Social Innovation When COVID-19 devastated communities and lives, SE Health was there, dedicated to
continuing that incredible impact and ensuring that the people we care for were safe
at home.

Purpose, Mission, Values, Vision We faced pandemic challenges head on; using Hope and Happiness as a vehicle to drive President & CEO
compassion, innovation and resilience. The result: a year of extraordinary social innovation,
demonstrating our empathy, kindness and leadership in a time of global crisis.
There were more than a few obstacles – limited access to PPE, continually changing
public health guidelines, fear, exhaustion, and grief – but in mere days, we transformed
our care provision, pivoting services to virtual where appropriate, supporting safe
in-person interactions, and keeping our clients and their families connected, the best
way we could.
From our work in the community to our redeployment efforts in hospitals and long-term
care homes, our incredible people answered the call, putting their own needs aside to
serve others. At the same time, our health care leaders made strategic investments to
improve the lives of all Canadians – expanding end-of-life care through our Foundation,
advocating for improved access to home care, creating new models of seniors housing,
and much, much more.
I am truly proud to share stories from the past year showcasing the extraordinary social
innovation we achieved. Our impact is heartfelt, and our purpose is timeless: to bring
Hope and Happiness to people and communities, at all times.
Our team members travelled 22 million+ kilometers
last year to serve their communities. That’s nearly
600 times around the world! There is no limit to
how far we will go to bring Hope and Happiness.

During COVID-19, Bonnie Jennings played peek-a-boo
with her adult daughter for the first time in over thirty
Empowering People years. It was a feeling the SE Health Customer Service
Coordinator savoured while working from home and
caring for her daughter at the same time. But that joy
COVID-19 Impact Snapshot for Bonnie didn’t come until months into the pandemic,
after many long and exhausting days.
Sarah, Bonnie’s daughter, is non-verbal and has a

Care Innovation stopped. It was extremely hard for Bonnie to balance
everything, and she felt overwhelmed.

“I cried every day for three months,” she said.
Research Spotlight
Bonnie says she’s not a spiritual person but when she heard about the virtual spiritual care
services at SE Health, she wanted to try it.
Did You Know? “A wonderful woman named Susan Morgan responded immediately. She told me that she
has experience living and working with people who have disabilities. She was supportive,
non-judgmental and kind.”
In March 2020, when home spiritual care visits halted, Susan and her colleagues seamlessly
switched to Microsoft Teams to support staff and clients to ensure continuity of care.
This care was a lifesaver for Bonnie and her family.
“If I wasn’t at home, I may have missed the chance to play peek-a-boo with Sarah,” said
Bonnie. “I am so grateful.”

IMPACT: Despite the challenges of COVID-19, the people we serve said they
felt just as safe receiving our care at home as they did before the pandemic.
We are proud of our 20,000+ hours invested in one year into transforming
our digital systems, creating COVID-19 screeners, making virtual care more
accessible, and continually developing our technology (equivalent to almost
900 days of 24/7 work).

Aging and Mental Health:
A Virtual Success
Melanie Brown, RPN, is proud to deliver holistic care, ensuring
the mental, emotional and physical well-being of her clients.
Mary is making important choices – at home. She’s been doing that for over 70 years.
“I choose survival, it’s always been that way for me,” said Mary, an Ontario-based artist. “As a baby, I was thrown
between haystacks during the Second World War, when bombers flew over England. I am destined to survive.”
During the first wave of the pandemic, Mary’s “survival” was about making the choice of how to receive care at home
for a foot wound.
In March 2020, when the pandemic began, SE Health quickly mobilized a virtual care strategy, ensuring clients like
Mary had “choices” in receiving care. The strategy certified that all clients would continue to receive safe, quality and
personalized care using communication tools like the telephone and email to promote continuity in care and ensure

A Virtual Success
How do you learn what matters most to Canadians about aging and mental health? You ask them! That’s what the SE
Research Centre did in collaboration with the Canadian Mental Health Association, National Office. Nationwide online
engagement included monthly steering group meetings, 1,000+ survey responses and 50+ Canadians engaged in 4
virtual workshops. A list of top 10 unanswered questions on aging and mental health were generated and prioritized

IMPACT: We know that Canadians want to live, age, and die at home. SE
Health is a leader in delivering palliative care at home and our Foundation’s
mission is to support end-of-life care for people most in need. In the last
year, we helped Canadians to journey through end-of-life safe at home, we
made strategic investments into hospice care at Oak Ridges Hospice and
Journey Home Hospice, and offered 17 eLearning courses for First Nations,
Inuit, and Métis health care providers in communities across Canada to learn
essential skills such as caregiving, end-of-life care, cancer care, and more.

Bergeron – the Saint Elizabeth Foundation’s Medical Director, and her family – than

Launched in 2020, the Building with Mission project is bringing together social innovation and housing initiatives to provide
healthy aging options for older adults. The SE Health team is helping to create a country-wide playbook and assist three
demonstration projects in Ontario and Alberta to initiate and accelerate housing developments with human-centred
design methods. These projects will bridge the existing gap between independent and congregate living for aging
Canadians. Informed by 25+ subject matter experts and stakeholders, and steered by 100+ hours of industry expert
time, our playbook of step-by-step instructions and resources will assist Canadian non-profits to develop innovative

sectors, working to deliver and promote a positive future for older adults. By 2054, Canada’s population of adults over
65 will make up more than 20% of the population, managing multiple health diagnoses, and at the same time, older
adults have made it clear they prefer to live and age at home. The COVID-19 pandemic has highlighted concerns related

Looking to the future of health care is more than just health systems influence for SE Health. It’s about social connectivity
– looking through the lens of culture, society, and how we collaborate and interact as social beings. This is one way we
are lending expertise to the conversation.
Intergenerational bonds are an important factor in how health and aging are addressed and viewed as opportunities or
deficits. Bonds can manifest organically, but according to Justine Giosa, Managing Director, SE Research Centre, there’s
now a greater demand for the health care system to respond to the rising need for social connectivity among younger
and older adults alike. “We need to focus on care that is less about bricks and buildings and more about people and
communities” she says.
A new research paper, for example, showed the benefits of intergenerational connections after a group of nursing
students brought together older adults, people with disabilities and preschool aged children to create a community
garden, finding that if everyone came together to build something, they learned more about each other and from
each other, thus extinguishing any bias or tension between generations.
SE Health has extraordinary thought leadership and
work on the future of aging. We cherish this content
partnership as our joint energies are about broadening
the care and aging conversation beyond the health

Connecting with loved ones promotes happiness,

with Future of Good But for a large portion of Canada’s aging population,
specifically older adults in long-term care homes, that
connectivity was lost – without the opportunity to see

Older Adults Connected During
the Pandemic and Inspires In December 2020, SE Health and the Saint Elizabeth
Ontario Health Study Foundation partnered with technology suppliers to help
older adults across the country to connect with their
loved ones. SE Health staff nominated older clients in
their care and delivered 700+ donated devices, gathering many stories of hardship, but creating enormous Hope and
Happiness for clients and their families along the way. This wonderful partnership has led to a ‘first of its kind’ quality
improvement study, guided by leaders at SE Health, with Ontario Health, Central Region, that aims to better understand
the use of digital devices in long-term care homes, provide options to enhance digital connections, and therefore work
on reducing social isolation for older adults.
