Out Of Hours
Call NHS 111 for medical advice and guidance when the Medical
Centre is closed

Call 999 for medical emergencies, such as chest pain, severe
bleeding or if the person you are calling about is unconscious.
Be ready to supply the nature of the problem and as much information
as possible such as:
    The patients name and date of birth
    The telephone number
    The address
    Simple directions if necessary

Visit Chard Minor Injury Unit, Crewkerne Road, Chard
The Minor Injury Unit is open between 8am & 9pm seven days a week
(last patient admitted at 8.30pm) including Bank Holidays and deals
with chest, eye, throat and urinary infections, sprains, strains,
emergency contraception, wounds and bites.

Get to know the Team
Doctors
Dr Carol Cooper BM DCH MRCPsych MRCGP joined the Practice in
2006, she is the Senior Partner and Safeguarding Lead. Dr Cooper
holds clinics on a Monday, Wednesday and Friday.

Dr David Trenchard MBChB Dip Orthopaedic Medicine
MSOM FP Cert is the second GP Partner who joined the Practice in
2011. He works closely with Community Nurses and Nursing Homes
as our frailty lead. He works Monday, Tuesday, Thursday and Friday.
Practice Manager
Dianne Kibblewhite has been Practice Manager since we moved to
our current building. Dianne manages the financial aspect of the
Medical Centre, as well as recruitment of staff and the overall
organisation of the Practice. Dianne is responsible for the smooth
running of the Medical Centre, she liaises with Doctors, Staff and
Patients to ensure Church View runs efficiently.
Dianne will review any complaints received but also welcomes
positive comments and is continually striving to improve services for
patients registered at the practice.
Office Manager
Linda is responsible for the Reception Team and administrative tasks.
She strives to ensure a high level of service for patients and engages
in new projects enabling patients to have better access to improved
services. Linda also works with the Patient Participation Group.
Receptionists/Medical Secretaries
Our Receptionists/Medical Secretaries are here to welcome patients
and visitors to the Medical Centre and will help with enquires and
make appointments.
Practice Nurses
Our experienced nursing team offers appointments to review chronic
conditions such as Asthma, COPD, Diabetes, Heart Disease and
Hypertension, as well as cervical smears, ECG’s, wound care and
dressings, blood pressure, ear syringing, immunisations and travel
advice.
Dispensers
Our Lead Dispenser and three fully trained Dispensers work together
to process medication requests, as well as handling medication
related queries.
Church View Medical Centre Core Values
Located in the beautiful village of Broadway the purpose build Medical
Centre was opened in 2014 and replaced portable accommodation
which had been onsite since 1998. The Medical Centre is fully
accessible to wheelchair users and people with limited mobility. There
is a Loop facility for the hard of hearing and an interpreter service is
available.
Situated behind Broadway Village Hall, there is ample parking,
including disabled parking for the use of patients and visitors to the
Medical Centre.
The Care Quality Commission has inspected the Medical Centre twice
and both times Church View received a `Good` rating. More

Church View Medical Centre is a small friendly practice and was rated
8th Best Practice in Somerset in a recent national poll. We are
constantly looking at ways to improve our services and work with our
Patient Participation Group to deliver services that meet local needs.
We work collaboratively with other surgeries in our local area to
deliver services which as a small practice we might not otherwise be
able to provide. These services include a Pharmacist, a visiting
Emergency Care Practitioner and a Village Agent. By working
collaboratively with other practices we hope to improve services to
patients while maintaining our autonomy and independence.
Clinical Pharmacist
The Pharmacist visits patients at home to review medication and offer
medication related advice.
Emergency Care Practitioner
Patients who are too unwell to attend the Medical Centre and require
a visit at home should telephone the practice before 10.30am so that
this can be arranged. The Emergency Care Practitioner will make a
clinical assessment and following their visit your problem will be
discussed with your doctor and appropriate action taken.
Village Agent
The Village agent works with the Medical Centre to provide non-
clinical support to patients in the community. Social support is offered
relating to transport issues, form filling, benefit claims, feeling of
isolation and much more. Please ask at reception for more
information.
Carers Champion
Carer support and advice is available from the reception team. The
receptionists are all trained to help, advise and support carers.

Care Quality Commission (CQC)
At our most recent CQC inspection carried out on the 7th March 2018
we received a Good outcome.
The inspection focuses on the following areas:-
    Are services safe?
    Are services effective?
    Are services caring?
    Are services responsive to patient’s needs?
    Are services well-led
We are constantly looking to move forward and one comment from
the inspectors was that they would like to see us working more
closely with the community that we serve. If you have any
suggestions or comments on how we may achieve this please speak
to the Practice Manager.
Registration
We welcome new patients from Broadway, Horton, Ashill, Ilminster
and the surrounding area to register at Church View Medical Centre.
All new patients are required to complete a Health Authority
Registration Form and a New Patient Questionnaire. Please return
these forms to reception, ideally in person, with proof of Identification.

Registration forms are available from reception, or on the practice
website www.churchviewmc.nhs.uk where useful information about
the Medical Centre, NHS services and health advice can also be
found.

All Patients are registered with a named GP although patients are
welcome to see the GP of their choice.

The map on the back page shows the area we cover and although
patients can be accepted outside of the boundary, home visits will not
be offered to these patients.

