• DOWNLOAD the nomination form and template document for the award that interests you.
• COMPLETE the required information:
-Submissions for our awards honouring a single individual are to be submitted in two sections. Section
A, which is a 250 word summary, is due on November 30, 2017. The summaries will be reviewed by the
selection committees and a shortlist of top contenders will be formed. Those shortlisted will
be asked to complete Section B of the nomination template. Section B of the template is due February 1,
2018.
-Submissions for team and organizational awards are due February 1, 2018.
• SUBMIT your application via email by the date indicated on the nomination form and template to
jbruyere@cchl-ccls.ca.

The National Awards Advisory Committee’s (NAAC) mandate includes both a strategic and an operation-
al focus relating to our National Awards Program. Membership includes board members, chapter chairs, and
members.
As part of its mandate, the committee will review proposals for new awards for submission to the Board of
Directors for final approval; review existing awards on an ongoing basis; recommend ways to improve the awards
program, suggest measures that can be taken to improve awareness of the program; and develop criteria for new
awards related to the College’s strategic plan.

3 National Awards Program 20183M Health Care Quality Awards The Award 3M Health Care encourages institutions and healthcare providers to embrace quality management by developing innovative approaches that bring about sustainable improvement. This award recognizes four important elements: innovation; quality; patient/family engagement; and teamwork, and provides Canadian heath leaders with an excellent opportunity to recognize team members who have applied the quality process to create measurable benefits in their network of services and programs. The winning team in each category will receive a certificate of recognition, a cash prize of $7,500, a travel subsidy of up to $6,000 for six team members to attend the awards ceremony, which will be held during the National Health Leadership Conference, and a $1,000 allowance will be made available for local celebrations. Both the College and 3M Health Care will provide the winning teams with communication support for the awards. Please visit www.cchl-ccls.ca for the full criteria, guidelines, and nomination template for the 3M Health Care Quality Team Awards. Eligibility Awards will be given in two categories: quality improvement initiative(s) within an organization; and quality improvement initiative(s) across a health system. Administrative and operational teams, including interdisciplinary teams, cross-functional teams across all sectors of healthcare are eligible to apply. A team must consist of a minimum of three people. Nominations Nominations must be submitted by February 1, 2018. Results-oriented projects that have created a significant, sustained change within an organization will be considered. Teams may submit one or several quality improvement projects. The project should have been implemented within the past three years and show evidence of sustainability over a minimum of 18 months. Teams may self nominate. The nomination package should include the following: • A completed nomination form; • Using the template, please include an executive summary highlighting the reasons why the team should be considered for the award (the summary statement must be signed by the chief executive officer); a written report; and a brief description of the project, including the name of a contact person, mailing address, email address, and telephone number for publication. 5 National Awards Program 2018Award of Excellence in Mental Health and Quality Improvement
The Award
This award honours a hospital, health authority, community based mental health and addictions program/service, or a leader in the field
that demonstrates evidence-informed and sustained quality improvements (QI) in the area of mental health and addictions.
The recipient will receive an etched glass trophy, paid travel and accommodations for one individual to attend the College’s Honouring
Health Leadership event and a complimentary registration to the National Health Leadership Conference. In addition, the recipient
will receive a $7,500 grant for the organization to use to continue their QI activities in their mental health program and $1,000 for an
educational leadership event and translation of best practices within the winning
organization.
Eligibility
This award is open to individuals and organizations that demonstrate: (a) recent (two years) application of evidence-based quality im-
provements in their health system which improves outcomes for patients with mental illness and/or addictions,
(b) sustained system/program improvements. Possible nominees could include, but are not limited to:
• a hospital based mental health program providing a continuum of services;
• a community-based mental health and addictions program that is publicly funded and has official partnerships with hospitals as part
of the continuum of care for the treatment of mental health and addictions;
• a school based program that supports mental health for children/youth;
• a corrections-based mental health and addictions program; and
• a recognized health systems leader in the field of mental health and addictions.
Nominations
Nominations must be made by a member of the College and be submitted by February 1, 2018. To submit a nomination, download

• the completed nomination form;
• a letter of nomination from a member of the College;

