Ethnic/Cultural Ethnicity Our ethnic background describes how we think of ourselves. This may be based on many things, including, for example, our skin colour, language, culture, ancestry or family history. Ethnic background is not the same as nationality or country of birth. The Department of Children, Schools and Families recommends that those with parental responsibility decide the ethnic background for primary pupils. The Information Commissioner (formerly the Data Protection Registry) recommends that young people aged over 11 years old have the opportunity to decide their own ethnic identity. Parents, or those with parental responsibility are asked to support or advise those children aged over 11 in making this decision, wherever necessary. Pupils aged 16 or over can make this decision for themselves. Please study the list below and tick one box only to indicate the ethnic background of the pupil or child named above. Please also tick whether the form was filled in my a parent or the pupil. Child’s Language Background – PLEASE FILL IN CAREFULLY – YOUR ANSWER MIGHT NOT BE AS OBVIOUS AS YOU THINK !! Many children have only ever had experience of hearing English as a spoken language in the family home. However, many others will have heard another language spoken within the family setting during their childhood. Please ring any such language below, but only one language, even if the child only experienced hearing it in early childhood and has English as their native and fluent language. If your child heard several such languages during their childhood, ring the language other than English to which they had most exposure. Note that declaring an “additional language” has no bearing on how your child is viewed or setted etc: but it is vital for us to get a good overall picture of the background of our student body. I ST GEORGE’S SCHOOL Miss Helen Barton MA Headmistress This paperwork is designed to be compliant with the GDPR and Protection of Freedoms Act. INFORMATION FOR STUDENTS AND PARENTS OF STUDENTS ATTENDING ST GEORGE’S SCHOOL 2021-22 Occasionally, we may take photographs of the children at our school. We may use these images in our school prospectus or in other printed publications that we produce, as well as on our website and social media platforms. We will not use the personal details or full names (which means first name and surname together) of any child in a photographic image on video, on our website, in our school prospectus or in any of our other openly available publications. We may use group or class photographs or footage with very general labels, such as “a Science class” or “the 1st Lacrosse XII”. We will not include personal email or postal addresses, or telephone or fax numbers on video, on our website, in our school prospectus or in other openly available publications. In general, we will be providing a much higher level of care in what we post than students sadly largely apply to themselves! You will understand that with social media a part and parcel of modern life, we do not want to miss out on the opportunities these platforms can give to share positive news and celebrate achievement of all To comply with the General Data Protection Requirement, we need your permission before we can photograph or make any recordings of your child apart from any needed for the fundamental functioning of the school as an educational establishment (for instance so that staff can have photo class lists to learn names, so that students can have photo id badges in the 6th form, and so on). You will understand that it would be very helpful to the school if ALL parents/students were to grant the level of permission we are requesting – otherwise, logging and tracking which children can and cannot be photographed will pose a considerable challenge. Further details of how we process imagery, who has access to it and control of its use, etc, can be found in the school privacy notice and the information security policy. Note that you can in law amend any consent given below at any time and without giving your reason. If you are minded to withhold consent, please be aware that a call to the schools Data Protection Officer (DPO) may allow us to reassure you and allay concerns. From time to time, our school may be visited by the media who will take photographs or film footage of a visiting dignitary or other high profile event. Pupils will sometimes appear in these images, which may appear in local or national newspapers, or on televised news programmes or internet platforms. In such cases we will always seek separate permission from students (aged 13 or over) or parents (up to age 13) regarding what identifiable data can be given.
Name of STUDENT (Block capitals) Date of Birth House Name of Parent/Guardian Having read the statement above, do you give your consent for photographs or other images to be taken and used? (please tick the appropriate box) YES, I give my consent for pictures to be taken and used. This includes the following media or platforms (PLEASE TICK ALL OF THOSE YOU ARE AUTHORISING): The School website The school prospectus and similar materials The School Facebook page Twitter accounts controlled by the Head and other staff in their professional role Instagram accounts controlled by the Head and other staff in their professional role. **Should you decide not to give permission for some platforms, it is easier for the school to decline permission for all photos** NO, I do not give my consent for pictures to be taken and used beyond those narrowly needed for the school’s core functions. If withholding permission on behalf of a child under 13, please make your child aware of the limits you are setting, and of the situations they may need to step back from to help us avoid a breach e.g. when team or music group or drama cast pictures are done, or group photos on an educational visit. Biometrics I understand that St George’s school will capture and retain the biometric data of such students who choose to use school paid catering and library facilities, and that this is done by converting a finger scan into a numerical form. This system takes measurements of your child’s finger and converts these measurements into a template (a string of numbers), which is then encrypted. The data held cannot be used to recreate a fingerprint image, nor can it be used in a forensic investigation. An image of your child’s fingerprint is not stored. The template is used to permit your child to access to the catering and library services by placing their finger on a scanner at the point of sale or service. (Please note that this registration also applies to boarders even though meals are included in termly fees). Under the GDPR and Protection of Freedoms Act I tick YES to confirm the school has my child’s and my permission to capture and retain this biometric data for this use Signature of Parent/Guardian (this is the substantive signature until the child reaches the age of 13) Signature of Student (this is the substantive signature once the child reaches the age of 13) Date
