The Chautauqua Center (TCC)is excited to break ground on a new state-of-the-art
facility in Dunkirk, NY. This capital campaign seeks $2M to bridge the funding gap for

In 2011, local stakeholders convened to create the
Chautauqua 20/20 Comprehensive Plan under the
leadership of the County Executive Greg Edwards. The plan announced strategies to maximize the
region’s untapped potential and includes healthcare priorities such as the creation of an FQHC,
implementation of incentives to attract medical professionals to the area, and the establishment of
health and wellness corridors in Dunkirk and Jamestown.

Economic Impact

The Community Health Care Association of New York State provides an annual report documenting
TCC’s economic impact and how the FQHC contributes to Chautauqua County’s economy. The most
recent figures from 2018 reveal that TCC generated $11,453,137 for the local economy. $7,592,079
resulted from direct spending while $3,861,058 was attributed to community spending.

The report also shows that:

    •   74% of our patients are below the poverty line
    •   17% of our patients are Spanish speaking
    •   36% of our patients are children

Community Impact
Young professionals consider a community’s education and healthcare system as top indicators when
deciding where to relocate. Our new clinic site in Dunkirk will be an asset to the community at large by
offering quality healthcare in a modern setting and will further enhance Chautauqua County’s offerings
for young families, giving people another great reason to relocate or stay in our area.

The FQHC adds to the sustainability of the local health system through referrals and partnership with
other organizations. TCC refers lab work, x rays, specialists and other ancillary services to the local
hospitals and community-based health organizations for patients that may not have had access to care
in the past.

Job Creation, Physician Recruitment and
Retention
Thousands of locals have come to rely on TCC’s affordable health services that were once out of reach
due to a variety of obstacles including a lack of medical professionals serving our region. The FQHC is
always accepting new patients, takes walk-ins and offers same day appointments for most services.

Setting us apart from other providers in the county, each practice site is recognized as an access point to
receive loan repayment incentives from the National Health Service Corps. This allows a variety of
licensed clinicians and mid-level health professionals to receive substantial loan forgiveness benefits in
exchange for a commitment of service to TCC. In addition to our loan repayment incentives, the modern
amenities that will be available at the new Dunkirk location will have a major impact on our ability to
recruit and retain providers.

Projected Growth
Based on current figures, TCC officials predict that by 2022:

As patient numbers grow, so has the urgency for a new site in Dunkirk where all services can be
delivered under one roof. The new facility in Dunkirk will serve as a one stop shop that allows for
improved communication among providers, streamlined services and greater access to care. This
integrated care model sets TCC apart and allows for a team-based approach to address a wider range of
patient needs during a single visit.

Other support services include care coordination, facilitation of referrals and transportation, medical
translation, and assistance with choosing and enrolling in health insurance enrollment through the NYS
health marketplace.

Similar to the new clinic in Jamestown that opened in 2019, the Dunkirk site is modern and industrial,
and will be located within a corridor of other health services in the downtown core. It will be built on
land that is currently vacant, and compliment the health care offerings by neighboring businesses such
as Chautauqua Physical and Occupational Therapy and GLPP Lakeshore Orthopedic Group, PC.

Dunkirk Site Budget
TCC officials have already raised a significant amount of the funds needed for the $6M project. In August
2019, TCC received a $3M grant from the Statewide Health Care Facility Transformation Program from
the New York State Department of Health to support the project. In September 2020, we received a
$1M grant through Federal Emergency Management Agency for the project.

We are reaching out to individual donors and community stakeholders to bridge the $2M funding gap
for the project. A complete list of sponsorship and recognition levels is included at the end of this
booklet.

Giving to the Campaign
Donors can pledge support to this campaign with payments of cash, checks, online giving or letters of
commitments. In some cases, donors may wish to pledge a commitment over a period of three years.

Are gifts to The Chautauqua Center deductible?
Yes. The Chautauqua Center is a 501(c)(3) charitable entity. Gifts entitle donors to a charitable income
tax deduction when submitting an itemized tax return. The Chautauqua Center will send donors a
receipt that will serve as confirmation of the gift for tax purposes.

How might my gift be recognized?
A variety of recognition opportunities have been developed for campaign gifts of $5,000 and greater to
provide visible recognition for donors. These include name recognition for a specific period of time
within targeted high-traffic areas of each site based on the donation level. Foundation and corporate
sponsors will also receive recognition in marketing and outreach materials for a time period based on
level of contribution. Donors may also elect to contribute anonymously or make a gift in memory or
honor of someone else.

Awards
Since opening its first site in 2013, TCC has been recognized as a health
care leader in the region. In 2017, TCC proudly achieved the designation
of a Patient Centered Medical Home (PCMH), reflecting its excellent
track record of providing advanced primary care by taking the extra step
for patients and those in the community.

In 2019, the Health Resources & Service Administration (HRSA) ranked TCC as 3rd
among all other FQHCs in WNY. Nationally, TCC was recognized in four categories of achievement:
Health Center Quality Leader, Health Disparities Reducer, Access Enhancer, and Advancing Health
Information Technology (HIT) For Quality

In 2020 we were once again recognized by the Health Resources & Service Administration (HRSA) as a