The Award
This award recognizes and honours the meaningful contributions of individuals or teams, who provide health services, for acts
of caring and compassion that go above and beyond the call of duty, which inspire others and have a profound and lasting
impact. This can be interpreted broadly to include any act that improves an individual patient’s health and/or spirit, the lives
of the patient’s family and/or the community. This award is a vehicle to share their stories with colleagues from across Canada
and to help inspire others.
The winner will receive an etched glass trophy, paid travel and accommodations to attend the College’s Honouring Health
Leadership event, and a complimentary registration to the National Health Leadership Conference.
Eligibility
This award is open to all Canadian healthcare professionals, leaders, teams, and volunteers who have had a marked impact on
patients, their family or the community. This award is open to College members and non-members alike. Programs,
stand-alone acts, and special initiatives from across the continuum of care are eligible for consideration.
Nominations
Nominations must be made by a member of the College. We are asking nominators to submit their nomination in two
sections. Section A, which is to be submitted by November 30, 2017, is a 250 word summary. The summary should touch
on the eligibility criteria and illustrate why you feel that the nominee should win this award. The summaries will be reviewed
by the selection committee and a shortlist of top contenders will be formed. Those on the shortlist will be asked to complete

7 National Awards Program 2018Chapter Awards for Distinguished Service
The Award
This award provides an opportunity for chapters to recognize a member who has made a significant contribution to their chapter. The
recipient will be recognized at a major chapter meeting and will be mentioned at the College’s Annual General Meeting. Recipients
will receive a certificate of appreciation from their chapter. Travel and accommodations to the College’s Annual General Meeting is not
included.
Eligibility
The nominees for this award must be members of the College with a minimum of three years of consecutive membership in the College.
In addition, nominees must have, within the past three years, participated in College activities and demonstrated leadership in
educational programming, member recruitment or other areas of voluntary contribution, not necessarily confined to the chapter level,
but within their respective province.
Nominations
Nominations must be made by a College member, preferably within the same province or chapter as the nominee. Nominations are to
be submitted to the nominee’s chapter’s executive committee, who will choose the winners.

The nomination package should include the following:
• a completed nomination form;
• a letter of nomination; and
• using the template, briefly describe the individual’s contributions to the chapter, any positions held by the nominee within the

8 National Awards Program 2018CCHL Distinguished Leadership Award
The Award
The CCHL Distinguished Leadership Award honours a Champion of Performance Improvement. Winners of this Award
will be passionate and visionary leaders who have led transformative change, demonstrated exemplary engagement and
collaboration and a dedication to building leadership capacity. The successful nominee will be:
A senior leader who demonstrates a commitment to life-long learning and the growth of emerging leaders;
Recognized by their peer community for their passion for health leadership; and
Has made lasting contributions to their communities, organizations, patients, and families.
The winner will receive:
A lifetime membership in the College;
An etched glass trophy;
Paid travel and accommodations to attend the College's Honouring Health Leadership event; and
A complimentary registration to the National Health Leadership Conference.
Eligibility
To be considered for this award, the nominees must hold, or have recently held, a senior leadership position within a
healthcare organization, regional health authority or local health integration network, CHE or FCCHL designation
preferred. (retired persons are eligible).
Nominations
Nominations will be accepted exclusively from the College's past chairs of the Board of Directors; College Fellows; Chapter
Chairs, and past recipients of the College Award for Distinguished Service and the College Honourary Life Membership
Award. The recipient will be selected by the Executive Committee of the College's Board of Directors. The award sponsor
will join the selection panel as an ex-officio participant. Nominations must be submitted by February 22, 2018.
Please include the following in your nomination package:
• the completed nomination form and nomination template;
• a letter of support, authored by a College member, other than the nominator; and
• the nominee's biography, not to exceed one page in length.

