WHEN COVERAGE BEGINS AND ENDS
For all health and welfare benefits ( not including 401K), your coverage would begin
the 1st of the m onth following date of hire. If your date of hire is on the 1st, your
benefits begin that day. Your coverage under the benefits plans will end at the end of
the month if you no longer m eet the eligibility requirements, your contributions are
discontinued, or the Group Insurance Policy is terminated.
Q UA L I F Y I N G E V E N T S
Eligible employees may enroll or make changes to their benefits elections during the
annual open enrollment period. As with most benefits, once you elect an option you
are bound to that choice for the entire plan year unless you experience a “Qualifying
Event”. These may include, but are not limited to:
• Changes in employment status
• Changes in legal marital status
• Changes in number of dependents
• Taking an unpaid leave of absence
• Dependent satisfies or ceases to satisfy eligibility requirement
• Family Medical Leave Act (FMLA) leave
• A COBRA-qualifying event
• Entitlement to Medicare or Medicaid
• A change in the place of residence of the employee, resulting in the
current carrier not being available

The Voluntary Accident is an insurance policy that is 100% EMPLOYER-paid
for employee only. This insurance offers financial protection by paying a cash
benefit if you or an insured dependent are injured as a result of a covered
accident.

For 2020 - 2021, you are offered a policy that includes a health
savings account (HSA).
This is how an HSA works:
A health savings account (HSA) is a health care account and savings
account in one. The main purpose of this account is t o offset the cost of
a qualifying high deductible health plan (HDHP) and provide savings for
your out-of-pocket eligible health care expenses – those you and your
tax dependents may have now, in the future, and during your retirement.
This is a “portable” account. You own your HSA! It’s included in your
employee benefits package, but after you set up your account, it’s yours
t o keep, even if you change jobs or retire.
Once your HSA is established, money is contributed t o your account by
you or friends and family, and you can then use your HSA dollars tax-
free t o pay for eligible health care expenses. You save money on
expenses you’re already paying for, like doctors’ office visits,
prescription drugs, and much more. Best of all, you decide how and
when t o use your HSA dollars.
The 2020 HSA contribution limit is: $3,500 for Individuals, $7,000 for
Family, and an over 50 catch-up addition of $1,000.

Perinatal Wellness Guidelines
Routine recommendations for a healthy pregnancy
First Prenatal Visit
Your health care provider* will evaluate your personal health and family history to plan the best care for

Later Prenatal Visits:
Follow-up visits usually take less time. Your health care provider may also:
Check your blood pressure Check your hands, feet and face for swelling Listen to the baby’s heartbeat (beginning as early as week nine)
Perform an ultrasound (usually at weeks 18 to 20) Measure the size of your uterus Ask about the baby’s movement
Normally, you will see your health care provider every four weeks for the first 28 weeks of pregnancy, then every two weeks until 36 weeks of
pregnancy, and then weekly until your baby is born. Talk with your health care provider and bring a list of questions to every visit.
After Giving Birth:
If you have had a C-section, you may have two follow-up visits at two and six weeks. It is important to follow up with your
health care provider about six weeks after the delivery at which time he or she:

May give vaccinations, as needed May ask about breastfeeding
You may also be asked if you are feeling low, have lost interest in usual activities, have
difficulty caring for your baby, or have difficulty concentrating, focusing or making decisions.
Depending on your health and the health of your unborn baby, your health care provider may wish to see you more often
and do additional testing.

Make Your Fitness Program Membership Work for You!
Fitness can be easy, fun and affordable. Well On Target makes it possible with the Fitness Program.
Since you are a Blue Cross and Blue Shield of Texas member, the Fitness Program is available exclusively to you and your
covered dependents (age 18 and older). The program gives you unlimited access to a nationwide network of more than 10,000
fitness locations. If you want, you can choose one location close to home and one near work. You can visit locations while you’re
on vacation or traveling for work.
Other program perks include:
No long-term contract: Membership is month to month. Monthly fees are $25 per month per member, with a one-time
enrollment fee of $25 per member.*
Blue PointsSM: Get 2,500 points for joining the Fitness Program. Earn additional points for weekly visits. You can redeem
points for apparel, books, electronics, health and personal care items, music and sporting goods.**
Complementary and Alternative Medicine (CAM) discounts: Save money through a nationwide network of 40,000 health and
well-being providers, such as acupuncturists, massage therapists and personal trainers.
Web resources: You can go online to find fitness locations and track your visits.
Convenient payment: Monthly fees are paid via automatic credit card or bank account withdrawals.

1. Go to bcbstx.com and log in to Blue Access for MembersSM.
2. Under “Quick Links,” choose “Fitness Program.” On this page, you can enroll, search for nearby fitness locations and
learn more about the program.
3. Click “Enroll Now.” Then search and select the fitness location that is best for you. Remember, you can visit any
participating fitness location after you sign up.
4. Verify your personal information and method of payment. Print or download your Fitness Program membership
ID card. You may also request to receive the ID card in the mail.
5. Visit a fitness location today!

The Fitness Program is provided by Tivity HealthTM, an independent contractor that administers the Prime Network of fitness
locations. The Prime Network is made up of independently owned and operated fitness locations.
