Greetings from the President...
Josh Campbell
Greetings Second District,
Get ready for an exciting year in The Second District Dental Society. I want to encourage every-
one to attend the meetings on a regular basis. The community that we as dental professionals
foster is critical to the profession’s success. I truly believe that the best and last hope we have in
dentistry is working through the past, present, and future together supporting each other and
bettering each other. I have observed the opposite of this in other professions.
In the Spirit of Sharing we have an excellent program put together by Dr. Fain. Dr. Malone kicks
off the summer sharing in July, followed by Dr. Trivette and Dr. Fain in August. We have re-
sponded to increased interest in OSHA by bringing in Olivia Wann in September. As always, Dr.
Jubran has an excellent Earl Henry planned. A life coach that has changed many lives, including
mine, is sharing in January and Lee Wilson and the GPR residents will follow in February. The
Ben Alley Lecture is a program on treatment of older patients (more mature as I like to say). A
charity will be featured at each meeting and Dr. Henson has agreed to gives us pertinent updates
on the Board of Dentistry throughout the year.
Thanks to everyone who served this last year. Second District had a great turnout for the board
of delegates. Congrats, to Dr. Fain on being elected as an ADA delegate and Dr. Susan Orwick-
Barnes being elected as TDA Trustee and ADA alternative Delegate. Lastly, thank you to Dr.
Jarrell for what she has done to engage the Young Dentists in Second District. The Orange and
White tailgate she planned was a tremendous success.
Please come be a part (not apart). If you are interested in MORE, come talk with one of the most
excellent officers or Diane Landers and we will work on plugging you into a role. We need people
on committees, as delegates, as mentors, in leadership and more.

Treating TMD & Maintaining Professionalism

Over a long span of years there have been many different ways of treating TMD. Some of these
methods seemed perfect at the time but later proved to have faults. This lecture will demonstrate
a better approach for the doctor who is treating TMD and how to better discuss treatment options
with patients and other professionals.
Dr. Stephen Malone received his Doctorate of Dental Medicine Degree
from the University of Louisville in 1994 and has practiced dentistry in
Knoxville for over 20 years. He believes in the value of continuing dental
education and has pursued hands-on and mentored advanced dental
education every year since earning his doctorate. He participates in multi-
ple dental study clubs and professional organizations where he has taken
a leadership role.
Among the continuing education programs he has attended, The Pankey
Institute for Advanced Dental Education is noteworthy. He was the
youngest dentist to earn the status of Pankey Scholar at this world-
Stephen Malone renowned post-doctoral educational institution and he is now a member
of its Visiting Faculty.
Dr. Malone along with his wife Lisa enjoy spending time with their two sons, Davis and Grant. He
also enjoys the Great Smoky Mountains and photography.

Dr. Trivette was born and raised in Knoxville. She is a Central High
School alumni and continued to earn her Bachelor of Science degree in
Biology from Furman University. Ann completed her Doctorate of Dental
Surgery from the University of Tennessee College of Dentistry in Mem-
phis, Tennessee.
She is a member of the ADA/TDA/SDDS, Academy of General Dentistry
and Pierre Fauchard Academy. She has completed curriculums with the
Pankey Institute for Comprehensive Dentistry and the Dawson Dental
Institute and pursues numerous continuing education opportunities each
year.
Ann is also part of a community of study. She currently is a member of
Ann Trivette Knoxville’s Pankey Study Club and The Oral Design and Implant Study
Club. Ann enjoys volunteering and has found care giving at the Volunteer Ministry Center fulfill-
ing. Ann enjoys time with her family and is active in her church.
Dr. Fain grew up in Knoxville, graduated from the University of Tennes-
see and has practiced here for over 40 years. He has attended thou-
sands of hours of continuing education courses including over 500 hours
at the Pankey Institute as a student and as a visiting faculty member. He
has presented programs at the state and local levels and is a delegate to
the American Dental Association.

