Looking back on 2016, we have much to be proud of – leading-edge programs in Home Health VNA, expanded and enhanced support
services for families in Merrimack Valley Hospice, and our private care division, HomeCare, Inc., which serves as a model in the private
duty industry. Thanks to the philanthropic support of individuals, foundations and businesses, our three not-for-profit agencies continue
to bring expert health, hospice and home care to patients in the 110 cities and towns that we serve.
Strong leadership from the Board of Trustees and executive management team, combined with our talented staff, working tirelessly to

4The Complex Care Management Program – Innovation in Action
George had been hospitalized multiple times over several years due to COPD, diabetes and other medical conditions. After three hospitalizations
in one month followed by six weeks at a rehabilitation facility, George needed to get his disease under control. Most importantly, he wanted to be
well enough to visit with Terry, his wife of 64 years, who lives nearby in a nursing home due to the effects of Alzheimer’s disease.
George was referred to Home Health VNA and admitted to the Complex Care Management Program. He was weak, on multiple medications and
unsure of how to manage the symptoms of COPD or monitor his diabetes. He had been unable to visit Terry for months and was determined to do
all he could to be back by her side.
The Complex Care Management team developed a plan of care for George that included nursing visits, physical and occupational therapy and a
social worker to arrange supportive services. But, even with an expert medical team committed to keeping him out of the hospital, George needed
a little extra support to make positive changes. For George that “little extra help” was Evelyn Peralta, his health coach.
Evelyn telephoned daily to check on his health, discuss his goals of care and supported George during his recuperation. She provided positive
feedback, listened when he was frustrated and encouraged him when he needed it most. “Evelyn keeps me out of the hospital,” said George.
“She’s calm, kind and helps me stay on the right track.”
With the help of Evelyn and his home health care team, George has been out of the hospital for more than four months. Most importantly, he’s
back to visiting his beloved wife Terry, spending every Saturday by her side.

6Pediatric Palliative Care – Comprehensive Care for Children with Complex Needs
Tamara Nguyen flashes a bright, excited smile as she shows Child Life Specialist, Maura Power, the pictures her new classmates drew to welcome
her to kindergarten. With the typical enthusiasm of a six year old, Tamara explains that she has fun at school, loves her teacher and wakes each
morning eager to greet the day. But, Tamara wasn’t always so outgoing.
Tamara has Spinal Muscular Atrophy Type II, a progressive neuromuscular disorder characterized by weak muscles and other complications.
She cannot stand or walk and relies on a feeding tube to ensure she receives all the nutrition she needs.
“Tamara was referred to the Pediatric Palliative Care program of Merrimack Valley Hospice by her pediatrician,” said Power. “Although she
responded very well to treatment, she was extremely shy, had minimal emotional expression and was having difficulty coping with her medical
condition. Her family was worried that she wouldn’t be able to make friends, express herself or talk about her medical condition and needs,
especially when classmates asked questions.”
Power began visiting Tamara weekly, providing age appropriate interventions as well as self-expression and play activities to help her cope with
the challenges of her illness. Power also visited Tamara’s kindergarten classroom to teach her classmates about her disease, familiarize them with
her medical equipment and answer questions. Slowly, over the course of several months, Tamara’s shyness began to diminish, and an outgoing
and happy personality emerged.
Today, Tamara has strong coping skills that she can rely on as she grows. She now willingly discusses her medical needs, asks questions about
her condition and has developed the ability to express herself emotionally.

8Our House Became their Home
In 2012, at just 49 years old, Daniel “Dan” Riley was diagnosed with a brain tumor. Although the diagnosis forced Dan to leave his job in the
interior design field, Dan functioned well at home. He even volunteered to help others coping with serious illness, often visiting nursing homes
or driving other patients to medical appointments. “Dan has always been upbeat and cheerful,” said his wife Janet. “He’s a jokester, with a positive
attitude about everything. But, in April 2016, the tumor returned, and he was told that nothing more could be done.”
Just after New Years’, as Dan became increasingly weak and unsteady, he knew it was time to transition to Merrimack Valley Hospice House. At
less than five feet tall and quite petite, Janet couldn’t lift or support his six foot frame, and both Janet and Dan could be injured if he fell. And, Dan
was determined to keep his dignity until the end.
Throughout his illness, Dan and Janet, a deeply spiritual couple, conversed openly and honestly about end of life, and they were resolute in
their desire to spend their last days together. So, when Dan was admitted to the Hospice House, Janet moved right in with him. Now, almost two
months later, Janet is cherishing the time they have together. “Every moment here has been a gift,” said Janet. “I’m so thankful for Dan’s wonderful
care and for the kindness and support of the entire staff.”

