WELCOME TO OPEN ENROLLMENT
Benefit Enrollment
▪ The open enrollment period runs from April 1, 2021 – April 30, 2021
▪ Elections made during open enrollment will become effective May 1, 2021      OPEN
▪ Sharpnack Auto offers you and your eligible family members a              ENROLLMENT
  comprehensive and valuable benefits program. We encourage you to take

  coverage for you and your family.
WHO IS ELIGIBLE

If you are a full-time employee OR an employee working 30 or more hours per week on a regular basis), you
are eligible to enroll.
When you enroll, the following will be eligible for coverage:
    ➢ Full-time employees who have fulfilled their probationary period
    ➢ Spouses
    ➢ All enrolled eligible children will continue to be covered until the child turns age 26
Qualified changes in status include:
                       •  Marriage
QUALIFYING LIFE        •
                       •
                          Divorce
                          Legal separation

   EVENTS              •
                       •
                          Birth or adoption of a child
                          Change in child’s dependent status
                       •  Death of a spouse, child or other qualifying
                  status dependent
                        • Commencement or termination of adoption
                          proceedings
                        • Change in spouse’s benefits or employment

                  ➢ It is important to get your changes in now or you will not have an
                    opportunity to change until next April 2022 unless one of the
                    obove events occur.
                  ➢ You have 30 days from the date of the event to notify your Human
                    Resource Department of a change.

          Prescription Drugs

                 The CARES Act permanently reinstates coverage of
                 over-the-counter (OTC) medications as eligible for
                 reimbursement from FSAs, HRAs, and HSAs
                 without need for a prescription.

                 This change is retroactively effective for expenses
                 incurred on or after January 1, 2020.
