Re: Quotation for purchase & supply of new fire equipments/AMC of fire equipments.

Dear Sir/Madam,

We intend to enter into rate contract for purchase and supply of fire extinguishers for Office/ Guest
Houses and their AMC. Kindly quote your rates for new fire equipments/AMC of fire equipments for our
Divisional Office in the enclosed form. The rates should be F.O.R. destination. GST should be shown
separately. The other terms & conditions for AMC will be as follows:

For new purchase

    1. The Fire Extinguishers will be supplied/installed by the vendor at our offices/guest houses (at no
       extra cost/F.O.R Destination) within one week from the date of placing the order.

    2. In the event of delayed delivery /installation of the fire extinguishers after the stipulated period,
       the vendor shall be liable to pay penalty at a percentage of the total order value subject to
       minimum of Rs.500.00 and maximum of 5% as detailed below. The first week will start after
       completion of 7 days from the date of order:

         a.       @ 1% for the first week
         b.       @ 2% for the second week
         c.       @ 5% for the third week & above

         For calculating the delay as above, part of the week will be considered as full week. Delay in the
         delivery obligations may attract any or all of the following action:

         i.       Forfeiture of deposits
         ii.      Imposition of penalty
         iii.     Termination of contract.

    3. The New Fire Extinguishers will be warranted for a period of at least one year from the date of
       purchase/installation.

    4. The above rate contract initially valid for one year which can be extended upto further one year
       on two occasions at the sole discretion of LIC on same rates, terms & conditions.
    5. On every new purchase 90% amount will be released and balance 10 % will be released after
       completion of 1 Year.
For Refilling/AMC

   1) Initially the AMC will be for one year which can be extended upto further one year on
      two occasions at the sole discretion of LIC on same rates, terms & conditions. The rates
      include yearly servicing and maintenance of Fire extinguishers including repairs
      and replacing any defective parts if required during contract period. No extra payment
      shall be made on this account. It may be noted that during the contract, the contractor will
      responsible to keep the Fire extinguishers in prefect running including fitting of parts
      accessories, repair etc.

  2) One service in each quarter has to be provided in addition to breakdown/complaint call.

  3) The service provider would attend to breakdown/complaint calls within 48 hours besides
     four quarterly mandatory services.

  4) When fire extinguishers are taken for re-filling, the vendor shall provide equal number &
     similar typeof fire extinguishers in working condition as temporary replacement at no extra
     cost whatsoever till the re-filled fire extinguishers are brought back and installed. If equal
     number & similar type of fireextinguishers are not provided as replacement by the vendor,
     then, penalty @ 1% of the total refillingcost will be charged per week till the fire
     extinguisher(s) taken by the vendor for refilling are returned duly re-filled.

  5) If AMC is not being done by the Vendor other than “Force Majeure Clause” AMC amount
     will not be paid.

  6) Payments will be made after service/refilling on quarterly basis on submission of original
     service reports signed by authorised official of concerned departments along with
     relevant bill.

  7) All refilling of fire equipments should confirm to latest BIS standards.

  8) All cylinders of water CO2 shall have validity for 1 year, CO2 type cylinders for 3 years,
     ABC type cylinders for 5 years, DCP type cylinders for 2 years and Modular ABC Type for 5
     years.

  9) AMC charges should be per Building/ location (list attached).

  10) The vendor shall be responsible for any injury to their workmen during performing the
      servicing/refilling/maintenance work etc.

  11) All Fire Extinguishers must be discharged/emptied before re-filling of fire extinguishers
     checking of nozzles, port hole, vent hole, cap assembly, siphon tube, safety pin/clip,
     discharge pipe etc. In case of breakdown of nozzles , port hole, vent hole, cap assembly,
     siphon tube, safety pin/clip, discharge the vendor has to replace the part at his own cost.

  12) After refilling, paste inspection card to the body of fire extinguishers indicating the serial
     number,date of re-filling, next date of re-filling, due date for hydraulic testing etc.

  13) Ensure that all joints are fully tightened and nozzle, vent hole etc are free of dust/dirt.
14) Operating instructions of fire extinguishers are clearly legible, and facing outward and in
     good visible condition.

  15) The vendor shall repair and refill the fire extinguisher free of cost within warranty period of
      re-fill, if weight loss is noticed in the fire extinguishers which have been refilled.

  16) Tender should be placed in a sealed envelope, super scribed with the name of work –
      “FIRE FIGHTING SYSTEM FOR LIFE INSURANCE CORPORATION OF INDIA”. The last date
      for submission of tenders is 10.02.2021 up to 17.00 hrs.

  17) All the Rates/Amount to be mentioned in words and figures and taxes should be shown
      separately. The tender shall be decided on the basis of total cost involved (New
      Purchase/AMC/Re-filling).

  18) LIC reserves its right to accept or reject any tender without assigning any reasons thereof.
      All tenders in whom any of the prescribed conditions are not fulfilled or are incomplete in
      any respect are liable to be rejected.

  19) In case of ambiguity with regard to conditions and qualification criterion and other related
      matters in the Tenders, if any, the decision of the Competent Authority shall be final &
      binding.

  20) All vendors to submit all pages of tender document duly signed by authorised person
     along with the quotation.

  21) Force Majeure Clause:

      a) The tenderer shall be liable for any delay in execution or failure of their respective
         obligations under this agreement except for delay caused by occurrence of events
         beyond control of the tenderer, including but not limited to natural calamities, fire,
         explosions, floods, power shortages, acts of God, hostility, acts of public enemy, wars,
         riots, strikes, sabotage, order/action or regulations of government , local or other
         public authorities.
      b) In case a Force Majeure situation arises, the tenderer shall immediately notify LIC of
         India in writing of such conditions and the cause thereof within two calendar days and
         prove that such is beyond the control and affect the implementation of the agreement.
      c) Unless otherwise directed by LIC of India in writing, the tenderer shall continue to
         perform its obligations under the contract as far as it is reasonably practical, and shall
         seek all reasonable means for performance not prevented by the Force Majeure event.

Sr.Divisional Manager,

Reg: Permission for Fresh Tender Rate Quotation for AMC/Refilling/New Purchase of
Fire Extinguisher.

We had given Purchase/AMC & re-filling of Fire Extinguisher to M/S BNS Computers Pvt Ltd,
B/O 99-A, Kanchanjanga Appts, Sector 53 Noida (UP) for the period 10.03.2018 to 09.03.2019
vide our letter dated 10.03.2018. The contract was extended for one year from 10.03.2019 to
09.03.2020 and further extended for one year from 10.03.2020 to 09.03.2021.

Now we want to float the fresh tender among our Empanelled vendors and from the vendors of
Rohtak Divison. Kindly allow us to call for the Rate quotation for AMC/Refilling/New Purchase
of Fire extinguisher from these vendors on total cost basis.
