What is Open Enrollment?
• This is your opportunity to:
Join the Plan, if you are not a current participant
Make changes in coverage
Add dependents
Confirm, change or add life insurance beneficiaries
• Elections will become effective January 1, 2021, and remain in place until
next year’s annual Open Enrollment, unless you experience a qualifying
event. You have 30 days from the date of the change to notify Human
Resources of a qualifying event.
Change in marital status
Change in number of dependents
Change in employment status

The annual enrollment period is a great time to review your current benefit
elections, make any necessary changes, elect new plans, update your
address, verify dependent and/or beneficiary information, etc.

Review the AWF Benefits Guide
Decide what benefits you would like beginning
January 1, 2021.
Gather social security numbers and dates of
births for all the dependents you are covering.
LOG-IN. All benefit eligible employees
are required to login to www.enroll.apbenefitadvisors.com to make
benefit elections (or waive coverage) for 2021. No paper enrollment
forms will be accepted this year. All changes must be submitted
online via the benefits website.

Log On to: www.enroll.apbenefitadvisors.com
You will be prompted to enter the following:
Username: Enter Full Last Name plus the last 4 digits of your
social security number (no dashes e.g. jones1234)
Password: Enter the last 4 digits of your Social Security Number
(e.g. 1234)
All passwords were reset this year.
You will be prompted to change your password upon entering
site.
Click START MY ENROLLMENT
Enrollment ends on November 11th

Coverage Details
Medical • Carrier remains the same with UnitedHealthcare.
• We continue to offer a reduced rate for wellness participants!
• No changes to the plan designs for 2021.
• There is a small increase on payroll deductions for 2021.
HSA • The 2021 HSA contribution limits are $3,600 for individual and $7,200
for family.
FSA • HCFSA limit remains $2,500
• DCFSA limit remains $5,000
Dental • Carrier remains the same with MetLife.
• We continue to offer two dental plans to choose from (high and low).
• Payroll deductions remain the SAME for 2021!
Vision • Carrier remains the same with UnitedHealthcare.
• One plan option - no plan design changes.
• Payroll deductions remain the SAME for 2021!

Life/Disability • Carrier remains Mutual of Omaha.
• AWF continues to offer 100% company paid: Basic Life and AD&D for
Employees, Spouse & dependent(s)
Short-Term Disability • Option to purchase Short-Term Disability (STD). You may select STD at
Open Enrollment without having to answer any health questions or
complete a health statement! Per-pay rates are available when
enrolling online!
Long-Term Disability • You now have the option to purchase Long-Term Disability (LTD). You
may select LTD at Open Enrollment without having to answer any health
questions or complete a health statement! Per- pay rates are available
when enrolling online!
Pet Insurance • Get a group discount on a pet insurance policy through Nationwide for
your furry family members.
Employee Assistance • AWF offers a comprehensive EAP program at no cost to you!
Program (EAP) • All employees are eligible!
8UHC Medical Plans
• UnitedHealthcare continues to be our
medical, prescription, and vision
insurance provider.
• Continue to have two medical plan
offerings:
• UHC Choice Plus High Deductible
Health Plan (HDHP) with Optional
HSA
• UHC Choice Plus PPO Plan
• In light of the pandemic, consider virtual
care options.

• Only employees who enroll in the Choice Plus Plan HDHP w/optional HSA
are eligible to participate in the HSA.
• Contribution Limits: You can choose to contribute additional funds to your
HSA on a before-tax basis up to the IRS annual Maximums.
• UHC will oversee the administration of both your plan and the HSA using
their partner- Optum Bank.
• IRS Maximum: $3,600 (Individual) & $7,200 (all other tiers)
• If you are or will be age 55 or over during the calendar year, you
may also make a "catch-up" HSA contribution of an additional
$1,000 each year.

Medical/Rx Per Pay Cost
Please note that if you elect coverage for a spouse who is eligible for
coverage under another plan, you may receive a surcharge of $23.08
per pay added to the cost of the medical plan elected.

If you had a wellness check in 2020, the wellness credit will apply
for 2021.
The deadline for wellness checks to be submitted by your doctor to
UHC is December 1, 2020, to guarantee that the wellness rate will
show in your 1st paycheck of 2021.
A new hire or any enrolled employee may move to the wellness rate
during the year without waiting until the next Open Enrollment
provided they had a physical within the appropriate time frames.

Employees who enroll their spouse in the AWF
Medical Insurance Plan may be required to pay and
additional surcharge of $23.08 per pay period should
the individual elect AWF coverage over or in addition
to their own employer’s plan.
NEW THIS YEAR: To remove the additional surcharge, a
spouse must certify they have no eligibility to enroll in
another plan by completing a Spousal Surcharge Form
and submit it by 12/1/20 to Human Resources.
This form can be obtained by downloading from the
“Library” in the enrollment site or from Human
Resources. The form must be submitted back to
Corporate Human Resources by 12/1/20 so the Spousal
Surcharge of $23.08 per pay is removed.

Open Enrollment begins
Wednesday, October 28th – Wednesday, November 11th.
AWF Benefit Guides are being distributed at each plant and will be available
online for immediate access.
Please note that if you elect coverage for a spouse who is eligible for coverage
under another plan, you may receive a surcharge of $23.08 per pay added to
the cost of the medical plan. Please see the Spousal Surcharge Form for
additional details.
This is an “ACTIVE” open enrollment, which means you must complete your
enrollment in the benefits website by November 11th! With the exception of
company paid benefits, current elections will NOT automatically carry over
into 2021!
*Note – after open enrollment, you cannot change your elections throughout the year unless there is a life event change
such as: marriage, divorce, birth or adoption of a child, reduction in work hours, change in spouse’s employment. You have
30 days after the life event to make changes to your benefits. You will need to provide verification of the life event. See your
Benefit Guide for types of verification needed.
