INTRODUCTION
This document contains answers to frequently asked questions (FAQs) related to the 2019-20 Community
Accountability Planning Submission (CAPS), MSAA Indicators and Target Setting.

GLOSSARY OF TERMS
CAPS: Community Accountability Planning Submission. The CAPS is the planning tool used by community-based
health service providers to inform the negotiation of the Multi Sector Service Accountability Agreement (MSAA).

MSAA: Multi Sector Service Accountability Agreement. The MSAA is the service accountability agreement that the
LHINs are required to enter into with the community-based HSPs pursuant to the terms of the Local Health System
Integration Act (LHSIA).

SRI: Self Reporting Initiative. SRI is the self-reporting solution for submission and review of information between
Health Service Providers (HSPs) and the Local Health Integration Networks (LHINs) and the Ministry of Health and
Long-Term Care (the Ministry). It includes any hardware or software that may be provided to the User for the
purpose of using SRI.

1. CAPS

1.1    Was the new community paramedicine functional center added to this year’s CAPS?
      A:       Yes it has been added to the CAPS.

1.2    Since the CAPS Narrative is uploaded to the Self Reporting Initiative (SRI), will a password be
       required to open the document?
      A: A: Toronto Central LHIN health service providers (HSPs) do *not* need to complete the CAPS
               narrative. Instead, we will be leveraging the HSP Supplemental which our HSPs already complete.
               The HSP Supplemental can be completed through www.HSP360.camh.ca. For questions on how
               to complete the survey, please contact Ting.Lim@tc.lhins.on.ca

1.3    Can the CAPS demo link be shared with any health service provider (HSP)?
      A:       Yes, it’s designed for that. The Q2 link in the Q2 completion guide can also be shared. The only link
               that the LHINs should not share with the HSPs is the link for the LHIN set-up of the CAPS
               Narrative.

1.4   The TPBE selected on the Service Selection screen does not carry over to the Act_Summary. (Affects
      both the CAPS and Q2 reporting) What is the work around?

      TPBE’s selected on the Service Selection Page do not carry over to the Act_Sumary page resulting in an “Error” in
      Column C. This is only applicable to the new functional centres added for this round as follows:

      A:       The workaround is as follows:

•    Providers should continue to select the Functional Centre (with an “x”) and the TPBE on the Service Selection
           page.
      •    On the Act_Summary Page click on the “HSP Specific- All Categories” Button.
      •    On the Keyboard press CTRL+SHIFT+U to unprotect the worksheet.
      •    Find all “Errors” in Column B and manually replace them with the correct TPBE for each functional Centre.

1.5   The plan columns for 2020-21 and 2021-22 are protected. (Affects only the CAPS reporting). What is
      the workaround?

      The columns for years beyond the current planning period of 2019-20 have formulas that carry forward the plans from
      2019-20. Providers wishing to adjust plans beyond 2019-20 cannot do so.

      A: The workaround is as follows:

      •    On the Keyboard press CTRL+SHIFT+U to unprotect the worksheet they wish to adjust and proceed to type over
           any of the numbers in the 2020-21 and 2021-22 columns.

1.6   On the Fin_Summary, changes to the 2020-21 and 2021-22 columns on the TPBE pages are not
      reflected. (Affects only the CAPS reporting) What is the workaround?

      The formulas on the Fin_Summary for 2020-21 and 2021-22 are referencing the 2019-20 column and not adding up
      the cells from the individual program sheets.

      A: The workaround is as follows:

      •    None. This requires that formulas are added to this column and is most likely beyond the capabilities of most
           providers
      •    For providers not changing the “out years” this issue will have no effect

      Note: The EForms will correctly reflect the totals from the individual worksheets. LHINs should review the
      individual TPBE worksheets which will correctly reflect the financials for their providers

1.7   The following error message occurs for CAPS reporting: #REF! Error in Cell I60 (Affects only the CAPS
      reporting). What is the workaround?

      On the financial pages, Cell I60 is returning a “#REF!” error. (Change from prior budget- Total Expenses (all funds))

      A: The workaround is as follows:

      •    On the Keyboard press CTRL+SHIFT+U to unprotect the worksheet.
      •    Copy the cell directly above the error (I59) to I 60

          Act_Summary Formulas 1
          On the Act_Summary page, the summary formulas at the bottom are referencing the wrong column. (Change from
          prior budget- Total Cost for all F/C)

          A: The workaround is as follows:

          •   Click on the Show All button at the top of the page.
          •   On the Keyboard press CTRL+SHIFT+U to unprotect the worksheet.
          •   Select the Range from K3161 to K3174 and Copy.
          •   Select the Range from K3175 and Paste

          Act_Summary Formulas 2
          On the Act_Summary page, the formula in cell K355 is referencing the wrong column.