10Keeping Elders Comfortable at Home with Coordinated Care and Support
For HomeCare, Inc. scheduling supervisors, Laurie Ouellette and Kathy Bowler, coordinating the schedules of home health aides is like putting
together a jigsaw puzzle. Both Ouellette and Bowler work closely with case managers from Elder Services of the Merrimack Valley (ESMV),
coordinating home care services to low income, frail elders who are struggling to stay at home and in their own community. Without this type
of support, most clients would be in a nursing home or assisted living facility.
The services for each client vary by need, which is determined by case managers from ESMV. Some clients require only light housekeeping, meal
preparation or grocery shopping once or twice a week. Others need support services and personal care three times a day, seven days a week.
“Every day is different,” said Bowler. “Some clients need services at 7:00 a.m. to help them get dressed and ready for adult day care. Others might
need help in the evening to prepare a hot meal and assistance getting ready for bed. We are constantly juggling home care aide schedules to
make sure all our clients’ needs are met, day or night, whenever they may need us.”
“Our aides are also specially trained to notice subtle changes in a client that might otherwise go undetected,” said Ouellette. “They get to know
each client and will report if someone is sick or confused. In this way, we can intervene early and arrange for a nurse to come to the home,
perhaps preventing a hospitalization.”
“We are so fortunate to work with aides who are committed to clients in their care," said Bowler. “Our aides make sure clients are safe and
comfortable at home – where they really want to be.”

Many Steps Toward Charitable Care
Hal Curtis was determined to honor his mother in a very
special way. Lillian Margolis, Hal’s mother, was a passionate
advocate of home health and hospice care. She was introduced
to our agencies when Home Health VNA provided care during
her recovery from a broken hip. Later, she spent more than
17 years as a volunteer for Merrimack Valley Hospice and
provided significant financial support to the Campaign to
Build Merrimack Valley Hospice House.
6 In the fall of 2014, Lillian began her end-of-life journey as a patient of Merrimack Valley
Hospice, passing peacefully in her own home on November 2, Hal’s 60th birthday.
To honor his mother and her compassion for others, Hal pledged $50,000 to start the
Lillian P. Margolis Charitable Fund and challenged his family and friends to match his gift.
To demonstrate his commitment, Hal agreed to hike the entire Appalachian Trail beginning
on his mother’s birthday, May 13, 2016. Hal took a short break this winter, but will resume
his hike this spring. To date, the fund in honor of his mother has raised well over $100,000!
Medical Carts
The Friends of Merrimack Valley Hospice support the patients and families served by
Merrimack Valley Hospice House with an annual gift. In 2016, the group purchased three
new medication carts to be used by nurses who dispense complex medication regimens to
patients at this specialized facility to help those nearing end of life. This gift is part of the
Friends’ second annual gift to benefit the Hospice House and support the care provided

by Merrimack Valley Hospice. In addition, this past year the Friends also donated funds to
landscape the small meditation garden, provided weekly meals to families and contributed
to the Campaign for Subsidized Care.
Arbor Society
Arbor Society members support our roots and the care provided in the homes of thousands
of patients each day. Through the dedicated generosity of our Arbor Society members, we
are able to ensure critical care to patients, provide comfort and support to families and offer
guidance and strength to those facing the challenges of advanced illness or end of life.
Donors who make cumulative contributions totaling $2,500 or more in one year, or have
consecutively given financial support of $1,000 annually over three years or more, are
among our most loyal supporters and are recognized with an inscribed leaf on the Giving
Tree displayed at Merrimack Valley Hospice House.

On any given day, Home Health Foundation cares for more than
3,500 patients in 110 cities and towns. Traveling to provide care in
such a large geographic area means that, in a typical year, our clinicians

ensure patients receive the care and support they need.
Home Health Foundation recently rolled out a fleet of new leased
cars to be used by nurses who typically put significant miles on their
own car in an average work week, traveling to care for patients. The
leased vehicles are branded with our agency name to increase our
visibility in the community.
