2019 Client Agreement
I, _____________________________________________________________ (First and Last name of
pet owner), hereafter referred to as “Pet Parent”, do hereby entrust Kokoro Pet Resort, LLC and/or
Kokoro Veterinary Center, LLC, hereafter jointly referred to as “Kokoro”, to provide boarding, daycare,
training, grooming and/or veterinary services for my pet, __________________________ (Pet’s name),
hereafter referred to as “Pet”.

______ I request that my pet be accepted into the Kokoro doggie daycare, and/or boarding, and/or
additional activities, such as, but not limited to training, grooming, etc, hereafter referred to as
“Program”. In accepting my pet, I acknowledge that Kokoro will be relying on my understanding and
agreement with the provisions of this “Client Agreement”. Kokoro has relied on my representation that
my pet is in good health, has not harmed or shown aggressive or threatening behavior towards any
person or any other animal, unless stated otherwise on the “Pet Information and Enrollment Form”. I
understand and agree that this “Client Agreement” contains a limitation of liability clause.
______ In case of emergency or my pet’s need for emergent medical attention, I understand that an
attempt to contact me will be made as soon as is possible. I understand that my pet may be provided
first aid, stabilization, and/or pain medication prior to my being contacted. I authorize the staff and
agents of Kokoro to take appropriate action, in their sole discretion, for the care of my pet. I understand
and authorize that this action may include, but is not limited to, first aid or treatment for stabilization
and/or pain control or other medical attention as is indicated, transport to and/or care at another Facility
or necessary location (within 10 miles). I recognize the risk of injury that may accompany said transport
and/or care and authorize such transport and/or care without liability to Kokoro, Kokoro staff and
agents. I accept any and all conditions, rules and regulations promulgated by Kokoro Veterinary
Center, LLC or another caring Facility associated with the activities, use of the Facility and transport,
and hereby agree to comply with them. I understand and agree that upon entry or transfer to Kokoro
Veterinary Center, LLC and/or another Facility/Veterinary Provider, I will be relying on said Veterinary
Provider for the care of my pet and assume full financial responsibility for payment or expenses
incurred under said Veterinary Provider for any and all treatment given. I agree to pay the transport fee
of $55 if my pet needs to be transported to another Facility.
1/6______ Non-emergent medical care will NOT be provided without my consent. I understand that
Kokoro Pet Resort, LLC staff gives all pets showing signs of illness or involved in any type of incident a
cursory examination, however, Kokoro Pet Resort, LLC staff members are not veterinarians and will not
be liable for the location, treatment or diagnosis of any injuries incurred or illness. In the event of an
incident and/or identification of a non-emergent medical concern, I understand that an attempt to
contact and notify me will be made. I will be given the option, at my discretion and cost, to see a
licensed veterinarian with Kokoro Veterinary Center, LLC or to seek attention, from a licensed
veterinarian of my choosing. I agree that I will be relying on said veterinary provider for the medical care
of my pet and assume full financial responsibility for payment or expenses incurred under said
veterinary provider for any and all treatment given. If I request that my pet be transported to another
Facility for veterinary care, agree to pay the transport fee of $55.
______ I understand that every pet reacts differently and animals by nature are unpredictable and
therefore present a certain level of risk. I acknowledge and understand that there are certain risks
involved in participating in Kokoro Program(s) and in owning, training, caring for, handling and being in
the presence of animals, including, but not limited to; damage or injury to people, property and / or
animals, fights, bites to humans or other pets and the transmission of disease.
______ I understand that some pets, especially those not familiar with Kokoro, may experience
separation anxiety when apart from their human companions. I authorize Kokoro Veterinary Center,
LLC to treat my pet for separation anxiety utilizing appropriate behavioral and / or medical treatment, if
necessary, for my pet. I agree to pay for all veterinary fees that apply.
______ I understand that pets not regularly socialized do not necessarily know how to behave politely
with other pets. These pets are at higher risk of incidents including, but not limited to, bites, fights, fear
aggression, object guarding, and behavior problems and/or acting out. For the safety of my pet and
other pets, at Kokoro staff’s sole discretion, the staff may limit social interactions, as needed. Any
behavior deemed dangerous or inappropriate by Kokoro staff may result in dismissal of my pet from
Kokoro and its Program(s). Any determination by Kokoro management personnel, at their sole
discretion, will be final and have authority.
______ I understand that I am solely responsible for any physical harm or property damage caused by
my pet while my pet is at Kokoro.
______ I understand that pets not regularly exposed to the level of activity they experience at Kokoro,
may feel discomfort of sore muscles, sore joints and / or fatigue. Some pets may sleep excessively for
1 - 3 days upon returning home. Pets not regularly exposed to physical activity or play on hard surfaces
may experience sore paws, blisters, bruises or abrasions on the feet. In extreme cases, pets may
experience weight loss from increased activity.