          A: The workaround is as follows:

          •   Click on the Show All button at the top of the page.
          •   On the Keyboard press CTRL+SHIFT+U to unprotect the worksheet.
          •   Select the Range from K50 and Copy.
          •   Select the Range from K35 and Paste

1.9       The CAPS narrative cannot be uploaded to the SRI system. What is the workaround?

       A: Toronto Central LHIN health service providers (HSPs) do *not* need to complete the CAPS
          narrative. Instead, we will be leveraging the HSP Supplemental which our HSPs already
          complete. The HSP Supplemental can be completed through www.HSP360.camh.ca. For
          questions on how to complete the survey, please contact Ting.Lim@tc.lhins.on.ca

1.95      The formula in H60 on the CAPS form is causing an edit Check to fail?

          A: The workaround is as follows:

          •   This requires a fix in future versions. Submit the CAPS with this edit showing failed.

2.0       Toronto Central LHIN 2019-20 Webinar Q&A
Q: Will we receive a document that summarized the changes from the previous MSAA?
   A: Yes, you will receive a document summarizing the changes from the previous MSAA to the new
       three year agreement. This document will be made available- likely in January 2019.

Q: Do we need to do a CAPS narrative for Toronto Central LHIN for 2019/20?
   A: No. Toronto Central LHIN expects that our health service providers (HSPs) will complete the HSP
      Supplemental instead of completing the CAPS narrative. The HSP Supplemental can be
      completed through www.HSP360.camh.ca. For questions on how to complete the survey, please
      contact Ting.Lim@tc.lhins.on.ca

Q: For the LHIN CAPS, do we need to report the planning for LHIN programs and services only, or do
   we need to include all Fund Type 2 programs and services? I.e., both MOHLTC and LHIN.
   A: You need to report all programs and services for Fund Type 2.

Q: Do Long-Term Care Homes have any requirements under the MSAA or CAPS?
   A: No, the Long-Term Care Homes hold a separate agreement with Toronto Central LHIN- the Long-
       Term Care Service Accountability Agreement.

Q: Could you describe what you mean by rate harmonization by giving an example?
   A: Most LHINs in the past used a targeted funding approach, whereby an agency provided a
      proposal to the LHIN and that program was funded based on the respective negotiated
      agreement. Over time, this created funding inequities between programs providing similar
      services that were funded at different points in time. Community rate harmonization will move
      the community sector away from a targeted funding model toward a capitation model to fund
      agencies on a consistent rate X volume approach. Additional details will be presented in the
      near future, and your respective Performance Management Consultant will be happy to provide
      answers to questions as appropriate.

Q: What is the timeline for Rate Harmonization engagement?
   A: Rate Harmonization is a multi-year initiative. The first phase involves a select number of
      Functional Centres, and all impacted agencies will be notified to inform and support CAPS
      submissions.

Q: Can you confirm the Functional Centres being reviewed for Rate Harmonization? Does it include Case
Management?

    A: The Toronto Central LHIN intends to move forward with a specific number of Functional Centres
    in this initial phase. The Functional Centres in this phase are being chosen to ensure the least
    possible disruption to the system as a whole as we plan the best approach to fully implement Rate
    Harmonization. Over the next couple of years, all Functional Centres will be included. Additional
    targeted communication will be sent to those agencies impacted in 2019/20 (i.e. those agencies
    with functional centres that are being reviewed) and broader communication to all sectors at each
    phase to keep you updated.

Q: Can my Functional Centres be included in Rate Harmonization? Do we have any choice in whether
   a functional centre changes?
   A: Yes- it will apply to all functional centres, however the review will be done in a phased
       approach. As mentioned above, this is a phased initiative and all functional centres will
       eventually be included.

Q: What type of information will be collected from Community Health Centres about non-insured
   clients?
   A: We will be collecting the number of non-insured clients being served by each Community Health
       Centre annually. Shama Umar, Lead for Community Health Centres will reach out directly with
       additional information.

