lhsgradnight2022@gmail.comGeneral Information Attention Parents! Graduation Day and the All-Night Graduation Party will be Thursday, June 9, 2022. The Liberty High School All-Night Graduation Party is sponsored by parents of LHS students and local businesses. We want to make sure EVERY student has an opportunity to attend and that every parent has an opportunity to participate. What is a Grad Night Party? It's an opportunity for your graduate to celebrate their graduation with their friends in a safe and sober environment. They will be taken to an undisclosed location where entertainment and food is provided. It's the last time the Class of 2022 will be together as a group! Each student should know what is expected of them so that those supervising the party can provide a safe environment for everyone. Please read the information below to ensure everyone has a fun time. Admission: The completed Authorization & Waiver Form and payment need to be in before Friday, June 3, 2022. This form can be found in the front office of Liberty, as well as many Liberty events through the year. IMPORTANT NOTICE: No tickets will be distributed — your student's name will appear on a list at the “Check-in” registration tables the night of the party. There will be many opportunities to purchase admission as we will be at many events. In addition, you may turn in your Authorization & Waiver form and payment to the front office at any time. You may also pay for your student's admission through the Grad Night web page for details: http://www.libertyboosters.com/gradnight If you pay online, please be sure to turn in the mandatory Authorization & Waiver form to the front office. Your student will not be able to attend if the Authorization & Waiver form is not turned in. Only 2022 graduating seniors may attend Grad Night 2022. (No friends or relatives are permitted to accompany students unless they are volunteering and have been approved in advance.) To maintain security, all students must be at the school by 11:00 pm to check in. If a student has purchased admission and does not check-in at the school that night, their parent or guardian will be called. Security: The event site has security and will not allow anyone on the premises that is not on the approved list. Parent volunteers will also be present and stationed throughout the building all night. What to bring: Cell phones are permitted but students are responsible to keep track of them. Prescription medication if needed. (Medication must be listed on the signed waiver form.) Students are NOT allowed to bring any personal items including purses, wallets, bags, backpacks, fanny packs, etc. Bags will be provided for students to store car keys, prizes, etc. during the party. The bag room will be maintained by parent volunteers and students will be allowed access to their bags as they need it with supervision. NO outside food and/or drink is permitted. There will be a large variety of food and drink at the party. Dismissal: All students will be brought back to Liberty High School around 4:30 am, Friday, June 10 th. We HIGHLY recommend parents pick up their graduate to prevent them from driving after being up all night.

AUTHORIZATION & WAIVER FORM
The undersigned parent(s) or legal guardian(s) of the minor child or child over 18 years of age named below
states as follows:
l, for myself, or on behalf of my child, and/or my child over 18 years of age aware that normal and usual
entertainment, athletic, and sports-related activities have certain inherent risks and may cause injury to
participants. However, l, for myself, or on behalf of my child, and/or my child over 18 years of age want to
participate in the All-Night Graduation Party including, without limitation, in activities such as, but not limited to:
inflatables, dancing, general entertainment, athletic activities, equipment, and transportation to locations
determined by the All-Night Graduation Party Committee. l, for myself, or on behalf of my child, and/or my child
over 18 years of age give my unqualified permission and consent to participate in the activities, subject only to
any specific limitations noted below. I/My child has the necessary skills and is able to participate in all
reasonably anticipated aspects of the activities and has no limiting health conditions except as noted below.
l, for myself, or on behalf of my child, and/or my child over 18 years of age hereby indemnify, release, hold
harmless, and forever discharge the Liberty All-Night Graduation Party Committee, the Liberty Community
Club, Hillsboro School District and its agents, employees, officers, directors, affiliates, successors, and
assigns, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits,
damages, and liabilities, or every kind and nature, whether known or unknown, in law or equity, that I or my
child ever had or may have, arising from or in any way related to my child's participation in any activities
conducted by, on the premises of, or for the benefit of, the All-Night Graduation Party Committee, and the
Hillsboro School District; provided that this waiver of liability does not apply to any acts of gross negligence, or
intentional, willful, or wanton misconduct.
This Authorization & Waiver is binding upon me, and my minor child, and/or child over 18 years of age, our
heirs, executors, legal representatives, successors, and assigns. The provisions of this Authorization & Waiver
will continue in full force and effect even after the termination of the activities conducted by, on the premises
of, or for the benefit of, the All-Night Graduation Party Committee, and the Hillsboro School District whether by
agreement, by operation of law, or otherwise.
This Authorization & Waiver is governed by the laws of the State of Oregon and is intended to be as broad
and inclusive as is permitted by that law. If any provision of this Authorization & Waiver is held invalid or
unenforceable by a court of competent jurisdiction, the remaining provisions will continue to be fully
effective.
This Authorization & Waiver contains the entire agreement between the parties and supersedes any prior
written or oral agreements between them concerning the subject matter of this Authorization & Waiver. The
provisions of this Authorization & Waiver may be waived, altered, amended, or repealed, in whole or in part,
only upon the prior written consent of all parties.
In the event I am unable to authorize medical decisions, or I cannot be reached on behalf of my child, I
authorize and direct any adult activities sponsor, chaperone, or group leader representing the All-Night
Graduation Party Committee to make emergency medical decisions for my child chaperones and to disclose
such information to a medical professional in the event my child should require medical care.Medical Conditions, Prescription Medication and/or Food Allergies: I/My child has

