1. How will you ensure all workers know how and are able to
keep themselves safe from exposure to COVID-19?
Actions:
• H&S signed up for Ontario ministry newsletter and alerts
• Review changes in guidance provided through the Ontario government and public

COVID-19 safety plan 12. How will you screen for COVID-19?
Actions:
• All employees and essential visitors must complete the Daily COVID-19 Screening
Declaration form before or upon entering the workplace. Managers (or designate) to
track and retain questionnaires at each facility.
• Signs must be posted at all entrances to main work areas, instructing persons to not
enter the premises if they or persons they are in close contact with have COVID-19 like
symptoms. Refer to appendix A for daily screening declaration.
3. How will you control the risk of transmission in your
workplace?
Actions:
• Reduce workforce in office settings, provide equipment and supplies to workers who
can work remotely.
• Make use of virtual training and meeting techniques.
• If training or meetings must be completed face to face, ensure meeting protocols are
being followed. Refer to appendix B for training/meeting protocols.
• Install signage at facilities to remind employees about physical distancing measures,
hand sanitizing, and face covering where required.
• Have only one person in the office washrooms at a time and install signage for locations
with multiple stalls.
• All employees are provided with an individual reusable cloth mask and instructions for
use and maintaining. Disposable face masks are to be maintained at each location.
• Cleaning supplies (disinfectant cleaners and hand sanitizer) and PPE (face coverings, face
shields, and disposable gloves) are maintained at head office and distributed to all
locations.
• Plexiglass barriers are installed in some work areas such as customer service counters
and administrative areas where physical distancing measures are difficult to maintain.
COVID-19 safety plan 2• Signage on proper hand wash techniques should be posted at all sink areas. Soap and
disposable towels to be provided by all sink areas. Handless garbage cans and
dispensers should be used.
• Hand sanitizer stations to be set up by entrance/exit areas to buildings, and in other
popular areas such as near photocopiers.
• High touch areas and surfaces, such as door handles, counter tops, office equipment,
payment machines, kitchen appliances, are to be cleaned throughout the day.
• Where possible, improve and increase building and vehicle fresh air intake, by opening
windows and maintaining HVAC systems
• Use available outdoor space whenever possible for meetings, and customer interations.
• Support self-isolation for workers with symptoms and workers who are close contacts of
COVID-19 cases.
• Communicate by way of email to employees and posted signage proper cough and
sneeze etiquette.
• Lunches and other work breaks should be staggered so that employees can maintain
physical distancing. Limit seating according to the size of the facility.
• Where possible, employees should be scheduled to regularly work in predetermined
groups or pods to minimize contact with other location members.
• A monthly COVID19 site safety inspection to be completed and submitted to head
office. Refer to appendix D.
4. What will you do if there is a potential case, or suspected
exposure to, COVID-19 at your workplace?
Actions:
• Employees are encouraged to complete the self-assessment tool online and to report all
incidents of feeling unwell or potential contact.
• Any employee experiencing COVID-19 like symptoms, or who is in close contact with
someone experiencing such symptoms, is to stay home and immediately call their
supervisor. Such an employee will be directed to complete the government online self-
assessment or call either Telehealth Ontario at 1-866-797-0000 or their primary care
provider (for example family physician).

department for tracking.
• Complete contact tracing for the affected employee and affected workplace. Refer to
appendix C contact tracing form.
• Work with the local public health unit if any workers have COVID-19 or are exposed
to someone with COVID-19. Refer to list of local health units by company facility.

5. How will you manage any new risks caused by changes to
the way you operate your business?
Actions:
• Managers will establish regular check-ins with remote workers.
• We will set up a mental health workshop available for all staff.
• Send out reminders regarding the availability of the employee assistance program (EAP).
• Review in person training requirements and demands to ensure sufficient distancing and
mask requirements are being met.
• New hires or visitors are asked to complete the COVID questionnaire prior to start or
entry of workplaces.
• New employees are provided with face coverings at the time of hire.
• Visiting other work locations should be kept to an absolute minimum and performed
only on a necessary basis.
• Office doors are to be locked to visitors and customers. Arrangements for drop offs or
pickups shall be made in advance of arrival.
• Deliveries - Phone calls, or other means of communication, should take place prior to
making or receiving any deliveries so a scheduled appointment can be made, and
additional instructions can be provided. Any associated paperwork should be left in a
designated area, and where possible the requirement of signatures should be omitted.
• Company fleet vehicles and mobile equipment should be allocated to one driver or
operator as much as possible. When vehicles and mobile equipment are shared
between multiple employees, all touch surfaces must be cleaned between drivers.

Actions:
• HR and H&S will meet on a weekly basis to discuss COVID related activities and issues.
HR and H&S will connect with location managers and operations on a periodic basis to

COVID Meeting/Training Protocols
Prior to scheduling a face to face meeting or training session, evaluate the need for an in-
person event. Determine whether the same results can be accomplished by conducting the
event virtually. A virtual event should be considered as the first choice and is always the
preferred method. However, if an event must be completed in-person, ensure the following
procedures are adhered to.
Prior to the event
Prepare the facility
• Set up the meeting room so all seats are at least six feet apart. Limit the number of
attendees and facilitators to the newly arranged space. The maximum occupancy
number of the meeting room must be based on COVID protocols.
• Ensure the facility has an adequate supply of cleaning products, soap, hand sanitizer,
and additional face coverings. If additional supplies are required, contact the Guelph
office.
• Determine the best route of entry and exit to limit possible interaction with others (ie:
using a side entrance instead of using an entrance through an office space).
• If food or drinks are being provided, ensure each item is individually contained. No
buffets or community style drinks or food are permitted.
• Ensure proper signage and barriers are in place throughout the facility.
• Inform cleaning staff of any additional cleaning that may be required.
• Clean and sanitize all AV equipment, tables, chairs, handles, switches, and other touch
surfaces.
Communication
• Provide information to all attendees and facilitators about entrance and exit
requirements.
• Have all persons complete the daily screening declaration form on the day of.
• Remind all persons to bring their own supplies (ie: pen, notebook, needed PPE), along
with a proper face covering. No one should share equipment, supplies, or PPE.
• If a non-GMK person (ie: outside trainer, member coop employee, contractor, etc) is
attending the event, have the person answer the screening declaration questions in the
24-hour period prior to the event date.
COVID-19 safety plan 11• Inform all attendees and facilitators that if they experience any COVID-19 like symptoms
up to three days after the event, they must inform the event host.
During the event
• Upon arrival, have all persons sign into the event with their name and contact
information. Retain the sign in document.
• Remind all persons to maintain physical distancing and to wear a face covering when
they are unable to do so.
• Remind all persons to sanitize items that they use throughout the event time (ie:
sanitize the washroom after use) or have a designated person to sanitize after each use.
• Stagger people accessing food and drinks during breaks so there is sufficient space to
move around. If more than one doorway, designate and sign an entrance and exit door.
After the event
• Rearrange the facility to a suitable state.
• Remove all left-over items.
• Clean and sanitize all touch surfaces (ie: tables, chairs, door handles, light switches, AV
equipment, etc).

Contact Tracing Form – General Timeline
If you are experiencing sickness, please answer the following questions and put together a
timeline of the two days leading up to the first sign of symptoms.
