Health Teams across Tonbridge, Tunbridge Wells, Sevenoaks and Dartford have been
restructured to move away from a static service of weight loss and physical activity to
offer a more holistic service. This capacity allows more robust signposting to a wider

range of health related services while the client is supported on a journey to better
health.

online health quiz, called ‘How Are You’, to identify
where they can make small changes.

One to One holistic lifestyle assessment for anyone referred by either GP, Self or from a delivery partner
Assessments are designed to uncover and address underlying causes of conditions such as obesity and
poor mental health
Advisors link in with internal council services such as housing, hoarding, community safety, revenues and
benefits as well as external partners to support the client with wider issues that maybe preventing them
rom changing an unhealthy behaviour.
200 referrals received for TMBC from October to March 18’ and 117 from April to JuneOne You Weight Loss One You Weight loss, a 10-12 week weight management programme designed to help people lose 5% o Page 9 their weight within 6 months and keep it off for good. Followed up at 6, 9 and 12 months Run in community venues. Times and venues change regularly to meet demand. Programmes are run in locations based on need and has the flexibility to move venues if required. Elements of behaviour change and nutrition. Clients with additional or specific needs can also have a 1:1

• 47,000 people attend our health walks every year! Led by friendly volunteers they are especially good fo
people who are recovering from illness, or who just need a bit of extra motivation to take more exercise

• TMBC have 8 walks running from 5 locations.NHS Health Checks • People aged 40-74 may be entitled to a free NHS Health Check. The health check will measure blood Page 11 pressure, cholesterol levels and body mass index. Clients will also be asked some simple lifestyle and family questions. • The check only takes around 30 minutes and clients will be given a personal plan to help them achieve and maintain a healthy lifestyle.

Case Study
• Bob is a 50 year old man who has a BMI of 38, smokes 10 a day, is physically
inactive due to back pain and takes medication for anxiety and depression. He
also struggled to get up and down the stairs and into his bath. He also reports
that he is lonely.
• What did we do?
• Booked bob in to see a One You advisor

• Advisor asked eligibility for NHS Health Check
• Informed Bob about TM active leisure services.
• Passed on referral on to KCHFT stop smoking service
• Provided information regarding healthy walks and social activity groups in
the area such as men in sheds and community cuppa
• Refered to OT in our housing team who done an assessment of his home
and provided equipment to aid mobility in the house and lower the risk of
falls.
• Set GOALS with bob and book an appointment with him for a couple of
weeks.
