Open Enrollment 2022
  Itasca County annual Open Enrollment period is November 1 - November 16, 2021
      • Open Enrollment is your opportunity to change your benefit elections for the upcoming plan
          year.
      • To participate in the Flexible Spending Accounts (pretax), you must enroll every year. Even if you
          are currently participating, re-enrollment is required if you plan to participate in 2022.
      • Due to changes in health insurance providers, all employees must complete open enrollment by
          November 16th, 2021.

  What’s changing and staying the same in 2022
  Premiums
  The premiums for the Medica plans will decrease by 5%. New rates are:

HIGH DEDUCTIBLE OPTION 1

  The premiums for the Delta Dental Plan will not increase. The same premiums, $0 per month for
  single coverage and $19.60 per month for family coverage.
  The optional life insurance for employee, spouse, and child will remain the same for 2022. Remember,
  premiums for employee and spouse optional life increase when you reach the 5-year age bands.

Medical
    •   Bulletin changes: Specialty Coupons – The amount of out of pocket (OOP) you spend on
        your specialty drug is the only amount that is applied to your annual deductible.
    •   Medical coverage is changing from MEDICA of MN to Medica.
    •   A generic mandate penalty is when a non-generic drug is used, Medica will only apply the
        generic value to your annual deductible and the balance is not applied.

Delta Dental Benefits are not changing for plan year 2022.
Reminder Include your Dependent Information
Eligible dependents may be added to coverage during Open Enrollment. When adding new
dependents, be sure to include required personal information (i.e. date of birth or SSN) Verification
of eligibility may be required.

Gather and review information
Review your current benefits through KRONOS > My Benefits > Benefits Plans.
Enroll in Kronos
   •   Once in the system, select “My Benefits”, “Benefit Enrollment”
   •   Click on “Start” in the Open Enrollment window.
   •   Review your contacts. You can update or confirm your contact information.
           o To add a new contact, use the “Add+” and complete the required information. Determine if they are
               an Emergency contact, Dependent or Beneficiary. (you can select more than one option)
           o To edit your contact information, use the edit “pencil” to review and/or update.
   •   Once contacts are complete, click on “submit” to start enrollment.
   •   Your available selections are listed. Click on each blue link on the left side of the screen to read instructions
       and to make a change to an election.
   •   Under each selection you may change your coverage or add/drop dependents.
   •   Dependents can be covered under your medical and dental plans until age 26
   •   After you have made all your changes:
           o Select “Confirm & Submit.” Review all of your changes and ensure that your beneficiaries are all up
               to date on your Life Insurance
           o Complete your enrollment by clicking on the “Submit” button.
           o To confirm your elections, you will be prompted to electronically sign your selections using the last 4
               digits of your SS#.
           o Once you electronically sign your changes, you will receive an email confirmation. If you do not
               receive an email confirmation from Kronos, your election has not been submitted. If you need
               assistance, contact the Human Resources Department at 218-327-7836 or
               HumanResources@co.itasca.mn.us.
Reminders
   •   You must enroll in a pretax Flex Spending and Dependent Care pre-tax accounts every year to participate.
   •   You must click on “Accept” prior to 11:59 p.m. on November 16, 2021 or your elections will not be
       made.
   •   All premium changes will affect your December pay periods for your January 1st, 2022 coverage.
After you’ve enrolled
   •   A printable Confirmation Statement of your online elections will appear at the end of your
       enrollment. All enrollment information will be maintained in KRONOS for you to view at anytime.
Medica will mail members new cards to your home address after the first of the year.
Reminder: Receiving a card is not a guarantee of eligibility.
All submitted and accepted elections are final as of 11:59 p.m., November 16, 2020.

What’s open
   MEDICAL
        •   Coverage is with Medica with Medica Passport network
        •   Enroll in or waive coverage.
                o There are 2 plan options:
                           • High Deductible Option 1: 1750/3500
                           • High Deductible Option 2: 3500/7000
                           • There are pre-tax and post-tax options
        •   Change or maintain your medical insurance plan
        •   Add or drop eligible dependents.
        •   Effective dates: January 1, 2022 – December 31, 2022.
   DENTAL
        •   Coverage is with Delta Dental
        •   Enroll or waive coverage.

        •   Effective dates: January 1, 2022 – December 31, 2022.
   LIFE INSURANCE
        STANDARD Life Insurance
        •   Enroll in or increase additional, spouse and dependent Life and Accidental Death & Dismemberment (AD&D)
            coverage.
        •   Effective date: January 1, 2022.
        •   Rates are based on employee age as of January 1.
        •   Spouse rates are based by employees age on January 1
        Prudential Life (NCPERS)
        •   Enrollment is annual
        •   Effective date: January 1, 2022
        •   Rates are based on employees age cover spouse and dependent children
        •   Application forms need to be completed and in the HR department by 11/30/2020. No late applications will
            be accepted.

FLEX SPENDING ACCOUNTS (FSA & Dependent Care)
        •   Enroll in or re-enroll in the 2022 Dependent Care (daycare) t (DCAP) and the 2022 Medical Savings
            Account (FSA) If you currently participate in a any FSA, and want to participate in 2022, you must re-
            enroll during Open Enrollment.
        •   If you are enrolled or contributing to a Health Savings Account (HSA) medical flex is limited to dental
            and vision expenses.

        •   Effective dates: January 1, 2022 through December 31, 2022.