Olivia Wann founded Modern Practice Solutions in the year 2000. She
graduated from the Nashville School of Law with a Doctorate in Juris-
prudence and continues to consult with dental practices on managing
risk in a complex regulatory environment.
Olivia graduated magna cum laude from St. Joseph’s College of Maine
with a Bachelor of Science in Health Care Administration. She spent her
early years as a dental office manager and later served as a dental
software support technician for SoftDent. She attended Tennessee
Technology Center’s dental assisting course and was registered as a
Olivia Wann dental assistant for many years. She participated in Georgia Tech’s
Course in Occupational Safety & Health Standards for General Industry.
Olivia served as a course instructor for Cross Country Seminars and instructed over 250 lec-
tures in 50 states. She has authored several compliance manuals and has had numerous
articles published in industry journals.

Christopher D. Ramsey, DMD was raised and currently practices in Jupiter Florida. His
practice focuses on comprehensive esthetic and restorative dentistry. Dr Ramsey re-
ceived his dental degree from Temple University School of Dentistry in Philadelphia
Pennsylvania in 1999.
Dr. Ramsey is a member of the American Academy of Esthetic Dentistry (AAED), a
member of the American Society for Dental Aesthetics (ASDA), and an Accredited mem-
ber of the American Academy of Cosmetic Dentistry (AACD). He is a Graduate of the
Kois Center in Seattle Washington and also an Alumnus of the Pankey Institute in Key
Biscayne Florida.
Christopher Ramsey Dr. Ramsey is on the editorial review boards for the AACD Journal and the Journal of
Implant and Restorative Dentistry. He has published numerous articles on customer
service related topics, adhesive technology, implant therapy, scanning technology and cosmetic dentistry in
several publications such as Journal of Implant and Restorative Dentistry, Journal of Esthetic and Restorative
Dentistry, PPAD, Dentistry Today, Contemporary Esthetics, Signature, and Dental Products Report. Dr Ramsey
is a product consultant for The Dental Advisor, and to numerous dental manufacturers, which allows for greater
insight in to the newer materials and techniques.
Dr Ramsey is called upon to teach the latest techniques in Body Language and Non-verbal communication and
what they mean to the success of our practices.
Dr. Ramsey takes over 200 hours of continuing education a year to stay on the cutting edge of cosmetic den-
tistry, which is a rapidly changing profession. He is the past President of the Florida Academy of Cosmetic Den-

Osteonecrosis of the Jaws – Update 2018

Osteonecrosis of the jaws represents a controversial and complex collection of diagnoses includ-
ing radiation tissue injury, chronic medullary bone infection, and a wound healing dilemma asso-
ciated with antiresorptive and antiangiogenic medications. The latter pathologic entity has per-
plexed the professions of dentistry and medicine since a plethora of cases were described in
2003. While the pathophysiology of medication related osteonecrosis of the jaws (MRONJ) con-
tinues to baffle clinicians, the surgical treatment of this disease demonstrates very favorable
outcomes and an opportunity for functional and esthetic reconstruction of these patients. It is the
purpose of this presentation to review the pathophysiology, staging and surgical treatment of
MRONJ while emphasizing the role of the dental professional in the treatment of this complex
disease.

Krahwinkel Chairman, Department of Oral and Maxillofacial Surgery,
Director of Oral and Maxillofacial Surgery Residency Program, Director
of Oral/Head and Neck Oncologic Surgery Fellowship Program, & Uni-
versity of Tennessee Medical Center, Knoxville and the University of
Tennessee Cancer Institute.

