Christ Church Spring Break Camp

Registration deadline – Friday, March 5, 2021
Payment in full due at time of registration.

We will offer 5 days of Spring Break Camp. Spring Break Camp will run the same hours as summer camp —
9am-3pm — with extended care available. For your convenience, we will run a manned drop off in the
morning. We will offer more fun and games during extended care, which begins at 8am if you need to arrive
early, and runs until 5:30pm.

Our day will be full of arts and crafts, games, a Bible lesson, and an afternoon movie.

We will not provide snack or lunch. Please see the snack and drink policy at the end of the packet.

Safety information: Spring Break Camp, Afternoon Adventures and One-Day Camps will follow Christ
Church School procedures. Our main goal is to provide your child/ren with a safe environment. The
following protocols have been set:
        1. Both Spring Break Camp team members and students Kindergarten - 5th grade are required to
wear face coverings at all times. Students in PK-4 will be encouraged to wear face coverings. Children will
not be required to wear masks during snack time while they are eating and drinking, but masks must be put
back on when snack is over.
        2. Children at Spring Break Camp will be grouped according to grade and teacher. This will allow the
daily cohorts to remain together while social distancing.
        3. If anyone in Spring Break Camp becomes ill during the day, they will be escorted to the isolation
area where a parent will be called to pick up the child, and they will remain under observation until they are
picked up.
        4. Time has been scheduled to allow additional hand washing breaks and cleaning time between
rotation.

                 You can also pay per day if you cannot come for the entire week.
                            Each day is $50, which covers 8am-5:30pm.

HOW DO I REGISTER?
Fill out the attached Registration Form. You may mail your forms with your payment to Christ Church, 4845
NE 25th Avenue, Fort Lauderdale, FL 33308, or bring them to the church/school office, which is open
Monday-Friday. 8am–4pm.
We will guarantee your spot if you are registered and paid in full. We will not be able to take walk-ins.
We will not give refunds. Deadline for registration is Friday, March 5, 2021.

MORNING DROP-OFF
Drop-off will be manned from 8:50am–9:10am for the regular camp day. If you arrive after 9:10am you will
need to call the Afternoon Adventures phone, 954.789.9367. If you need to arrive earlier then 8:50am
you must register for extended care.

We are permanently changing the morning drop-off and afternoon pick-up to the west circle drive
facing Northeast 24th Terrace. It is the circle drive used by the school for rainy day dismissal.
AFTERNOON PICK-UP
The regular camp day is over at 3pm, and parents should be in the pick-up line by that time. We will escort
your child to your car. We are aware that an emergency may arise, or a traffic jam may cause a parent to be
late in picking up a camper. If you are involved in this sort of situation, do not worry. Any child who has not
been picked up by 3:10pm will be taken to aftercare and may stay as late as 5:30pm. A “Drop-In” fee of $10
will be collected for this service when your camper is picked up. IF YOU KNOW YOU’RE GOING TO BE

EXTENDED DAY PICK-UP (8am–5:30pm)
Please call 954.789.9367. We will bring your child to you. A late fee will be charged to any child not picked up
by 5:30pm; the late fee is $10 per child for every 5 minutes after the 5:30pm pick-up time

EARLY UNPLANNED PICK UP If you know ahead of time that you will need to pick up your child early for a
doctor’s appointment or other planned event, please give us a note when you drop off in the morning, and
we will bring your child to the West Circle Drive at the requested time.

DOES MY CHILD NEED TO BRING LUNCH? Yes, because we will NOT provide lunch or snacks. Please see
the snack and drink policy at the end of the packet.

WHAT SHOULD MY CHILD WEAR TO CAMP? Campers should wear play clothes, sneakers and bring a
sweatshirt. NO sandals, flip-flops, or Crocs. Please put your child’s name on everything. Money, stuffed
animals, toys, handheld games, iPads, iPhones, etc. are not allowed. We will provide all necessary
equipment and supplies. We will not be responsible for lost items.

