• The time of year LBH benefit programs renew, and
                you can change your enrollment by:

Dependent Verification Process
• All required documentation must be received by the Benefits Team as soon as possible. You can submit documentation via
  email at LBH_Benefits@lifebridgehealth.org. You can also fax your required documents to the ATTN: Benefits Team at 410-469-
  5161.

• Your dependent(s) will not have coverage until we receive the documentation. If documentation is not received within 30 days
  of your enrollment, your dependent will not be eligible for coverage until the next open enrollment period unless they
  experience a qualifying life event. Please reference your Benefits Guide for further details and list of required supporting
  documents.

• If you have any questions regarding this process, please contact your site HR team or benefits at
