W-9. Awards made to incorporated dealers, as demonstrated in fully executed W-9 forms, will

What are the Qualifications?
Total points earned by a distributor will determine automatic qualification in the Phoenix XL
travel program. One appointment (trip for two) will be awarded when 750,000 points are
earned in the program period by a distributorship. No more than two trips for two will be
awarded to the same dealer or OEM from points accumulated from the end user for any
reason. Point distribution within a dealership is the responsibility of the dealership. In the
event the destination becomes overbooked, EnerSys reserves the right to limit participation or
to substitute an alternate destination. Awards are made to the dealership, not to individuals,
and the dealership shall decide which salesperson shall participate provided they meet the
details of “Who May Participate” section of this brochure. Distributorships or branches earning
the requisite points for qualifications will be awarded appointments only if they are in good
credit standing with EnerSys. Should the participant’s account not be current at the end of the
program, no award shall be made.
Point Registration
Orders will be credited automatically when written if the distributorship or factory branch
placing the order is known. When in doubt, submit a points claim, or write (providing order
number, equipment covered, and point value claimed) to: EnerSys, 2366 Bernville Road, Reading,
