Superannuation Forms you need to complete
If you have opted for Choice in Super, you need to attach a Everyone MUST complete the following forms:

attach a document confirming the SMSF is an ATO regulated
super fund. Further, you need to provide your Super Fund’s
electronic address, BSB Code and account number. If you do
not provide the full information regarding your superannuation,
then your contributions will default to GESB, the Commission’s
default fund.
