      SCHOLARSHIP GUIDELINES and APPLICATION
The Scholarship Committee of the Avant Garde Academy Foundation Inc. is pleased to
announce its 2019 scholarship opportunities (application is included) for students who
meet the following requirements:

      •   Attends an Avant Garde Academy School
      •   Plan to attend an accredited 4-year College or University, Fall Semester 2019.
      •   Have a minimum cumulative GPA of 3.0 on a 4.0 scale
      •   Demonstrate good character and possess evidence of leadership abilities.
      •   A color photograph of themselves (head shot is preferred).
      •   An Official High School Transcript with ACT and SAT scores.
      •   Student Aid Report (SAR) from your completed Free Application for Federal Student
          Aid (FAFSA).
      •   The completed scholarship application, including all supporting information.
      •   Two (2) Letters of Recommendation. One (1) from a school official and one (1) from
          a non-family member.
      •   The signed Applicant Certification and Photograph Release form (Page 5).
      •   Application materials should be mailed to (together in a single envelope) to:
          Avant Garde Academy Foundation Inc. Scholarship Committee
      •   Application materials must be mailed and postmarked by April 30th, 2019.

ABOUT THE AVANT GARDE ACADEMY, INC.
MISSION
It is the mission of the Avant Garde Academy Foundation to enhance the quality of education
and educational opportunities with selected programs and projects and to generate and distribute
financial and other resources for the benefit of the students, families and staff of the Avant Garde
Academy schools and targeted communities.

The Foundation’s intent is to engage the community in support of Avant Garde Academy’s
unique public education mission, provide ancillary funding for programs and activities that
elevate the quality of education and extend learning opportunities for our students.
Fundamentally, we are supporting extraordinary learning experiences to help students succeed.

Our Vision:
Avant Garde Academy will provide a student-centered leaning environment that develops the
whole child and provides them with 21st Century skills to become responsible leaders in their
community.

   •   Academic Excellence: The Avant Garde Academy Foundation seeks to maximize
       community investment in its schools to deliver innovative, creative programming and
       experiences for every student, removing stringent school budgetary constraints, to
       provide students with the highest quality education possible.

   •   Community Outreach: The Foundation further believes in providing the unique
       mentoring, tutoring and scholarship opportunities for low-income and at-risk students to
       help them end the cycle of poverty through education and hope in the future. Further, the
       Foundation shall provide need based grants and emergency support for homeless and
       needy families in transition for the purposes of aiding with paying rent, electric bills,
       relocation, deposits, medical/dental expenses, food and transportation.

•   Academic Opportunity: Students shall be offered college scholarships, caring mentors
       from the community, and an opportunity for a better future. Comprehensive services will
       start in middle school, continue through high school, and provide support in attaining a
       post-secondary degree.

       ABOUT THE SCHOLARSHIP PROGRAM & Board
                                       Members

Avant Garde Academy Foundation Inc. scholarships awards are based on academic
achievement and other relevant factors. Each scholarship award is $2,500. All awards
are for one year.

Frank Bolaños is the Co-Founder of Avant Garde Academy, Inc. (Avant Garde
Academy). Avant Garde Academy oversees four public charter schools (2 in
Kissimmee,1 in Hollywood, and 1 in Tampa Florida). Mr. Bolaños helped develop the
schools vision for the development of the total child, anchoring the student’s
technological foundation with the positive values that will help them become well
balanced, responsible citizens. Mr. Bolaños is an educational and community leader
accustomed to developing synergistic public-private partnerships. As Chairman of the
Miami Dade County School Board, the nation’s fourth largest school district, he led the
school district in a county wide strategic planning process. Mr. Frank Bolaños is a
former Fortune 500 senior executive that utilizes his leadership skills, experience and
energy to favorably impact the lives of others.

REQUIREMENTS

Family members of Avant Garde Academy Inc. Board of Directors are not eligible for
SMOFI scholarship awards. Applicants should be high school seniors who have been
(or will be) accepted for admission to a 4-year accredited College or University— Proof
of enrollment will be required before any funds are disbursed. Scholarship recipients will
be notified of their award on or before May 30th, 2019.

