             Subject(s)   Santiago, Jose R., male, age 27, 3717 W. Wilson Ave., Chicago IL 60625
                          Lake Forest Police responded to the area of Rte. 41 and Deerpath, after dispatch received a 911 call of a reported vehicle described as driving all
                          over the road, southbound on Rte. 41, passing the Chevy Exchange in Lake Bluff, Illinois. The vehicle was described as a silver Lexus or GMC.
                          Officers located a silver GMC Arcadia traveling south on Rte. 41 that matched the description provided by the caller. Officers began following the
                          vehicle and, after observing the vehicle commit several lane violations, conducted a traffic stop on the vehicle. Officers spoke with the driver,
                          Santiago, concerning the reason for the stop. Officers immediately observed an open 12oz can of Coors Light in the center console cup holder
                          and that Santiago's eyes appeared bloodshot/glassy and an odor of an alcoholic beverage could be detected coming from his breath when he
             Summary      spoke. Officers asked Santiago how much alcohol he had consumed that evening, to which he replied, "A few beers". Santiago was unable to
                          produce a license or insurance, and officers determined his driving privileges were suspended at the time of the stop. Officers requested
                          Santiago step from the vehicle and complete some standard field sobriety tests to determine his ability to drive. Based on the officer's
                          observations of Santiago's driving, his performance on the field sobriety tests, and admittance to drinking prior to driving, Santiago was placed
                          under arrest and charged with Driving Under the Influence of Alcohol, No Vehicle Insurance, Open Transportation of Alcohol, and Driving While
                          License Suspended. Santiago was transported to the Public Safety Building, where he was processed and provided a breath sample that
                          registered 0.95 BAC. He was released on bond with a June court date.

                         On November 12th, 2020, at approximately 10:47 am, a pedestrian was struck by a white two‐door Ford F150 pickup truck in the 500 north block
                         of Mayflower (near Mayflower & Rosemary). The driver of the truck then fled the scene south on Mayflower. Police located the victim, Stephen
                         Kennedy (male, age 62), at the scene. Kennedy was able to speak with officers at the scene and, after that, was transported by the Lake Forest
                         Fire Department to Condell Hospital for treatment. Kennedy later succumbed to his injuries caused by the crash.

                         Lake Forest Police investigated the crash, and after an extensive investigation, detectives were able to identify and locate the white Ford F‐150
             Summary     as the vehicle involved in the crash. Police were issued a search warrant for the vehicle and subsequently seized it as evidence. Based on further
                         investigation, detectives were able to identify the driver of the F‐150 at the time of the fatal crash as Dubofksy, Zachary. Lake Forest Police
                         Detectives, working closely with the Lake County States Attorney’s office, were able to develop enough physical evidence, in this case, to go
                         before a Lake County Judge and present sufficient probable cause to obtain an arrest warrant for Dubofksy for the crime of Leaving the Scene of
                         an Accident Involving Death/Personal Injury (625 ILCS 5/11 401b) (class 1) with an attached bond in the amount of $100,000.00. On Tuesday,
                         April 20th, 2021, Lake Forest Police Detectives served the arrest warrant on Dubofksy, processed him at the Lake Forest Public Safety Building,
                         and released him after the bond was posted. Dubofksy’s next court date is on May 4th, 2021, at 9:00 am in courtroom T‐110.

                          Police conducted a traffic stop on a dark‐colored SUV after observing the vehicle speeding on Rte. 41 and getting a positive radar lock at 96mph
                          in a posted 55 mph zone. Police spoke with the driver, identified as Maisonet, concerning the high‐speed driving. When speaking to Maisonet,
                          officers detected the odor of alcohol coming from her, and she was demonstrating signs of impairment. When asked if she had been drinking
                          before driving, Maisonet stated she had been at a bowling alley and consumed beer. Maisonet was requested to complete some standard field
                          sobriety tests to determine her ability to drive. Due to the strong odor of an alcoholic beverage emitting from Maisonet, obvious signs of

                          impairment, admittance to consuming alcohol, and signs of impairment on her SFSTs, Maisonet was placed into custody and charged with
                          Driving Under the Influence of Alcohol. Other charges Maisonet was subsequently charged with included Open Transportation Alcohol, Driving
                          with No Vehicle Insurance, and Speeding 96/55. Maisonet was transported to the Public Safety Building, where she was processed and released
                          on bond with a June court date. A female passenger in the vehicle was transported to Lake Forest Hospital by LFFD after officers on the scene
                          determined she was in danger due to extreme alcohol intoxication. Her blood‐alcohol level was later confirmed as 0.50 BAC (6x the legal limit).

                          Police conducted a traffic stop on a 2010 gold Chevrolet Impala, which was driven by Gordon. During the traffic stop, officers noted Gordon was
                          demonstrating signs of some type of chemical impairment. When asked again, Gordon admitted to having smoked marijuana. He stated he had
                          smoked approximately an hour prior and included eating marijuana edibles. Gordon was requested to exit the vehicle and perform some
                          standard field sobriety tests to determine his ability to drive. Due to Gordon’s physical signs of impairment, signs of impairment on SFSTs, and
             Summary
                          admittance to smoking weed and being high, Gordon was taken into custody and charged with Driving Under the Influence‐Drugs. 70 grams of
                          cannabis was found in his possession, and he was also charged with Possession of Cannabis by a Driver. Gordon was transported to Lake Forest
                          Hospital to complete a DUI kit and was then taken to the Public Safety Building, where he was processed, released on bond, and given a June
                          court date.