9 National Awards Program 2018Energy and Environmental Stewardship Award
The Award
This award recognizes a progressive healthcare organization that has implemented organizational wide initiatives demonstrating environmental
responsibility through the reduction of energy usage, the preservation of natural resources, and effective waste diversion solutions. Possible initiatives
include the implementation of energy efficient building systems and solutions, comprehensive energy and environmental awareness programs, waste
management initiatives, and green procurement practices.
The winner will receive an etched glass trophy, a framed certificate of recognition, paid travel and accommodations for one individual to attend the
College’s Honouring Health Leadership event, and a complimentary registration to attend the National Health Leadership Conference. In addition,
the winner will receive $2,000 cash prize to be directed to the organization’s foundation or charity of choice.
Eligibility
All Canadian healthcare organizations are eligible for this award, regardless of size. However, the following criteria must be satisfied:
• Demonstrate steps taken to ensure sustainability of environmental initiatives;
• Show evidence of results achieved through the implementation of environmental initiatives;
• Illustrate initiatives taken to engage staff and the community;
• Explain how these initiatives have resulted in an improvement in the overall environment; and
• Provide evidence that these initiatives are in line with the corporate business plan endorsed by the senior management team.
Nominations
Nominations must be made by a member of the College who is an employee of the nominated organization, and be submitted by February 1, 2018.
To submit a nomination, please download the nomination form and template from www.cchl-ccls.ca. The nomination package should include the
following:
• A completed nomination form;
• A letter of support from the chief executive or C-level designate, who is a member of the College; and
• Using the template, describe the energy and environmental stewardship program undertaken, including results. Describe any additional

The Award
This award honours a forward thinking healthcare organization that has demonstrated leadership in creating and promoting diversity and inclusion to
improve the environment for its employees, and to better service their customers/patients, and the community.
The recipient will receive an etched glass trophy, paid travel and accommodations for one individual to attend the College’s Honouring Health
Leadership event, and a complimentary registration to the National Health Leadership Conference.
Eligibility
The award is open to all Canadian healthcare organizations. Those organizations that have demonstrated effective diversity and inclusion are eligible to
be nominated. While special acts or other one-time achievements will be considered, this award will place special emphasis on effective and sustained
efforts worthy of recognition. This justification will serve as the principal basis for selection of the award recipient.

Nominations must be made by a member of the College and be submitted by February 1, 2018. To submit a nomination, please download the

• the completed nomination form;

The Award
This award recognizes individuals and/or teams that are committed to improving patient safety within the healthcare environment, through leadership,
culture, best practices, innovation, and change management expertise.
The recipient will receive an etched glass trophy, a cash prize of $1,000, paid travel and accommodations for one individual to attend the College’s
Honouring Health Leadership event, and a complimentary registration to the National Health Leadership Conference.
Eligibility
Individuals and teams are eligible for this award. To be considered for this award, nominees must demonstrate significant progress, investment,
commitment, and validation in the area of patient safety. Health and safety programs and initiatives must demonstrate measurable and sustainable
improvements in the following areas:
• use of leading practices;
• establishment of a collaborative approach, that includes engaging senior management;
• introduction of new technologies/innovative practices;
• adoption of a comprehensive approach to program implementation (i.e. performance indicators, educational component, benchmarking,
system-wide implementation);
• evidence of sustainability (e.g. integration into the business/operational plan).
Nominations
Nominations must be made by a member of the College, and be submitted by February 1, 2018. To submit a nomination, please download the
nomination form and template from www.cchl-ccls.ca. The nomination package should include the following:
• a completed nomination form;
• a letter of nomination;
• no more than two letters of support, with one being from the nominee’s chief executive officer or vice-president, who is a member of the
College;
• using the template, outlining the above criteria, demonstrating the measures that have resulted in a significant reduction in the number of
injuries sustained, trend lines to show the progress, reduction statistics, actual safety records, number of people impacted; and
• other considerations the nominator feels may be of interest to the selection committee

12 National Awards Program 2018Mentorship Award
The Award
The Mentorship Award is presented to a leader in the healthcare system who demonstrates exemplary, sustained commitment to mentoring, and
inspiring healthcare leadership.
The winner will receive an etched glass trophy, paid travel and accommodations to attend the College’s Honouring Health Leadership event, and a
complimentary registration to the National Health Leadership Conference. In addition, the winner will receive a $2,500 cash prize to be directed to
the recipient’s health system foundation of choice, to support education and/or leadership initiatives.
Mentorship is a one-on-one relationship in which one person (the mentor) helps develop another (the mentee or protégé) by sharing wisdom, insight
and skills, and creating valuable and multifaceted access to organizational dynamics and processes.1 The mentor may or may not be employed in the
same organization as the protégé or be in the protégé’s chain of command or profession.2
Eligibility
The nominee must be a member of the College. The successful nominee will be viewed by the profession as a recognized leader who, through dedicated
service, has made a substantial contribution to healthcare mentorship within their own organization and/or within the health system. To be considered
for this award, the nominee will have demonstrated mentorship skills in the following areas:
• providing constructive feedback in a professional manner;
• assisting in building social networking connections within their own organization and/or within the health system;
• serving as a role model and leader;
• providing advice with respect to professional development; and
• creating a positive, trusting relationship amongst professionals to foster personal and professional growth.
As a testament to their commitment to mentorship, the nominee will have directly or indirectly mentored numerous healthcare leaders, leaving a

This award recognizes a corporate member who has consistently, over a period of several years, helped the College achieve its
vision, mission, values, and strategic directions.

