 PREMISE: Planned, methodical work where hazards are identified and
 eliminated/mitigated is Safer to execute.

 Quality work reduces exposure to hazards - now and in the future.

 This is safer for Turnaround Contractors/Clients now – and Owner’s
 Operators and plant Maintenance personnel long term.

A process used to drive strategic and tactical decisions for implementation
and execution success

Provides real-time, comprehensive, multi-discipline assessment of an
upcoming event’s preparation efforts
    - Tied to Customer’s Milestone Deliverables Schedule

Interview-based approach with Field Walk Scope Review

Begin early, sometimes multiple visits; rule of thumb T - 45 days

TYPES OF QUESTIONS WE ASK

Has the JVIC safety lead discussed site safety performances with the Client's safety
representation (i.e. inquiries to facility safety performances and trends, unique site safety
considerations, etc.)?
Has the scope of work been reviewed (including scope walk down) with the Safety Lead?
Does the work scope consist of flare work? If so, what measures have been exercised to
ensure that Zachry/JVIC is in alignment with the Client's flare policy? Has the PPE been
identified / approved (i.e. high-temp and/or flare PPE).
Based on the known work scope are there any potential asbestos, lead, and/or silica hazards
that should be identified within the scope planning / scheduling tool?

Will the scope of work require any variances (i.e. cut-off discs, blinding - first flange, wedges
other than brass, goose neck hammer wrenches, pipe wrenches, etc.)?

What short-service worker initiatives will we have in place?

Are rescue plans in place for all confined space entries? Are ventilation plans required for
confined space work? Who develops them?

Hand Safety Specifically. Do we have adequate plans in place relative to hand PPE that clearly
states PPE expectations (i.e. Cut 3 gloves while handling tower packing/demisters or milling
flange faces)?

Does the Client have specific expectations and/or initiatives with respect to Industrial Hygiene
topics (i.e. weld fumes, Hex Chrome, gouging samples, etc.)?

Has the contract been reviewed in an effort to understand PPE provisional responsibilities?

Is there a clear and concise waste management plan supported by waste receptacle staging in
place (i.e. non-haz waste, haz waste, etc.)?

How will we ensure that the culture of "stop work authority" remains a present component to
our culture?

Have Safety Action Plans and/or Project Closeout safety performances from previously
executed jobs at the project been reviewed?
Does the Safety Action Plan consider the following:
a. PPE Requirements

g. Confined Space (regens and heaters, multiple crafts inside, communication plan, etc.)
h. Unit Specific Issues (Regulated chemicals Alky, Benzene, H2S, NORM, etc.)
i. JSA/JTA Process
j. Fatigue Program
k. Heat Stress Monitoring
l. Special PPE condition zones (i.e. FCC structure requiring foam-lined eyewear when on the
   structure, flare suits, etc.)?