For the protection of everyone who attends the ALL-NIGHT GRADUATION party, we require that SENIORS
AND Parents/Guardians read and sign below, which is necessary for admission to the party.
I. I understand that the purpose of the ALL-NIGHT GRADUATION PARTY is to celebrate a great
achievement in a fun, responsible and safe manner.
II. I understand that by attending the ALL-NIGHT GRADUATION PARTY, I have agreed to remain at the
party until the end (anticipated return to Liberty High School is approximately 4:30 AM), unless prior
arrangements are made in advance with All-Night Grad committee or in the case of illness.
III. I agree that I will NOT arrive at the ALL-NIGHT GRADUATION PARTY under the influence of any
intoxicating substance NOR bring or use any alcohol or other drug, including tobacco or nicotine
products, during the duration of the party.
IV. I understand that my bags and other personal effects will be searched for illegal or intoxicating
substances and prohibited substances before the party. Any intoxicating or illegal substances
including tobacco and nicotine products will be confiscated by police officers or chaperones.
Offenders will be subject to legal consequences and not allowed to attend the party.
V. I understand that ample food and beverages will be provided at the party and I agree to not bring
beverages or containers of any kind unless special circumstances apply and have been made known
to the All-Night Grad Party Committee at the time of registration.
VI. I recognize the danger of driving without sleep; therefore, it is recommended I arrange to be picked up
after the party.
VII. I understand that if I violate this agreement, my parent or guardian may be asked to pick me up from
the party at any time during the night.
Refunds are not possible, but we will work with you to try and find another student to exchange with if needed.
I understand the terms of this Authorization & Waiver and I have willingly signed it as my own free act. Please
note that both the participating Senior and parent/guardian must sign, even if the student is 18 years or older.Please write legibly.

ACKNOWLEDGEMENT AND WAIVER OF LIABILITY
By signing below, I acknowledge the potential risks associated with participating in Liberty High Schools All
Night Safe and Sober Grad Night Party on Saturday June 12th, 2021. Those risks include, but are not limited
to the possible exposure to COVID-19 or other communicable diseases. I and/or my parents/guardians (if I am
under the age of 18) agree to hold the Hillsboro School District and any associated planning groups harmless
for any illness I may contract as a result of participating in this activity.
I agree that I will not participate if I have knowingly been in close contact with anyone who has been diagnosed
with COVID-19, or if I am experiencing any of the symptoms associated with COVID-19 (fever, cough,
shortness of breath, diarrhea, vomiting, etc.).
I further agree to a health screening prior to entry at the event, to include a temperature scan of my forehead
and questions about both my personal health and whether or not I have been in close contact with an ill
individual. I agree to wear a mask at all times, except while eating or drinking, maintain appropriate physical
distancing, and follow all other health and safety guidelines, rules, and instructions that are both posted and
given as verbal reminders by adult supervisors at the event.

Al firmar a continuación, reconozco los riesgos potenciales asociados con la participación en la fiesta All Night
Safe y Sober Grad Night Party de Liberty High Schools el sábado 12 de junio de 2021. Esos riesgos incluyen,
entre otros, la posible exposición a COVID-19 u otras enfermedades. Yo y / o mis padres / tutores (si soy menor
de 18 años) acordamos mantener indemne al Distrito Escolar de Hillsboro y a cualquier grupo de planificación
asociado por cualquier enfermedad que pueda contraer como resultado de participar en esta actividad.
Acepto que no participaré si a sabiendas he estado en contacto cercano con alguien que haya sido
diagnosticado con COVID-19, o si estoy experimentando alguno de los síntomas asociados con COVID-19

distancia física adecuada y sigo todas las demás pautas, reglas e instrucciones de salud y seguridad que se
publican y se dan como recordatorios verbales por parte de los supervisores adultos en el evento.
