HOW TO APPLY   Please take your time to accurately complete the
               pre-application and mail with a postmark date
               between September 14, 2020 and September
               28,2020.
               Each household may only submit one pre-
               application. Duplicate household pre-applications
               will not be considered.

Submit a pre-application on-line or by mail between
              September 14,2020-September 28,2020.
              After pre-application is reviewed and if qualified, a leasing
              associate will schedule an appointment with you to go
              through the additional paperwork required. Credit checks,
              criminal background screening, landlord references, and
              income and asset verifications will be required for all
              applicants.
VISTA VERDE   Interview-At your scheduled appointment we will confirm
APARTMENT     the information supplied on your application and answer
              any concerns you may have. The interview normally takes
APPLICATION   45 minutes. All persons 18 or older must be present. Please
PROCESS       note although applications are processed in the order of the
              waitlist, apartments will be offered on a first-qualified, first
              offered bases.
              Apartment offer-When all documents have been received,
              verified and approved, qualified applicants will be notified
              and move in dates confirmed.
For household members 18 and older you will need to submit
            the following:

                   Valid State or National picture ID (i.e. Drivers License,
                   Passport etc.).
                   Social Security Cards if applicable.
                   Employment: Copies off the first pay-stub for the
                   current calendar year and the most current 3 months
INTERVIEW          of consecutive pay-stubs. A copy of the most recent
                   IRS Tax Returns required for cash paid employment.
CHECKLIST          Unemployment: Printout of statement or copy of last
                   letter showing current monthly benefit.
                   Self-employment: Copy of 1-year IRS Tax Return
                   including Schedule C.
                   SSI or SSA/Disability: SSI-Printout of benefit letter
                   (the date of the letter needs to be within 120 days of
                   move in). SSA-Current year award letter.
GA/AFDC/TANF: Print out of benefits paid in the last
            12 months or last Notice of Action letter (dated
            within 120 days).
            Bank Accounts and Assets: Copies of the 6 most
            resent bank statements for checking and savings
            accounts. 401K or IRA we will need current
            statement.
            Pension & Annuities: Copy of most current statement.

INTERVIEW   Real Estate: Copy of the most recent mortgage
            statement & other relevant owner information.
CHECKLIST   Financial Assistance (this includes regular gifts or
            payments from anyone outside the household): We
            will require a written letter from that person
            providing assistance stating amount and frequency.
            Other: Documentation for regular pay for members
            of the Armed Forces, severance payments,
            settlements, lottery winnings, inheritances etc.
1. Credit (An exception for medical expenses and/or student debt may be
              permitted).
                           Open lines of negative credit with no positive offset.

                          A non-discharged bankruptcy.

              2. Rental History
                          A judgement against an applicant obtained by the current or
              previous landlord.

GROUNDS FOR               An unmet obligation owed to a previous landlord.

DENIAL                    The applicant must have made timely payments of the last year’s
              rental payments.

              3. Personal History
                           Blatant disrespect, violent, threatening, or disruptive behavior
              toward management, the property or                  other residents exhibited
              by an applicant or family member any time prior to move-in.
4. Criminal Background Check
                             The manufacturing, selling or possession of any drugs or illegal
              substances, or established pattern of manufacturing, selling or possession of any
              drugs or illegal substances.

                            Conviction for physical violence to persons or property, violent
              criminal activity, sexual abuse,          illegal      weapons possession, any
              form of assault or act that would threaten the health, safety or right to
                            peaceful enjoyment by other residents or employees who work on
              the property.

              5. Annual Income/Occupancy Standard/other program regulations

              ØPets-We allow one pet, 25 lbs. maximum weight. All
               pets must be licensed and proof of shots with

KNOW ABOUT    ØParking-There are limited numbers of resident
               spaces at Vista Verde Apartments. Parking will be by
VISTA VERDE    permit only. Parking will be restricted to cars owned
               by residents. All cars must be registered in the name
               of the resident and residents must provide proof of
               current auto insurance and a valid driver’s license.
              ØSmoking-Vista Verde Apartments is a 100% Smoke
               Free Community.

ØThe lottery will take place between September 29th-
           30th at Ontario City Hall.
          ØKeeping with COVID restrictions we will have City
           and National CORE employees on-site for the
           lottery process. This will not be a public viewed
           event.
LOTTERY   ØLottery will be done by a excel workbook that will
PROCESS    have first and last name of applicant and a unique
           identifier. The Excel workbook will randomly sort all
           pre-applicants.
          ØLottery winners will be notified through emails
           and/or phone calls by November 1, 2020.
1.   Who can apply for the apartment? Anyone can apply but applicants must
                  meet income, household size and residency status qualifications in order to
                  be placed on the waiting list.

             2.   How many people can apply for an apartment? Depends on the size of the
                  unit. 2 bedroom 2-5 occupants and the 3 bedroom 3-7 occupants.

             3.   Is there a fee to be placed on the waiting list? No.

             4.   What if my income changes and I still have not been called? If your income
FREQUENTLY        changes, you are required to keep us updated of any changes. This would
                  include your address, phone number, number of occupants, etc.

ASKED        5.   Do we need to have a social security card to apply? No, you can also provide
                  an individual taxpayer identification number (ITIN).
QUESTIONS    6.   Is there a fee once my name is called to apply for an apartment? Yes, there
                  will be an application fee of $20 per adult and a holding deposit fee of $100.

             7.   Is there a fee for a pet? Yes, there is a $400 security deposit required for a
                  pet.

             8.   Can the pre-application be delivered in person? Yes, if you are unable to
                  mail or apply on-line, a drop box will be placed at the corporate office
                  located at 9421 Haven Avenue Rancho Cucamonga CA 91730, between
                  September 14, 2020-September 28, 2020.