At the time of the application submission, applicants must comply with one of the
following conditions:

   •   Be a U.S. citizen (either born or naturalized)
   •   Be a legal permanent resident with a valid permanent resident card or a passport
       stamped I-551 (not expired)
   •   Have documented DACA status (Deferred Action for Childhood Arrivals)

•   Be an eligible non-citizen as defined by HHSFMO. These include:
        o U.S. permanent resident, with a Permanent Resident Card (formerly
           known as an Alien Registration Receipt Card or "Green Card”)
        o Conditional permanent resident (I-551C)
        o Other eligible noncitizen with a valid Arrival-Departure Record (I-94) from
           the Department of Homeland Security showing any one of the following
           designations: Refugee, Asylum Granted, Indefinite Parole, Humanitarian
           Parole, or Cuban-Haitian Entrant

Application Instructions
 •   Complete this application in its entirety.
 •   Include a color photograph—headshot is preferred.
 •   Please type—items 1-7, below.

 On separate sheets, type or word process (single sided, single spaced and use 12-
 point font size) the responses to items 8-14. Correct grammar, punctuation,
 sentence structure and word count is required. Read, sign and submit the Applicant
 Certification and Photograph Release form. Submit your Official high school
 transcript. The Official Transcript must show your cumulative G.P.A. through seven
 (7) semesters with SAT and/or ACT scores. Do not staple individual sheets together.
 Label essays and arrange all documents in order, as listed in the Submission
 Checklist. Return your completed application with all supporting materials, together
 in one envelope by April 30th, 2019.

     1. Write an introductory statement about yourself—tell us who you are.
        Include family values, short-term goals and where you see yourself in
        five years. (No more than 400 words).

     2. Extracurricular Activities, Honors & Awards, Leadership Positions and
        Work Experience – Provide a summary sheet of your extracurricular
        activities, honors, community service, leadership positions held
        and/or work experience for the current and previous school year.
        Include specific details, i.e. organization/activity/honor, date(s),
        position(s) held, responsibilities, etc.

3.     Provide two (2) Letters of Recommendation. One (1) from a school
       official and one (1) from a non-family member. Each letter should
       include: · How you know the applicant and the length of time you
       have known the applicant. · A description of the applicant’s strengths
       and weaknesses. · A discussion of why you believe the applicant can
       succeed in college/university. · Reference Information – printed
       name, original signature and contact Information.

Submission Checklist

All items must be submitted for the application to be considered. Please
verify that you have included:

o Color Photograph

o Official Transcript with SAT and/or ACT scores

o First page of Student Aid Report (SAR) from your FAFSA that shows
Expected Family Contribution (EFC) or First page of Parent’s 2017 IRS tax
form 1040

o Completed Application (items 1-11)

o Typed Essays and Supplementary Documents (items 12-19)

o Two (2) Letters of Recommendations (school official and non-family
member)

o Signed Applicant Certification and Photograph Release (pages 9-10)

Application and all supporting materials should be mailed to (together in a

     • If requested, I agree to be available to participate in an interview with the
     Scholarship Committee.
     • As part of this application, I authorize school personnel and/or other
     individuals to provide data or information about me directly to the 2018
     Avant Garde Academy Foundation Inc. and to waive the right to review
     any such submissions.
     • I understand that falsification of information may result in termination of
     the granted scholarship and result in having to return any disbursed
     scholarship funds.
     • I understand that I may be asked to verify any income information,
     provided in this application.
     • I agree that the Avant Garde Academy Foundation Inc. has permission
     to use general (non-financial) information provided in this application, for
     publicity purposes.
     • I understand that all information provided in this application will remain in
     the possession of the Avant Garde Academy Foundation Inc.

•   I understand that if selected, I must provide proof of College/University
    enrollment prior to receiving a scholarship award.

• It is my intent to enroll in and complete a degree program at a 4-year
  accredited college or university and maintain good academic standing as
  required by the Avant Garde Academy Foundation Inc. I understand that,
  if selected, I may be required to maintain a certain G.P.A. to receive
  ongoing scholarship payments. I will keep the Avant Garde Academy
  Foundation Inc. informed of my academic status—as requested.

• I understand that, if selected, the scholarship award will be used for
  college expenses for the Fall 2019/20 school year.
• Disbursement will be divided into three distributions, initial 1/3 portion of
  your scholarship will be disbursed by June 15th. The remaining two
  disbursements will be distributed at the end of your 1st and 2nd semesters
  of college, once you submit your grades averaging a C or higher.