______Water is available at all times. I acknowledge that my pet may be thirsty after attending Kokoro.
The reason for this is that some pets drink less while in a boarding or daycare environment. Upon pick
2/6up / discharge, I will be aware of my pet’s water intake as excessive amounts may cause a vomiting
and / or other problems. I will be careful to not allow my pet to drink a large volume of water at one time
and am aware that should my pet do this, my pet may vomit the water back up shortly after drinking it. I
also acknowledge that some pets drink more than usual while at Kokoro and thus will urinate more than
usual in the 12 - 24 hours after leaving Kokoro.
______ I understand that pets over 6 months of age that are not spayed or neutered will not be
permitted to socialize with pets outside of those with which they live (if they live with other pets). This is
for the safety of my pet and others.
Raw meat-based diets (RMBD) can be homemade (for example, the BARF Diet) or commercially
made. Commercial RMBD are typically frozen or freeze-dried, but some are available as fresh
refrigerated products and others can even look like commercial dry food (dry food with a raw coating).
This includes both freeze dried raw foods and treats. In addition to the nutritional imbalances of these
diets, raw meat products are likely to carry bacteria (like any raw meat products we encounter at home
or in restaurants). These diets pose a health risk, not only to the individual animal eating them, but also
to other animals and people exposed to the animals eating them. Pets fed raw foods are a unique
source of “hospital acquired infection-associated pathogens” in the hospital (or pet resort) environment
because they may shed organisms (often asymptomatically) that can cause illness in humans or other
pets. Patients fed raw meat products within the past 30 days likely pose the greatest risk. R eferences
provided upon request.
______ I attest that my pet has NOT been fed a raw meat based diet or treat within the last 30 days.
______ My pet has been fed a raw meat based diet or treat within the last 30 days. I will notify Kokoro
of this on my pet’s “Pet Information and Enrolment Form.” I understand that my pet will be kept in
isolation and will not be fed a raw meat diet while at Kokoro. I understand that this is for the safety of
other animals and staff members. I understand that my pet will be fed “house food” (Royal Canin brand)
at the applicable additional fee. I understand that an additional Isolation fee of $80.00 per 24 hour
period, or any portion thereof will be incurred.
______ I will provide an adequate amount of my pet’s food during its stay and will divide individual
meals or portions in separate containers. Sudden changes in food and/or food quality may cause
vomiting, diarrhea and /or colitis. Should my pet run out of food while at Kokoro, I authorize Kokoro to
feed my pet “house food” (Royal Canin brand) at the applicable additional fee. In the event my pet has
a food allergy, and specific dietary requirements, I will state this on the “Pet Information and Enrollment
Form”. In the event that my food allergic pet runs out of food, I agree to pay for my pet’s required food
plus a special service fee.
______ Pets at play during daycare or boarding or involved in other Program(s) get dirty. Should
Kokoro staff, at their sole discretion, deem it necessary to bathe my pet, while in their care, for sanitary
or health reasons, I agree to authorize this action and accept responsibility for payment of such service.
3/6Dogs that board for 3 or more days, are given a mandatory bath on their last day of boarding. I agree to
pay for this bath.
______ Pets with longer hair can get matted from the high level of activity. Baths and brush outs can be
requested and scheduled at pet parent’s expense.
______ Excessively long toenails may cause harm or injury. I will be notified if my dog’s nails are too
long. If, at my next visit, the nails have not been trimmed, Kokoro will trim them at my expense.
______ If my pet is on medication, I agree to provide the medication and my pet’s preferred vehicle for
delivery (i.e., cheese, peanut butter, marshmallows, pill pockets, etc) and pay the additional fee for
medicating my pet while it is at Kokoro during daycare or boarding. I will keep my pet’s “Pet Information
and Enrollment Form” up to date if there are medication or dosing changes.
______ I hereby attest that my pet is on year round flea and tick prevention and a product that is on
Kokoro Flea Preventative Approved Product List is being utilized.
______ If the Kokoro staff determines that my pet has fleas, I authorize Kokoro staff to give my pet one
dose of one of the following products (which provide 1 month of treatment) and pay for the applicable
additional fees. If my pet has any history of seizures, I will note it on the Pet Enrollment and Information
Form and I understand that the topical will be used, if needed. If my pet has a food allergy, I will note it
on the Pet Enrollment and Information Form and I understand that the topical will be used, if needed.
● Canine
○ Credelio (oral for dogs > 4.4 pounds and 8 weeks of age and older)
○ Advantage Multi for Dogs (topical solution for dogs over 7 weeks and 3 lbs)
● Feline
○ Comfortis (oral for cats > 4.1 pounds and 14 weeks of age or older)
○ Advantage Multi for Cats (topical solution for cats over 9 weeks and 2 lbs)
______ I hereby assert that my pet is up to date on vaccinations in accordance with Kokoro’s
Vaccination Policy, which was developed based on the American Animal Hospital Association, World
Small Animal Veterinary Association Vaccination Guidelines and/or American Association of Feline
Practitioners Vaccination Guidelines. I understand that if my pet is not vaccinated, there are significant
risks to my pet of contracting disease (or illness) by entering the Program(s). In addition, I recognize
that my pet could transmit disease or illness to other pets, even if it is not showing signs of illness,
because it could be an asymptomatic carrier. Vaccination documentation must be provided by a
veterinarian. I will be charged a “special needs fee” if it is necessary for Kokoro staff to secure
documentation and verification.