Eligibility
The award will be given to a corporate member organization in good standing who has, at the minimum, five years of
continuous involvement with the College and has supported the College’s activities and programs in a tangible manner
(i.e. provided company marketing resources to the College and/or financial support for College events or awards, and
participated on College committees or task forces). In addition, the successful nominee must have provided advice on ways
and means to enhance the College’s corporate membership program and assistance to the College in the achievement of its
objectives.

The Recognition
Value-based healthcare is gaining solid traction in Canada and globally, as health systems seek to improve delivery of highly integrated care.
Value-based healthcare is about creating value for patients throughout the full cycle of care and aims to maximize health outcomes per dollar spent
which is referred to as patient value1. Aligning with a value-based approach means reforming an entrenched system in which payments are made for
volumes of service to a new approach where payment is tied to value and quality.2
This recognition aims to increase the profile and understanding of value-based healthcare by honouring an organization, or team, that is deliberate
in changing the way that care is delivered, resulting in improved patient outcomes. This means that patients are being optimally cared for at the right
time, in the right setting and at the right cost.
The winner will receive an etched glass trophy, paid travel and accommodations to attend the College’s Honouring Health Leadership event, and a
complimentary registration to the National Health Leadership Conference.
Given that value-based healthcare is in its early stages in Canada, nominated teams or organizations may have limited experience to reference
regarding their new initiatives based on the patient value created. As such, this recognition will evaluate:
• the organization/team’s approach to value-based healthcare;
• the patient-centered outcomes captured;
• cost to the fullest extent it can be captured for the patient journey in the initiative; and
• most importantly, the patient-reported outcomes including how their care and experience has improved under this model of care.
Eligibility
Teams and organizations are eligible to apply. Teams that have collaborated on the initiative can exist within one single organization, or within a health
system.
The successful nominee will embrace the concept of value-based healthcare with a focus on partner and patient engagement. Moreover they will
measure patient relevant outcomes and will use these outcomes to inform improvements to care. The successful nominee will strive to create value for
patients, moving beyond effectiveness, determined to create efficiency in care. Their approach to care will be inclusive of patients, be driven by the care
team in a collaborative manner with other partners under a focus of continuous learning and improvement.

It is presented to one student from each of the six Canadian universities offering graduate programs in health services administration. Robert Wood Johnson earned a world-wide reputation for his innovative management techniques, farsighted socioeconomic reforms, and dedicated public service. He helped establish the world’s first school of hospital administration, created a unique industry/hospital collaboration for improving hospital management efficiency, and helped set up a prototype nurse recruitment program. The award consists of $2,000 cash prize, a certificate of merit, and paid travel and accommodations to attend the awards ceremony, which will be held during the National Health Leadership Conference. Eligibility Students must be enrolled in a full or part-time graduate program of health services administration at the University of British Columbia, University of Alberta, University of Toronto, University of Ottawa, Université de Montréal, or Dalhousie University. The Robert Zed Young Health Leader Award is presented to a Canadian healthcare leader who has demonstrated leadership
in improving the effectiveness and sustainability of Canada’s health system.

Leadership event, and a complimentary registration to the National Health Leadership Conference.
Eligibility
Nominations are invited for leaders, age 40 or younger, and must satisfy the following criteria:
• be a member of the College;
• has, in the last 5-10 years, shown leadership growth and advancement in their healthcare career, and demonstrates
potential for senior healthcare leadership positions in the future, in any area of health services, including public health,
health prevention, health promotion, acute care, long term care, rehabilitation, mental health and drug dependency
services, national, provincial or local health organizations, health charities, private sector health organizations or any
related area; and
• has demonstrated outstanding vision and leadership through a project, program, publication, community involvement,
philanthropy, or other activity that has resulted in measurable improvements within their organization and/or within the
health system.