                         Police conducted a traffic stop on a Hyundai Sonata after observing the vehicle speeding 112 mph on Rte. 41, which is a posted 55 mph zone.
                         Officers spoke to the driver, Esquivel, concerning the reason for the stop, which was excessive speed. During the conversation with Esquivel, he
                         admitted to not having a driver’s license and had no form of physical ID on him.
                         Officers could smell the odor of burnt cannabis coming from the car. Esquivel was asked to step out of the car, and he was placed under arrest
                         for Driving without a Valid Driver’s License. During the traffic stop, Esquivel was involuntarily vomiting and excessively sweating. Officers could

                         smell the odor of alcohol coming from his breath, and out of caution, a Lake Forest Ambulance was called to the scene to determine if Esquivel
                         was experiencing a medical emergency. Esquivel was checked out by paramedics and subsequently refused medical treatment/assistance. Inside
                         Esquivel’s vehicle, officers located a plastic bag full of cannabis, open alcohol in the center console, a half‐full bottle of Hennessey, and various
                         drug paraphernalia items scattered around in the car. Police transported Esquivel to Lake Forest Hospital for completion of a DUI kit. Esquivel
                         was then transported to the Public Safety Building, where he was processed, released on bond, and given a mandatory May court date.

                         Police conducted a traffic stop on a red Volkswagen Jetta after observing the vehicle speeding 59mph on Green Bay Road. Police spoke to the
                         driver, Avelino, concerning the reason for the traffic stop. During the conversation, officers noted the smell of alcohol coming from Avelino’s
                         breath and demonstrated signs of alcohol impairment. Avelino admitted to drinking, and officers observed open alcohol containers in his vehicle.
             Summary     Avelino was requested to complete a standard sobriety test to determine his ability to drive. Based on all of the circumstances, Avelino was
                         subsequently placed under arrest for the following charges: Driving Under the Influence of Alcohol, Driving While License Suspended, No Vehicle
                         Insurance, Open Transportation of Alcohol, and Speeding 59/30. Avelino was transported to the Public Safety Building, where he was processed
                         and submitted to chemical breath testing, resulting in a 0.214 BAC. Avelino was released on bond and given a mandatory May court date.

                          Lake Forest Police were dispatched to Lake Forest Acute Care on Everett Road after receiving a 911 call concerning a possibly intoxicated subject
                          acting aggressively. They were worried he might attempt to drive a vehicle. Police were able to interview Foster and determine that he had
                          driven to Lake Forest Acute Care for his job. During the conversation, officers detected a strong odor of an alcoholic beverage emitting from
                          Foster's breath, and Foster was demonstrating signs of alcohol impairment. Officers requested Foster complete some standard field sobriety
                          tests to determine his ability to drive. Due to witnesses placing him behind the wheel of the Mazda, a strong odor of an alcoholic beverage
             Summary      emitting from his person, his admission to driving the vehicle, glossy/bloodshot eyes, and signs of alcohol impairment, and his performance on
                          the SFSTs, Foster was placed into custody for Driving Under the Influence of Alcohol. Foster was transported to the Public Safety Building, where
                          he was processed and provided a breath sample with a reading of 0.224 BAC. Officers determined that Foster's driver's license was revoked for
                          previous DUIs, and contacted the Lake County States Attorney for appropriate charges. The Lake County States Attorney approved several
                          felony charges for Foster. Foster was transported to Lake Forest Hospital before he could be released, and officers had to obtain a warrant for
                          his arrest at the direction of the LCSA's office.

                          Lake Forest Police investigated an incident involving fraudulent credit card use after several lake Forest Hospital employees reported that
                          someone had stolen their credit cards from their belongings and used the cards to buy items illegally. The employees also stated the offender
                          stole varying amounts of US currency from their belongings. Police reviewed the hospital security video and were able to develop Fonza as a
                          suspect. The video showed that Fonza entered the hallway at approximately 1401 hours by waiting for an employee to leave through an access
             Summary
                          door next to the ER exterior and left in a black Nissan Altima registered in his name. Fonza has an extensive criminal history of Burglary and
                          Identity Theft and is on Parole. Lake Forest Police conducted further investigation and, on 10/29/2020, presented their evidence to the Lake
                          County States Attorney, and an Arrest Warrant for Fonza's arrest was issued for Financial Institution Fraud and other offenses. Cicero PD
                          subsequently arrested Fonza for resisting a peace officer and other active warrants.

                         An officer on routine patrol observed a traffic crash between two vehicles at the intersection of Rte. 41 and Westleigh Road. The officer spoke to
                         the drivers and determined the driver of a Nissan, driven by Deloya, struck a Toyota from behind as it was starting through the intersection after
                         the light changed to green. As the officer spoke to Deloya, he could smell the odor of alcohol coming from Deloya’s breath. When asked, Deloya
                         admitted to drinking 6‐7 beers prior to driving. The investigating officer requested Deloya complete some standard field sobriety tests to
             Summary     determine his ability to operate a motor vehicle. Based on the officer’s observations of Deloya causing a 2‐vehicle accident, admitting to drinking
                         6‐7 Corona beers 1 hour prior to driving, having bloodshot, glassy eyes, observing an odor of an alcoholic beverage emanating from his breath,
                         and his performance on the field sobriety tests, Deloya was placed under arrest for DUI‐Alcohol. Deloya was also found in possession of a small
                         quantity of cannabis and drug paraphernalia. Deloya was transported to the Public Safety Building, where he was processed and subsequently
                         provided a breath sample that registered 0.193 BAC. Deloya was processed, released on bond, and given a mandatory May court date.