2021 Child/Youth Release, Permission Indemnification, & Medical Authorization Agreement

I understand Christ Church United Methodist, Inc. is a not-for-profit church that sponsors various programs (hereinafter “Ministry Programs”) and
that precautions are taken to ensure that the programs and activities with the Ministry Programs in which I am participating are conducted in a
safe and responsible manner. However, I further understand that, because of the nature of activities within the Ministry Programs, in which I am
enrolling my child/youth, __________________________________________

I, do hereby release, indemnify, forever discharge and agree to hold harmless Christ Church United Methodist, Inc. and the Florida United
Methodist Annual Conference, their officers, directors, employees, pastors, volunteers, successors and assigns (collectively called “RELEASEES”),
From any and all liability, claims, or demands from personal injury, sickness, or death, as well as property damage and expenses of any nature
whatsoever, whether legal or administrative, arising out of or in connection with any personal injury, including death, or property damage,
resulting in any way from or in any manner arising from PARTICIPANT’S participation in the Christ Church Ministry Programs whether caused in
whole or in part by negligent acts or failure to act of Christ Church, its agents, servants , employees, pastors or volunteers.

This release, indemnification agreement, and wavier specifically includes, but is not limited to, any claim which might otherwise be brought on
behalf of myself or the PARTICIPANT for (a) negligence or negligent supervision by RELEASEES; (b) any injury or harm, including death, that may
occur while PARTICIPANT is participating in Christ Church Ministry Program or Special Event set forth above, including travel to and from, if
required, before, during, or after the Ministry Program or Event and (c) any negligent act or failure to act on the part of those chosen to
administer emergency medical care to the PARTICIPANT. I acknowledge by my signature below that I have carefully read this release and that I
fully understand that this is a waiver of claim and release of liability of the RELEASEES from any and all claims made by me, or on my behalf, or on
behalf of PARTICIPANT minor child, regardless of whether those claims are caused by the negligent acts or failure to act by Christ Church, its
agents, servants, employees, pastors, or volunteers.

The undersigned further consents to the administration of first aid and/or doctor’s care or any form of medical treatment necessitated by illness
or injury that may require the same.

I further agree to indemnify RELEASEES for any costs or attorney’s fees incurred by the RELEASEES resulting from any claim that I, my spouse, my
child(ren), or heirs may make against any third party as a result of participating in the program activity or event set forth above, even if that claim
is caused in whole or in part by the negligence of the RELEASEES.

By my signature below, it is my understanding that Christ Church will attempt to notify me in case of a medical emergency involving my child/
youth, but if the Church cannot reach me, I consent to the administration of first aid and/or doctor’s care or any form of medical treatment
necessitated by illness or injury for the minor PARTICIPANT and I agree that I will pay for any medical expenses incurred.

Christ Church is granted permission to use any individual or group photograph taken of participant at the event for publicity or brochure purposes.

As a ministry of Christ Church, Christ Church Afternoon Adventures strives to be a place of grace and love for campers,
  counselors, and families. Please review the following rules with your child and help us maintain a safe and loving
                                             environment for your children.

Christ Church Afternoon Adventures Behavior Rules & Expectations:

   •   Treat all others with courtesy and respect. (“The Golden Rule”)

   •   Use appropriate language.

   •   Keep hands and feet to yourself.

   •   Everyone participates in scheduled activities and our daily Bible lesson.

   •   Everyone helps to keep our rooms clean.

    Please understand that parents of students who have difficulty with these rules will be notified. Students who
                    purposely hurt others will be asked to go home for the remainder of the day.
SPRING BREAK CAMP HEALTH POLICY

 The health and safety of children at Spring Break Camp is a top priority. Spring Break Camp staff adhere
to the same policies and procedures of Christ Church School regarding social distancing, face masks, and
  sanitation. Please see the Christ Church School policy regarding these practices. Upon arrival children
 enrolled in Spring Break Camp will be screened with a temperature- and symptom-check by the Spring
Break Camp staff. All children will wash their hands before entering the Spring Break Camp classrooms. If
   a child exhibits COVID-19-like symptoms, a guardian must be available for pickup within 30 minutes.
   Children who exhibit COVID-19-like symptoms will go a designated area in Spring Break Camp away
                                            from other children.

                                      SNACK AND DRINK POLICY

  Due to COVID 19, Afternoon Adventures will not provide snacks or drinks for students. Please pack an
extra snack and water bottle for aftercare. Please send snacks that do not require heating and are able to
                      be opened and enjoyed by the children without assistance.