______ I hereby attest that my pet has had negative direct fecal (screening for clostridium and
campylobacter), fecal float (parasite screen) and negative giardia test within the last 6 months. In the
event my pet has tested positive for one of the aforementioned, my pet must complete a full course of
4/6treatment before returning, based on the Kokoro Intestinal Parasite Protocol. Test result and, if
applicable, treatment documentation must be provided by a veterinarian. I will be charged a “special
needs fee” if it is necessary for Kokoro staff to secure documentation and verification.
______ I hereby attest that my pet is on year round heartworm prevention. Documentation must be
provided by a veterinarian. I will be charged a “special needs fee” if it is necessary for Kokoro staff to
secure documentation and verification.
______I understand that Kokoro Pet Resort, LLC is staffed 24 hours a day, 7 days a week. Daily
reception hours will be limited from 11:00 pm to 6:00 am and as posted on holidays* so that all staff
members present can focus on the care of the pets. However, for my convenience, reception by
appointment for drop off and / or pick up will be available for an additional fee of $25.
______Holiday* boarding reservations require a non-refundable $40 deposit. This will be applied to
your boarding fees. Holiday* reservations cancelled less than 14 days prior to the holiday* will result in
charges in full. Extending stay beyond your holiday* scheduled check out date will result in an
additional $25 per night charge, in addition to the applicable boarding rate.

______ I have reviewed the pricing structure on the Kokoro.pet website and / or have discussed it with
Kokoro staff. Discounted packages are provided only if they are paid in advance. Pre-paid packages
are non-refundable. Published fees and charges are subject to change without notice. I must pay for all
services in full, at the time of discharge / check out. Applicable fees and charges may include, but are
not limited to, daycare, boarding, grooming, baths, nails, feeding, medication administration, veterinary,
transportation, other ala carte services, etc. I understand any unpaid fees, by me, will be sent to
collections, and I will be responsible for all collections and legal fees incurred by such actions taken.
5/6______The safety and health of all pets in Kokoro’s care is Kokoro’s main priority. To help Kokoro
provide a safe environment for all pets, I agree to maintain a complete, up-to-date “Pet Information and
Enrollment Form” on file, at all times. If my pet has not been here for 6 months or more, I understand
and agree that a new assessment and “Pet Information and Enrollment Form” is required.
______ I do NOT authorize Kokoro to release my pet to an unauthorized party. All authorized parties
must be listed in writing and submitted to Kokoro prior to any arrangements made for pick up.
______ Photographs, videotape or digital recordings are taken of the facility, pets, customers and staff
on a daily regular basis for, among other things, use in advertising by Kokoro. I acknowledge that all
such images (electronic, negatives and positives), together with prints and the copyrights therein, are
the property of Kokoro. I give Kokoro and persons authorized by Kokoro my consent, permission and
authorization, without compensation to me, to use, reproduce and alter the images, in print or electronic
format (including on the internet), either alone or in combination with other images, text and graphics. I
waive my right to approve the finished photograph, advertising copy, print material or electronic files
that may be used in conjunction with the images. I represent that I am at least 18 years of age and
acknowledge that the rights granted in this paragraph are irrevocable worldwide, and perpetual and are
binding on the heirs, successors and assigns.
______I agree that in consideration of Kokoro accepting my pet for boarding, daycare, or
hospitalization during hurricane season, this contract HEREBY RELEASES, satisfies, and forever
discharges Kokoro from any and all manner actions, causes of actions, suits, covenants, promises,
damages, claims and demands whatsoever, in law or equity, which the undersigned pet parent or which
any personal representative, successor, heir or assign of said undersigned pet parent, hereafter can,
shall or may have, against Kokoro, for upon or by reason of any matter, cause or thing in any manner
whatsoever relating, regarding or concerning all natural disasters. Kokoro makes no representation
regarding the structure of the building or the elevation of the property upon which Kokoro is located, nor
do they represent that there will be any person on the premises during any period of time that
evacuation of the area has been advised or recommended.
______ Waiver and Release of Liability: Animal behavior can be unpredictable and participation in
Kokoro training, education, obedience classes, day care, transportation, boarding or other programs
and activities is not a guarantee of your or your pet’s behavior or safety. By signing this form I
acknowledge that I, and anyone acting on my behalf, understand the risks associated with pets and
their activities. I hereby release and hold harmless Kokoro, its owners, employees, associates,
contractors, heirs and assigns from any and all claims of any kind including but not limited to: bodily
injury to you, other persons, animals, or property, whether such illness, injury or damage occurs during
or after any activity, transporting or at any time in the future. Kokoro shall not be liable in contract or tort
(including negligence and strict liability) for any indirect, special, incidental or consequential damages
arising out of or in connection with its performance, non-performance or otherwise.
