• The service has conducted an Excursion Risk Assessment which is available for your review upon request.
• Your child must be at OSHC 30 minutes before excursion departure time- families will still incur the full cost if the
bus is missed.
• We operate on a ratio of 1 staff member to 8 children on excursions. We anticipate we will have 30 children
attend this excursion and 4 educators.
• This form is signed on behalf of waivers at external business- once singed this will act as a formal waiver.
COVID-19 changes:
Due to current restrictions and social distancing, the number of attendees for each day will be
capped at 30 to ensure we can meet all requirements for OSHC and external companies.
If your child is experiencing any flu-like symptoms, we ask that you contact the OSHC Manager as
soon as possible to cancel your booking.
Regular temperature checks will be conducted and any child with a reading of 37.5 and above