Q: If we want to propose a new type of program that is not Rate x Volume, do we submit an HSIP as
   per past process?
   A: At present, the Toronto Central LHIN is in the early stages of Rate Harmonization. Until further
       notice, please continue to submit proposals for programs using the HSIP format, all the
       information is on our website at http://www.torontocentrallhin.on.ca/forhsps/hsip.aspx

Q: What is the difference between a Functional Centre and an Account Header?
   A: Functional Centres can be considered as individual files within an Account Header.

Q: What are the timelines for Community Services CAPS?
   A: The 2019-2020 fiscal year timelines are in the presentation slide deck and your agency would
      have received an email with this information in early October. To clarify, we are currently in the
      2018-2019 fiscal year and you will be submitting CAPS for the 2019-2020 fiscal year.
   A: Please refer to the timelines outlined in the presentation and the email sent in early October. If
      you wish to be added to our email distribution list, please contact Victoria Williams, Toronto
      Central LHIN Performance Analyst, Victoria.Williams@tc.lhins.on.ca.

Q: 2018-2019 CAPS submissions were due November 23rd, 2017. Is there any consideration to align
   CAPS submissions to that date going forward?
   A: Submission due dates for 2019-2020 CAPS were chosen to be consistent across all sectors.
      Please note that the November 19, 2018 deadline refers to the first draft CAPS submission, and
      that the board-approved CAPS is due on or before February 15th 2019 once negotiations with
      the LHIN have been finalized by both parties.

Q: Do we need to complete the draft 2019-2020 budget by November 19th, 2018?
   A: Yes, these are to be submitted through SRI and we will be encouraging all HSPs to submit on
      time, as per the Performance Management framework outlined in the webinar.

Q: Who should I connect with if I have questions about Community Services CAPS or SRI?

      additional resources, please review the webinar and resources posted on our website here.
      Some examples of useful information on our website are: Health Data Branch contacts for
      Ontario Healthcare Reporting Standard (OHRS) guidelines, workarounds for formula that are not
      working on SRI, etc. For questions relating to the appropriateness of your current targets or, if
      you wish to make changes to service delivery or target population, please contact your

Q: Four Functional Centres were removed for 2019-2020; does this mean we can no longer use them?
   A: Correct, you can no longer use those Functional Centres. New Functional Centres have been
      created to be used for the services previously captured by those Functional Centres.
      Additionally, you may contact Health Data directly at the contact information in the
      presentation as a resource to confirm questions related to the Ontario Healthcare Reporting
      Standards.

Q: Can I call Nello with questions related to Community Services?
   A: Absolutely; please feel free to contact your respective Performance Management Consultant as
      needed.

Q: Should we include money for programs that were approved but had funds frozen due to the
   change in government?
   A: Please include service volumes and planning for the funds you have in your base allocation.

Q: Is the 2% increase included in the 2019-2020 planning?
   A: For those HSPs that received funding letters outlining any increases to base funding, this funding
       and associated volumes should be included in your planning submission as CAPS submissions
       include base funding.

2. MSAA Indicators and Target Setting

2.1   Are all the functional centers captured in the Service Activity by Functional Center indicator? Do
      LHINs need to calculate the indicator or are LHINs sent a report?

      A:      This indicator is a core indicator for all sectors. It is referenced in the Performance Schedule E2A,
              Clinical Activity – Details, that is pulled from the CAPS. While it is identified as a single indicator in
              the schedule, it is broken out based on the information the provider submits in their CAPS. The
              schedule will populate automatically from CAPS and then on your SRI quarterly reports.

2.2   For CHC Performance and Explanatory indicators, is there a way for LHINs to extract the data from
      the Alliance for Healthier Communities database (BIRT) or do LHINs need CHCs to report it manually?

      A:      The typical process has been for CHCs to input the data. The Work Group is not aware of a
              process to retrieve the data by extracting from BIRT, but will review this possibility for the
              2020/21 SAA.

2.3   Which of the new categories would actually appear in the SAA agreements?

      A:      Performance, explanatory and monitoring indicators will appear in the relevant SAA agreements
              once approved by the LHIN CEOs. Note that only the performance indicators have a target
              associated with them and result in the HSP having an obligation to achieve results.

2.4   Is the target setting methodology the same for the CHC performance indicators?

      A:   Yes.

3. Q2 2018-19 Reporting

3.1   I have made a budget adjustment to the Act_Summary page to capture the CAPS budgets in the new
      Functional Centres introduced this year. The Edit on row 18 in my Q2 report now fails.

      A:      This is an exception to the Edit rule. Enter a comment on the comments page in your Q2 report to
              explain that you have updated the budgets in the new Functional Centres