Health Care Benefit Programs
Minnesota Health Care Consortium in collaboration with the North East Service Coop and Medica to provide
benefit programs for medical enrollees:
•     Sanvello – online mental health programs and clinical assessments for employees and families
•     OMADA – Diabetes and disease preventative programs
•     Calllink – Free service to connect with certified nurse practitioners
•     Fit Choices – Fitness Incentive Program
•     Amwell – Licensed physician available by webcam/facetime, available 24/7 in every state
•     Healthy Savings Program – Save up to $50 a week on food with the Healthy Savings Program

    Dental plan highlights
      Dental insurance is open for election changes or additions during Open Enrollment. Dependents to
      age 26 are eligible for dental coverage through Delta Dental.

      Dental Plan features
      Dental Plan Features:
          •   $1,500 maximum payable benefit per member per year
          •   Coverage for both In-Network and Out-of-Network dentists
          •   100% coverage for In-Network Diagnostic preventive care
          •   80% coverage for In-Network basic and major restorative services
          •   80% coverage for Endodontics and Periodontics
          •   80% coverage for Oral Surgery
          •   50% coverage for prosthetics
          •   50% coverage for Prosthetic repairs and adjustments
          •   There is no deductible for this plan

    STANDARD LIFE INSURANCE
    You may purchase additional term life insurance for yourself spouse and your children.

For you: If you are currently enrolled in Additional Life Insurance for an amount less than $150,000, you may
elect to increase your coverage by $5,000, $10,000, $15,000 or $20,000 annually, up to, but not to exceed
the guarantee issue amount of $150,000 without having to answer health questions. If you are currently not
enrolled in Additional Life Insurance, you may elect $5,000, $10,000, $15,000 or $20,000 without having to
answer health questions. While the benefit is in force, your beneficiary would be paid the value of your
policy in the event of your death.
For your Spouse: IF your spouse is currently enrolled in Dependents Life Insurance for an amount less than
$50,000, you may elect to increase coverage by $5,000 or $10,000 annually, up to, but not to exceed the
guarantee issue amount of $50,000 without having to answer health questions. If you are currently not
enrolled in Dependent Life Insurance, you may elect $5,000 or $10,000 without having to answer health
questions. While the benefit is in force, your beneficiary would be paid the value of your policy in the event
of your death.
For your Child(ren): If your child(ren) is/are currently enrolled in Dependent Life Insurance for less than
$10,000, you may elect to increase coverage by $2,000 or $4,000 annually, up to the maximum coverage
amount of $10,000 without having to answer health questions. If your child(ren) is/are not currently enrolled
in Dependent Life Insurance, you may elect $2,000 or $4,000 without having to answer health questions.
One premium covers all your eligible children up to the age of 26.

PRUDENTIAL (NCPERS) Life Insurance
With Group Decreasing Term Life Insurance, your family can have insurance protection against the
unexpected. The money can go towards paying for funeral expenses, mortgage, rent, credit card bills and
other expenses.
    •   One low rate of $16 a month will cover you, your spouse and your child(ren)
    •   Guarantee coverage, no medical exam or health questionnaire required
    •   Completed paper application needs to be to the Human Resources department by 11/30/2020 to
        have coverage for the 2022 plan year.

Pretax plans, also known as Flexible Spending Accounts (FSAs)
These plans include the Medical Flex Spending Account (FSA) the Dependent (Daycare) Expense Account
(DCAP), These plans allow you to set aside dollars before taxes to cover out-of-pocket health, vision and/or
dental expenses with the FSA and dependent daycare expenses with DCAP. Participants generally save 25%-
30% on taxes.
The FSA can be used to help cover deductibles, copays, and coinsurance you may incur for medical, dental, or
out-of-pocket vision expenses, prescription drugs and eligible over-the-counter drugs. If you are currently
enrolled or contribute to a Health Savings Account, your FSA funds are limited to vision and dental.
FURTHER provides participants with a debit card for ease of access. The card will be issued directly from
FURTHER if you are a new member. If you are an existing HSA or FSA member, you will not receive a new card
and any new funds will be deposited to your current debit card. Participants have the option to use the debit
card or submit receipts manually for reimbursement of expenses. Regardless of the method used, you must
retain documentation of expenses. You may be asked by FURTHER or the IRS to substantiate your expenses.

The IRS has not yet announced the 2022 maximum amounts for the Flex Spending Accounts. If no increases
are allowed the 2020 maximums will remain in place for 2022. The following are the current maximums:
    • The FSA maximum is $2,750 per employee.
    • The DCAP annual maximum is $5,000 per family or household.—increased to 10,500??
Plan carefully when determining the amount of money you set aside. The minimum amount you may set aside
in FSA is $200 per year. FSA money that you elected and is not used or reimbursed during the plan year will
not be refunded due to the “use or lose” rule.
If you want to participate in 2022, you must re-enroll during Open Enrollment. All necessary information is
available on the 2022 Open Enrollment website.

Important Open Enrollment Notes
Electronic Enrollment
You can enroll via KRONOS seven days a week, 24 hours a day, during the period of November 1, 2020 –
November 16, 2020.
No changes may be made after 11:59 p.m. on November 16, 2020.
All benefit representative name’s and contact information can be located on the County Intranet under
the Employee Benefit section.

Payroll Deduction
The first deduction for your portion of all benefits, will be taken from the paycheck you are issued on
December 10, 2021. The first deduction for any coverage that requires evidence of insurability will be
taken when the coverage is approved by the carrier. That coverage will take effect the day it is approved.
