Notice Inviting Quotations
Sealed quotations are invited from the interested parties for preparation of Tableau for
26th January, 2019 Republic Day Parade at MAM Stadium, Jammu. The theme of the
Tableau shall be “Ayushman Bharat”. The interested parties are requested to submit the
sealed quotations, affixed with Rs.5.00 revenue stamp and clearly marked “Quotations
for Preparation of Tableau for Republic Day Parade - 26th January, 2019”, at
Regional Institute of Health & Family Welfare, Near Sainik School, Nagrota. The sealed
tenders shall reach this office by or before 14th December, 2018 (latest by 2.00 PM) and
will be opened in the presence of bidders on the same day at 3.00 PM. The acceptance
and finalization of tenders will be subject to following conditions:
Terms and conditions:
1. The rate quoted should include all the cost for preparation of Tableau including
preparation of script for the theme, making CD/ Tape & pen drive, cost of song
including cost of artists, studio cost etc. and refreshment charges for the participants,
hiring of tralla, display of cut-outs on ply boards of size 6 mm paintings of slogans
and messages, art work framing around tralla is approximately 40 feet long and 10 to
15 feet wide including decoration of Tableau (complete job).
2. A proposal can contain more than one design and sketches of the designs as seen
from different angles.
3. The proposal will include all aspects of fabricating and setting up theme based
exhibition and Tableau.
4. Agencies should have sufficient experience of conceptualization of theme,
fabrication & display of Republic Day Tableau at the National level or State level or
District level (copy substantiating the claim should be enclosed).
5. The job would also encompass providing of suitable/appropriate lyrics, composing
of music, arrangements of artists and actual pre-recording of the music for use in the
Tableau.
6. The project would also include any other work that may have direct or indirect
bearing with the fabrication and presentation of the Tableau and may facilitate the
project in any manner.
7. The size of artwork should not be less than 4 mm plywood, wooden fixture of size
(50 x 50) mm and (40 x 40) mm wood as required at Tableau.8. Besides a narrative explanation, the treatment should contain drawings and designs
explaining the manners in which the theme can be presented.
9. The group/ troupes of artists should be registered with Song & Drama Division, J&K
State.
10. Copyright of the work shall vest with the National Health Mission J&K State and it
would have rights on the awards won, if any. Attested copies of certificates/ work-
orders for any theme based, fabrication showing experience or conceptualization of
theme, fabrication and display either of Republic Day Tableau or at National Level
or State Level or District Level.
11. All the documents and pages of the proposal document should be signed by the
authorised signatory.
12. The tender shall accompany CDR/ FDR of Rs.5,000/- (Rupees Five Thousand only)
pledged to the FA/ Chief Accounts Officer, National Health Mission, J&K which
shall be released to successful tenderers after completion of work.
13. The Tableau should be ready by 20th January, 2019.
14. Full dress rehearsal by the artists/ participants shall be done from 22nd to 24th
January, 2019 at MAM Stadium, Jammu before final presentation on 26th January,
2019.
15. The full payment shall be made after successful display of Tableau on 26th January
2019 Republic Day Parade at MAM Stadium, Jammu.
16. State Health Society will have the right to modify, change, accept or reject the work
in totality or any part thereof or cancel the project at any stage without assigning any
reason whatsoever.
17. State Health Society, National Health Mission, J&K reserves the right to accept or
reject any offer without assigning any reason.
18. The decision of Mission Director, National Health Mission, J&K about the quality of
services will be final and shall not be challenged by the successful vendor/ agency on
any ground whatsoever.
19. Tender Form can be collected from the State Health Society, NHM, J&K after
depositing the Tender Fees (Non-Refundable) in the shape of Bank Draft of
Rs.1,000/- (Rupees One Thousand only) in favour of ‘Non-NHM Funds at SHS
Level’ and payable at Jammu. Alternatively, the same can be downloaded from the
website www.jknhm.com. In such case, quotation shall also contain the Bank Draft
of Rs.1,000/- (Rupees One Thousand only) in favour of ‘Non-NHM Funds at SHS
Level’ and payable at Jammu, failing which quotation(s) shall not be accepted.
20. In case of any change in venue of Republic Day Parade, as per instructions from the
Government, successful vendor/ agency will not be entitled for any additional rate/
compensation.
21. Quotation(s), properly sealed and clearly mentioning “Quotation for Preparation
of Tableau for Republic Day Parade - 26th January, 2019”, alongwith following
documents shall reach to the office of State Health Society, NHM, J&K at RegionalInstitute of Health & Family Welfare, Near Sainik School, Nagrota by or before 14th
December, 2018 (latest by 2.00 PM):
a) Quotation Form – Technical Details & Financial;
b) Bank Draft of Rs.1,000/- (Rupees One Thousand only) in favour of ‘Non-NHM
Funds at SHS Level’ and payable at Jammu in case the form is downloaded from
the website www.jknhm.com;
c) CDR/ FDR of Rs.5,000/- (Rupees Five Thousand only) pledged to the FA/ Chief
Accounts Officer, National Health Mission, J&K;
d) Copies of GST Registration Certificate;
e) Copy of PAN card alongwith latest ITR;
f) Work Experience Certificates; and
g) Design/ Drawings/ Detailed Proposal
