COVID-19 Update
Phase 4 - Effective Monday, February 1, 2021
Region 7 has moved to Phase 4 from Phase 4-Tier 1. IDPH will track various metrics to determine
if we can remain in Phase 4 or if additional mitigations are required. Below is the status of our
facilities during this mitigation portion of phase 4.
Prairie Bluff Golf Course
• Golf course will allow 4 golfers per group with single rider carts or duel riders. Carts will be on
a first come first serve basis. Tee times are in 10 minute increments
• Driving range is open with proper distancing
• Practice areas are open with proper distancing
• The pro shop is open with a limit of 10 people

All registrations must be filled out completely and accompanied by payment in full in order to be processed unless otherwise stated in the
brochure. Do not forget to fill out the waiver form as this will delay the processing of your registration(s).
The Lockport Township Park District is unable to guarantee enrollment and is not responsible for lost or late mail. Additional classes may
be added to accommodate enrollment. The Park District reserves the right to change or alter information printed in this publication such as
policies, fees, times or locations. We try to give as much notice as possible for these changes and apologize for any inconvenience.
Reasonable Accommodation Statement
The Lockport Township Park District is committed to accessibility to all individuals with disabilities and strives to comply with the 1990
Americans with Disabilities Act (ADA) through equal and integrated participation. Please advise the Lockport Township Park District of
any special assistance, accommodations, auxiliary aids or services we can provide for you to participate in our programs, activities, services,

or meeting. If a request is made less than 72 hours before the event the Lockport Township Park District will make a good faith effort to

Registration Policy
Programs will be filled on a first-come, first-served basis. Mail-in and drop-off registrations will be processed after walk-ins. All programs
have a minimum and maximum enrollment. If a class is filled, waiting lists will be formed and every attempt will be made to accommodate
persons on that list. Proof of residency, tax bill or voter ID card along with current driver’s license is required for early registration and
discounted price of programs. Residency must be updated once a year.

NSF Checks
A $30 fee will be charged to persons presenting NSF checks for Lockport Township Park District programs. This fee must be paid at the
administrative office prior to participation in the program.
Program Refund Policy
1. A full refund is granted on any program cancelled by the Lockport Township Park District due to insufficient program registration.
2. A $10 administrative fee will be charged to refund requests.
3. No refunds will be granted after the start of the program except for a medical excuse or relocation out of the area. A doctors note is required
   with the refund request form. The refund will be pro-rated from the time of the request along with a $10 administration fee.
4. All refund requests must be made in person in writing at the DPCC, 1811 Lawrence Ave., or Challenge Fitness, 2021 Lawrence Ave. in
   Lockport.
5. Credit balances of $10 or less will be kept on account.

                     WAIVER, RELEASE OF ALL CLAIMS AND HOLD HARMLESS AGREEMENT FOR THE LOCKPORT TOWNSHIP PARK DISTRICT
Please read this form carefully and be aware that, in signing up and participating in Lockport Township Park District programs, you will be waiving and releasing all claims for
injuries arising out of these programs that you and other named participants might sustain. The terms “I”, “me” and “my” also refer to parents or guardians as well as participants
in the program. In registering for these programs, you are agreeing to the following:
   As a participant in these programs, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of any injuries, damages, or loss
which I may sustain as a result of participating, in any manner, in any and all activities connected with or associated with programs. I further recognize and acknowledge that all
athletic activities involving strenuous exertion or potential body contact are hazardous recreation activities and involve substantial risks of injury.
   I agree to waive and relinquish any and all claims I may have as a result of participating in these programs against the Lockport Township Park District, any and all other partici-
pating or cooperating governmental units, any and all independent contractors, officers, agents, servants, and employees of the governmental bodies and independent contractors,
and any and all other persons and entities, of whatever nature, that might be directly or indirectly liable for any injuries that I might sustain while participating in these programs.
(The parties described in the preceding sentence are referred to as “released parties” in the remainder of this agreement.)
    I do hereby fully release and discharge the Lockport Township Park District and the other released parties from any and all claims for injuries, damages and loss which I may
have or which may accrue to me on account of my participation in these programs.
   I further agree to indemnify, hold harmless and defend the Lockport Township Park District and any and all other released parties, from any and all claims for injuries, damages
and losses sustained by anyone, and arising out of, connected with, or in any way associated with my conduct and the activities of these programs.
I further understand and agree that the terms such as “participation,” “programs,” and “activities” referred to in this Agreement, include all exercises and physical movements of any
nature while I am participating in these programs and further include the provision of or failure to provide proper instructions or supervision, the use and adjustment of any and all
machinery, equipment, and apparatus, and anything related to my use of the services, facilities or premises involved in these programs, and transportation to and from my events.
   I understand the nature of these programs for which I am registering, and have read and fully understand this Waiver, Release and Hold Harmless Agreement. I further understand
that any advisements or warnings of the particular risks of these programs that I subsequently receive will be incorporated by reference into and become a part of this agreement.

                                 ur primary goal is to provide your child with a stimulating environment in
                            which they can grow. This program will give your child a variety of experiences,
                            which will promote social, emotional, intellectual and motor skills. We strive
                            to give each child a positive feeling of self-worth. We believe in and value each
                            child’s uniqueness. The classroom and the many planned activities are designed
to provide an atmosphere where each child can freely express themselves. The children are encouraged to

                                         Musical Theater for Kids! Ages 9-15
StagePlay Productions emphasize the journey toward producing a show from start to finish, while creating a fun environment
to stretch dramatic wings. We will offer two sessions which are listed below with the production titles and descriptions. Our
environment offers a safe program for participants and staff which includes the implementation of social distancing and masks,
and a performance video with virtual singing. The production will be recorded and take place without an audience. The registration

                   The Lockport Township Park District’s before and after school OASIS program is designed to keep the
                   participants active, engaged, entertained and socially involved through a variety of activities with in a safe
                    and supervised setting. Activities such as: free play, sports, games, arts & crafts, snack time, and time for
                     homework is conducted on a daily basis by qualified staff right in the comfort of the children’s home
                      school.

                     Registration information and the parent handbook can be found on our website at www.lockportpark.
              org. Once on the home page, click “OASIS” in the brown box. Complete registration form, emergency form,
    handbook waiver, and EFT (if using monthly payment option) bring with on the day of registration.

  Registration will be taken at Challenge Fitness until Oct. 1, 2020. After Oct.1st, registration will be taken at the Dellwood
  Park Community Center between 8am-4pm. OASIS registration is taken IN-PERSON ONLY.

  Payment Options: 1. Pay in full by cash, check, or charge card.2. Pay in 7 payments. 1st payment due at registration with
  auto payments for the remaining balance. A voided check is required for this option with 6 auto payments deducted the 15th
  of each month Sept.-Feb.

  IMPORTANT: Refunds are given based upon the date that the refund request form is received by the Oasis Supervisor.
  Refunds will not be given based on your child’s last day of attendance. OASIS is not held on school holidays, early