Chaperones
For consultations involving examinations or tests patients may
choose to have a chaperone present. A friend or relative may attend
for this purpose or the practice can provide a chaperone if required,
please ask at reception for more information. GPs and nurses may
also request a chaperone be present for a patient’s consultation.

Making an Appointment
When booking an appointment the reception staff will ask questions in
order to decide how best to help patients and to make sure the
appointment is booked with the most suitable clinician. Please be
assured all the information given is treated in the strictest confidence.
We have a variety of appointment options available to enable patients
to plan ahead or to see a clinician at short notice. Please note all GP
appointments are 10 minutes, if more time is needed please advise
the receptionist on booking. When booking an appointment a text will
be sent confirming the date and time of the appointment and a
reminder will be sent two days prior to the appointment.
Pre-bookable appointments
Appointments available to book in advance. Planned ahead of time,
these appointments are used for managing long term conditions and
follow up appointments with a clinician.

Book on the day
These appointments are made available each day, they can be
used to book urgent on the day care with a clinician.

Telephone Consultations
Not all problems require a face to face consultation with a GP or
Nurse. Telephone consultations for those patients who have a
problem but don`t necessarily need to come into the surgery are
available.
Patient Access
This online service enables patients to book their appointments,
order medication and view aspects of your medical record at a time
to suit them. To register for this service please speak to a member
of the reception team.
Improved Access
We are working with several surgeries in our area to offer planned
appointments in the evening between 6.30pm and 8.00pm as well
as Saturday mornings. Patients will be offered an appointment with
a clinician, but not necessarily with the registered GP and you may
have to travel to another surgery to attend this appointment. Please
ask for further details

Engage Consult
This system is accessed via the Church View website
churchviewmc.nhs.uk and clicking on the Engage Consult icon.
Following the guidance given on screen patients can submit
information to the Medical Centre or ask about specific health
issues. For queries received prior to 12 noon, a same day response
is guaranteed. All other queries will be responded to within 24
working hours.

Direct Messaging Service
Information may be sent out to patients via text. This may include
test results, the need to make an appointment to see a clinician or
as a reminder that a review is due.
Dispensary
We are fortunate to have an onsite Dispensary where our qualified
dispensers process all medication requests for dispensing and non-
dispensing patients. Patients living more than one mile from their
nearest pharmacy are eligible to have their medication dispensed by
us. Patients living within one mile of a pharmacy can opt to have their
prescription sent via the Electronic Prescription Service to a pharmacy
of their choice, or it may be collected and taken a pharmacy of choice.
Medication can be ordered via:
 Patient Access Service to order medication online
 The Repeat Medication Ordering Service on collection of
   medication patients can complete and return the repeat prescription
   slip advising the dispensary of the items needed the following
   month. These items will be ready for collection on the due date.
 In person and/or by returning the repeat prescription sheet to the
   Dispensary, clearly marking the medication required.
 Batch Ordering—following a patient’s review a medication order
   can be placed for up to 12 months for items on repeat prescribing

Medication is issued on a 4 weekly basis, please allow at least 2
working days for repeat prescription requests to be processed.
Patients receiving regular medication will be required to attend for
regular medication reviews, usually in the month of birth.

Please store medication safely at home and return any unused
medication to the Medical Centre or your local Pharmacy for safe
disposal.
Prepayment Certificate and Proof of Exemption
Pre-payment certificates can be ordered by phoning 0300 330 1341
or online at www.nhsbsa.nhs.uk/ppc. Any patient receiving more than
four prescriptions in a three month period or 12 items within a twelve
month period will be saving money with a pre-payment certificate.
Currently the cost of a prepayment certificate for three months is
£29.10 and £104.00 for twelve months. The Dispensary staff will ask
to see proof of exemption, if this is not available patients can collect
and sign for medication but the dispenser will mark the prescription to
show that no proof was seen.
Patients who claim free prescriptions fraudulently can face being
fined.
Blood Pressure, weight, height and BMI Machines
Please feel free to use this equipment which is available in the waiting
room. Results can be taken in to a consultation or handed to
reception. Patients should make sure their name and date of birth are
written on the back of the slip so that the information can be recorded
on the medical record.

Minor Surgery
A wide range of minor surgery procedures can be performed at the
Medical Centre by Dr Cooper and Dr Trenchard.

Community Midwife
The midwife holds regular ante-natal clinics at the Medical Centre,
appointments can be booked at reception.

Podiatrist
We have a private podiatrist who visits the Medical Centre bi-monthly,
appointments can be booked at reception and fees are available on
request.

Non-NHS Services
Private medical examinations such as HGV/Taxi licences, insurance
forms etc can be arranged by appointment at the Medical Centre.
Private fees are applicable for all non NHS services; details of
charges are available from reception. A charge is also made for
private letters, completion of medical insurance forms, fitness to travel
letters and vaccinations etc. which are not covered by the NHS.

Patient Participation Group
We have an active Patient Participation Group which meets quarterly
to discuss the services offered at the Medical Centre and patient
needs. If you would like to be involved with the PPG please leave
your details at reception and a member of the team will contact you.

Sharps Disposal
Disposal of sharps containers is managed by Somerset County
Council. To dispose of these items please call Somerset County
Council on 0300 123 2224 who will arrange for doorstep collection.
We are unable to accept these in the Medical Centre.

        OUR CATCHMENT AREA

    The Medical Centre is located in the
   village of Broadway, just off the A358,
         adjacent to the Village Hall.
      There is ample parking available.