Dr. Wilson will give an update on the State of the Department of General Dentistry at the Univer-

Dr. O. Lee Wilson earned his Bachelor’s Degree from Spring Hill College
in Mobile, AL, in 1982 and then his Doctor of Medical Dentistry from the
University of Alabama in Birmingham in 1987. He then completed his
residency at the University of Tennessee Medical Center at Knoxville in
1989.
Dr. Wilson began practicing in Knoxville in 1989 before establishing Uni-
versity General Dentists in 1998.
Dr. Wilson was awarded a Fellowship in the Academy of General Den-
tistry in 1994, was inducted into the American College of Dentists in 2002,

Techniques for Older Patients

According to the ADA, older adults are the fastest growing patient demographic in the US. Pro-
viding quality dental care for them can be very rewarding but also very challenging due to an
array of complex dental and medical conditions that become more common as we age. This
often requires that the dentist “think outside the box” both in treatment planning and the proce-
dures needed to deliver care appropriate to that patient’s unique situation. Many times, those
techniques are not described in textbooks. Learn practical techniques in the use of silver diamine
fluoride and other new anticaries agents, restoring the older high-risk caries patient (and when to
give up restoring them!), unconventional removable prosthodontics, managing patients on the
new “blood thinners” and much, much more.
Dr. Randy Huffines is recognized as one of the country's leading experts on
aging and oral health. He received his DDS in 1983 from the University of
Tennessee. Dr. Huffines left practice for two years to become one of only a
handful of practicing clinicians in the United States who has completed a two
-year postdoctoral Fellowship in Geriatric Dentistry. During his career he has
treated older and medically complex adults in a traditional private practice, in
hospitals, and in nursing homes. He is a Past President and a Fellow of the
American Society for Geriatric Dentistry, Fellow of the American Association
of Hospital Dentists, and Diplomate of the American Board Special Care
Randy Huffines Dentistry and Fellow of the Royal College of Surgeons, Edinburgh, Scotland.
In 2004 he was appointed to the American Dental Association's Elder Care
Task Force. He has served on the Editorial Board of two major dental drug references. Randy
was the originator of the ADA’s Preventive Office of the Year Award. Randy holds faculty appoint-
ments in colleges of dentistry, medicine, nursing, and allied health sciences. Despite these re-
sponsibilities, Dr. Huffines has spent most of his workweek as a “wet-fingered” clinician providing
comprehensive dental care to older and medically complex patients. This extensive practical
experience is evident throughout his courses as he illustrates concepts with the thousands of
patients he has personally treated. For many years, Dr. Huffines has refused to take financial
compensation from the dental industry as a speaker, consultant, or author. He has taken this
stand to assure course participants that the information they receive will be based on scientific
research and clinical experience instead of on marketing hype.

Dr. Paul Cullum, a Columbia oral and maxillofacial surgeon, was
elected President-elect of the Tennessee Dental Association
(TDA) during the TDA’s 150th annual session in Gatlinburg.
Dr. Cullum is a 1979 graduate of the University of Tennessee
College of Dentistry after receiving his undergraduate degree from
the University of Tennessee, Knoxville. He completed his spe-
cialty training in oral and maxillofacial surgery in 1987. He is a
current member of the American Dental Association (ADA), the
TDA, the Sixth District Dental Society, the American Association
of Oral and Maxillofacial Surgeons (AAOMS), and Tennessee
Society of Oral and Maxillofacial Surgeons (TSOMS). He has
served in numerous leadership positions in these organizations,
which include President (1997-1998 and 2000-2002) of the Sixth
Paul Cullum District Dental Society, President (2009-2011) of TSOMS, Sixth
District Trustee for the TDA Board of Trustees, and Treasurer of
the TDA for the past five years. Dr. Cullum was awarded the TDA Fellowship Award in 2004.
In addition to clinical papers published and presented nationally, Dr. Cullum was instrumental in
the successful efforts of the TDA and the TSOMS in having the ADA-recommended definition of
dentistry incorporated into the Dental Practice Act. The many hours he spent with colleagues and
legislators to win passage of this measure have been recognized by his colleagues and by the
AAOMS.
Dr. Cullum’s dedication and perseverance were evident early as he was an Eagle Scout and
received the God and Country Award from the Boy Scouts. During his military service in the
United States Air Force, he was honored with the Surgeon General’s Award.
