Introducing Hammersmith and Fulham Primary Care Trust

The Hammersmith and Fulham Primary Care Trust was formed on April 1st 2002. The Trust is
responsible for providing and commissioning a range of services.

The Trust aims to deliver equitable services that demonstrate access to local NHS care for all.
The appropriate sourcing and development of primary care within the Borough is the
foundation stone for this, in collaboration with patients and the public, the secondary health
care sector, and Social Services.

The Trust adopts an integrated approach to the development of local health and social care
services, and plays a full part in the implementation of the National Health Service
Frameworks for Older People, Mental Health, Coronary Health Disease, and Diabetes. These
frameworks are designed to modernise health and social care and deliver better services to
people through improved quality and efficiency. Existing new service developments include
Walk-in-Centres, Healthy Living Centres, nurse-led clinics, and the transfer of services
previously delivered in hospitals.

London Borough of Hammersmith and Fulham

The London Borough of Hammersmith and Fulham’s current population is approximately
180,000. The Borough has the fourth highest population density in London, comprising the
three distinct communities of Shepherds Bush, Hammersmith and Fulham. There are
currently 30 GP practices with 185,700 registered patients.

Although the Borough is relatively small, it enjoys a wide range of cultural diversity of people
and communities. The Borough has a rich and vibrant future. Positioned near the heart of
London with excellent transport facilities, the Borough is home to an increasing number of
major companies. Hammersmith and Fulham is also host to a plethora of art and
entertainment venues, landmarks, shops, and three football teams: QPR, Fulham and
Chelsea. The Borough enjoys excellent communications due, in part, to its proximity to one of
the world’s leading international airports, Heathrow.

Hammersmith and Fulham has a very mixed population. Almost one in four is of Black or
Asian origin and around six per cent Irish. The Borough’s local children speak some 136
languages between them. The population is growing and expected to rise to 212,000 by
2021. The proportion of young people in the borough is comparatively high, with half the
population being between 17 and 39. Mobility is high, with half the households having lived at
their current address for less than six years.

Many of the traditional manufacturing activities of this part of London are now gone, with a
consequent loss of manual jobs. Unemployment has been high although declining in recent
years Much of the housing stock was built in the nineteenth century and needs extensive
repair. Larger, older council estates suffer from poor environments which exacerbate the
social and economic problems of those who live there. The borough is characterised by
extremes of wealth and poverty.

                                               2
The Hammersmith and Fulham Primary Care Trust has a particularly challenging task in terms
of addressing health inequalities, because the area has both extremely affluent and severely
deprived neighbourhoods. The main causes of illness and death within the Borough are the
same in the UK as a whole: cancer, coronary heart disease, stroke, injuries, and mental
illness.

So if you want the challenge and opportunity – come and join us!

Learning and development

The Trust values staff development as an important component in achieving its aim to provide
high quality patient care. We encourage a learning culture, where training and education
programmes are linked to individual, professional and changing service needs. The Trust
invests in its staff by ensuring that they each have access to training, professional and
personal development, clinical leadership, and guidance, to equip them to deliver the best
possible service to local people. In order to enhance the skills and knowledge of our staff, it is
recognised that these programmes will require protected learning time. By encouraging staff
development in this way, we aim to optimise job satisfaction and career progression for
everyone working with in the Trust.

The principles of multi agency working are followed and staff groups learn together. The Trust
aims to ensure that all staff have access to high quality support and supervision to maintain
clinical and administrative standards.

Improving Working Lives

Hammersmith and Fulham Primary Care Trust is totally committed to ‘Improving Working
Lives’. The Trust recognises that modern health services require modern employment
practices. It understands that staff work best for patients when they can strike a healthy
balance between work and other aspects of their outside work, and the Trust a range of
policies enabling them to do so. The Trust encourages and supports a joint responsibility
with staff to develop a range of flexible working arrangements that balance the needs of
patients and services, and values and supports staff according to the contribution they make
to patient care and meeting the needs of the service. We also provide personal and
professional development and training opportunities that are accessible and open to all staff
irrespective of their working patterns

 Parks and Gardens

Bishops Park, with its 15 tennis courts (nine floodlit) and boating lake, is situated alongside
the Thames. The park originally formed the grounds of Fulham Palace, the historic
residence of the Bishops of London. The gardens at Fulham became famous in the 17th
century when Bishop Compton imported rare species such as the magnolia, and grew them
in Europe for the first time. Although none of his original plants survive, many fine trees
remain. Visitors can also enjoy the botanic beds, herb garden and wisteria pergola within the
old walled garden.

The Hurlingham Club in Fulham offers rugby, football and tennis; and the athletics stadium
was used for the film ‘Chariots of Fire’. The 200-acre Wormwood Scrubs is one of the
largest areas of common land in London. The common has pitches for football, rugby,
softball and bicycle polo. The main park in Hammersmith is Ravenscourt Park, which has
tennis, netball, basketball, crazy golf and bowls facilities. Children’s attractions include
paddling pools, a sandpit and many other play areas. Ravenscourt Park also has an annual
fireworks event, which attracts crowds of over 30,000.
