACHE), specific qualified programs deliver a mini-
mum of 12 ACHE Face to Face Credits each year. These credits count toward ACHE advancement or recertification. The aver-
age attendance per event over the last 5 years is 52.
Networking Events: Opportunities to connect in a casual atmosphere with fellow CHEF members to foster continued
professional networking and sharing across all sectors of healthcare and experience level. The average attendance per event
over the last 5 years is 56.

Sponsorship Program Highlights:
 All Sponsorships are 12 Months in duration
 Six total Platinum Level Sponsorships available in 2018, January through December
 If Platinum level is filled for 2018, Gold and Silver Level Sponsors will have priority in future years, when available
 Gold and Silver Level Sponsorships can start at any time
 Sponsors do not influence educational program content and selected speakers
 Specific program benefits are offered only as listed

About CHEF Membership
The Chicago Health Executives Forum (CHEF) was established in 1976 as a not-for-profit organization to serve local healthcare
executive community by providing continuing education, networking opportunities, and leadership development. CHEF is the
largest independent chapter of the American College of Healthcare Executives (ACHE). ACHE, the parent organization of CHEF,
has over 40,000 members in all 50 states, organized within 7 Districts, and 70 Regions.

Senior Leaders - CHEF is proud to include some of the area's top healthcare executives among its ranks. These pinnacle leaders

counseling on the state of Chicago's healthcare industry.
Clinician Members - The business of healthcare is entrusted to leaders of many different backgrounds. Through education and
networking, CHEF provides clinicians the opportunity to increase their knowledge of business principles and leadership
competencies.
Academia - CHEF is dedicated to the success of the next generation of healthcare leaders. Our student members are exposed to

current topics that headline our educational events are a strong complement to the classroom education offered at colleges in
Chicagoland.
Fellows - Becoming board certified in healthcare management as a
Fellow of the American College of Healthcare Executives (FACHE®)
demonstrates a leader’s professionalism, competence, leadership skills,
commitment to lifelong learning, and service to the field and commu-
nity.
Early Careerists - This eager and vibrant cohort is a core constituency of
CHEF membership. Educational topics, networking events, and e-
communications are just a few of our hallmarks that are designed with
the Early Careerist in mind. CHEF offers volunteer opportunities and
instructive events to help develop today's managers into tomorrow's
executives.
